Cox's proportional hazards regression model is a useful statistical tool for the analysis of 'survival data' from longitudinal studies. This multivariate method compares the 'survival experience' between two or more exposure groups while allowing for simultaneous adjustment of confounding due to one or more covariates. In addition to the summary regression statistics, further insight on the exposure--response relationship can be gained by visually examining the covariates-adjusted survival curves in the respective comparison groups. Covariates-adjusted survival curves are usually computed by the 'average covariate method'. This method is, however, subject to potential drawbacks. A method that avoids these drawbacks is to estimate adjusted survival curves by the corrected group prognostic curves approach. We have written a computer program to construct survival curves by the latter method. The program is coded in the Interactive Matrix Language of SAS.  相似文献

The maximum elastance of the ventricle (Emax) is a strong candidate for a quantitative index used for determination of the timing of weaning the patient from a left ventricular assist device (LVAD). The authors present a new and less invasive method for deriving Emax of the left ventricle under the LVAD assistance. In this method (the CoP method), Emax can be calculated from two different end-systolic points which are produced by changing the drive phase of LVAD without any vascular clamping and any direct measurement of the left ventricular volume. Animal experiments indicated that the CoP method is useful when the measured left ventricular flow and pressure are employed. Moreover, a new technique for estimating the left ventricular flow was developed to make the CoP method less invasive without direct measurement of the flow. The technique could considerably improve the estimation accuracy of the flow in the co-pulsation mode in comparison with the previous one proposed by the authors. However, it has been revealed that the estimation accuracy of the left ventricular flow was not globally high enough to apply the CoP method to clinical cases in spite of its much less invasiveness  相似文献

The authors' group has recently developed an expert system realized by means of fuzzy logic algorithms for monitoring and malfunction diagnosis of the left ventricular assist device (LVAD), one of the many types of artificial hearts developed today. The system is named TOTOMES (Tohoku University and Toyohashi University of Technology, monitoring and estimation system). TOTOMES is suitable for clinical application because it operates in an online and realtime fashion on a widely used personal computer in Japan. This article introduces the structure and function of the TOTOMES and explains its techniques, such as multi-interrupt tasks and dynamic system identification; as well as fuzzy reasoning used for realizing state estimation, detection, and diagnosis of malfunctions; and monitoring for cardiovascular dynamics under ventricular assistance and the LVAD drive system  相似文献

Site-directed mutagenesis studies have shown that the isopenicillin N synthase of Cephalosporium acremonium (cIPNS) requires two essential histidine residues (H216, H272) for activity. The determination of iron bound to the wildtype cIPNS and its absence in the mutants lacking histidine at positions 216 and 272 clearly supports the essential role these two histidines play in iron binding. However, nuclear magnetic resonance (NMR) studies have indicated that there could be three histidine residues that possibly coordinate the essential iron at the active site. To search for a presumed third histidine ligand, mutant cIPNS genes containing mutations at two histidine codons were created by in vitro cloning of fragments from the expression vectors bearing the respective cIPNS genes each with a single histidine mutation at positions H49, H64, H116, H126 and H137. All ten possible double histidine mutant cIPNS constructs were subsequently expressed in Escherichia coli. If a third histidine had a participatory role in the iron active centre of cIPNS, then one of the constructed double histidine mutants would have lost its enzymatic activity. However, analysis of the cIPNS activities of these recombinant double histidine mutants indicated that none of them was totally inactivated. Thus, the involvement of a third histidine can be repudiated.  相似文献

UV-cured acrylate-jacketed optical fibres and nylon-jacketed optical fibres were heated up to 120°C and their changes in length measured. Experimental results show that residual strain of the acrylate-jacketed fibres is not caused by the temperature change of 20?120?20°C, while, for the nylon-jacketed fibres, compressive strain remains after the heat cycle. The strain is proportional to the cross-sectional area of the nylon layer.  相似文献

In sub-100-nm generation, gate-tunneling leakage current increases and dominates the total standby leakage current of LSIs based on decreasing gate-oxide thickness. Showing that the gate leakage current is effectively reduced by lowering the gate voltage, we propose a local dc level control (LDLC) for SRAM cell arrays and an automatic gate leakage suppression driver (AGLSD) for peripheral circuits. We designed and fabricated a 32-kB 1-port SRAM using 90-nm CMOS technology. The six-transistor SRAM cell size is 1.25 /spl mu/m/sup 2/. Evaluation shows that the standby current of 32-kB SRAM is 1.2 /spl mu/A at 1.2 V and room temperature. It is reduced to 7.5% of conventional SRAM.  相似文献

On designing optical fiber cable, it is necessary to deal with optical fiber weakness, such as small breaking elongation compared to metal materials and excess loss under both lateral and hydraulic pressure. This paper presents a structural design method for the submarine optical fiber unit and cable based on the study of both lateral and hydraulic pressure characteristics. This paper also clarifies that the fiber proof test level has been determined for new cables to be applied in sea areas 8000 m in depth.  相似文献

Forty-eight patients with a variety of primary renal diseases and systemic lupus erythematosus (SLE) were examined for the proportion of circulating T lymphocytes bearing receptors for IgM (T mu cells) or IgG (T gamma cells). Although the control group showed strikingly similar mean values for both T mu and T gamma cells, the whole group of patients with primary renal diseases and SLE showed a wide scatter of values. Sixteen patients with primary renal diseases and SLE had higher proportions of T gamma cells than the control group, whereas seven patients with chronic glomerulonephritis (CGN), membranoproliferative glomerulonephritis (MPGN), lipoid nephrosis (LN), and SLE showed very marked decrease in the proportions of T gamma cells in the peripheral blood. On the other hand, six out of the total group of patients had low proportions of T mu cells in the peripheral blood. However, no consistent relationship between the proportion of T mu and T gamma cells was found in our study. These findings indicate that there exists a heterogeneity of T-lymphocyte subpopulation distribution in some patients with primary renal diseases and SLE. The possible significance of these phenomena in the pathophysiology of renal diseases is discussed.  相似文献
