The purpose of this study was to investigate the influence of psychosocial variables in the prediction of children's pain intensity following surgery. Forty-two children, ages 7 to 17 years (M = 12.26, SD = 3.06), completed an interview 1 week prior to surgery assessing anticipatory distress related to their forthcoming surgery and history of coping strategy use. Following surgery, children reported the intensity of their pain using visual analog scales. Findings demonstrated that the majority of children experienced moderate to severe postoperative pain. Hierarchical multiple regression analyses revealed that psychosocial variables added to the prediction of children's postoperative pain after controlling for the influence of surgery-related and demographic variables. These findings lend initial support for the inclusion of psychosocial assessment measures (e.g., anticipatory surgery distress) in the preoperative assessment of pediatric patients who may be at risk for excessive postsurgical pain. 相似文献

BACKGROUND: Potentially lethal interactions between concomitantly implanted pacemaker and automatic first-generation cardioverter-defibrillator have been reported. We therefore evaluated the reliability and safety of simultaneous implantation of dual-chamber pacemakers with a fourth generation cardioverter-defibrillator incorporating an improved arrhythmia detection system. METHODS: Concomitant implantation of an automatic cardioverter defibrillator and a dual-chamber pacemaker was performed in four patients for malignant tachyarrhythmias and symptomatic bradycardia. Two Jewel 7219C and two Micro Jewel 7223 (Medtronic Inc.) implantable cardioverter defibrillators were connected to an endocardial lead (Medtronic Mod. 6936). In 2 patients with a previously implanted cardioverter-defibrillator, the pacemaker was connected to bipolar atrial and ventricular leads. In the 2 patients who already had a dual-chamber pacemaker, the unipolar pacing leads implanted previously were used. At the time of implantation of the second device, interaction testing was performed evaluating ventricular fibrillation detection during high-output asynchronous pacing. Testing for malfunctions, reprogramming or variations of pacing thresholds of the pacemakers after electric shocks was also carried out. Moreover, intracavitary signals, stimulation thresholds and electrical therapies delivered were evaluated during follow-up. RESULTS: During interaction testing, induced ventricular fibrillation was detected, with no delay, in 4/4 cases (100%). After a mean follow-up of 6.3 +/- 5 months (range 2-13), 5 shocks for ventricular fibrillation, 10 shocks for fast ventricular tachycardia and 72 antitachycardia pacings were delivered. All these electrical therapies were considered appropriate. No symptomatic sustained tachyarrhythmia remained undetected. CONCLUSIONS: These data suggest that concomitant implantation of a fourth-generation automatic cardioverter-defibrillator and a dual-chamber pacemaker using either unipolar or bipolar leads can be performed safely. Extreme caution and rigorous interactions testing is nevertheless advised when considering this device combination. 相似文献

Nanobiocomposites are a new class of biodegradable polymer materials with an ultrafine phase dispersion of the order of a few nanometers in a biodegradable polymer matrix that shows very interesting properties often very different from those of conventional filled polymers. In this work the morphology and the rheological and mechanical properties of a new nanobiocomposite made of a biodegradable copolyester based blend with an organomodified montmorillonite have been investigated to evaluate its possible use in several applications. SAXS diffractograms and TEM micrographs show that the in both the adopted processing conditions an hybrid intercalated/exfoliated morphology is observed. Rheological behavior is significantly influenced by the presence of the filler and more important, depends on the type of the applied type of flow. As for the mechanical properties, the elastic modulus strongly increases, while the elongation at break remarkably decreases. © 2015 Wiley Periodicals, Inc. J. Appl. Polym. In this paper a new procedure for the calculation of uniform corrosion current density is presented. The procedure is based on the following criterion: corrosion current and extent of anodic and cathodic areas are such that the entropy production rate is maximum. Experimental data for the reaction Fe(s) + HCl(aq) = FeCl2(aq) + H2(g), taken from literature, are used to test the new model. A comparison between corrosion current density calculated from weight loss measurements and current density calculated from polarization curves by means of the proposed model is carried out. The proposed model provides a better agreement, between Tafel line extrapolation and gravimetric measurement, in comparison to the mixed potential theory usually adopted. 相似文献

This work presents a formulation for thick plates following Mindlin theory. The fundamental solution takes into account an assumed displacement distribution on the thickness, and was derived by means of Hormander operator and the Radon transform. To compute the inverse Radon transform of the fundamental solution, some numerical integrals need to be computed. How these integrations are carried out is a key point in the performance of the boundary element code. Two approaches to integrate fundamental solutions are discussed. Integral equations are obtained using Betti's reciprocal theorem. Domain integrals are exactly transformed into boundary integrals by the radial integration technique. 相似文献
