Somatic treatments in psychiatry, ECT (electro-convulsive therapy) and pharmacotherapy, are based on a particular conception of our nature as human beings, and on a particular conception of clinically constructed reality. Acceptance of the basis for such treatments constitutes a tacit admission of our nature as being essentially materialist and deterministic biological machines, and undermines the connectedness which ensures that we each acknowledge the humanity and humanness of the other. By objectifying the other as different (by reason of disordered biology), these connections are severed and allow the imposition of physical forms of treatment in the name of cure. In this personal reflection, I draw upon my own experience of ECT from a nursing perspective, and attempt to explain the basis of my antipathy for this treatment. In the course of this exploration I also raise some of the issues which I feel nursing must deal with if it is to continue uncritically to adopt the biological paradigm as the basis of its praxis.  相似文献

Particles of two different sizes (0·105–0·295 mm and 0·84–1.00 mm diameter) of two marine algae, Sargassum fluitans and Ascophyllum nodosum, were crosslinked with formaldehyde (FA), glutaraldehyde (GA) or embedded in polyethylene imine (PEI), followed by glutaraldehyde crosslinking. They were used for equilibrium sorption uptake studies with cadmium, copper, nickel, lead and zinc. The metal uptake by larger particles (0·84–1·00 mm) was higher than that by smaller particles (0·105–0·295 mm). The order of adsorption for S. fluitans biomass particles was Pb > Cd > Cu > Ni > Zn, for A. nodosum copper and cadmium change places. Uptakes of metals range from qmax = 378 mg Pb g?1 for S. fluitans (FA, big particles), to qmax = 89 mg Zn g?1 for S. fluitans (FA, small particles) as the best sorption performance for each metal. Generally, S. fluitans is a better sorbent material for a given metal, size and modification, although there were several exceptions in which metal sorption by A. nodosum was higher. The metal uptake for different chemical modifications showed the order GA > FA > PEI. A comparison of different sorption models revealed that the Langmuir sorption model fitted the experimental data best.  相似文献

The BEACOPP (bleomycin, etoposide, adriamycin, cyclophosphamide, vincristine, procarbazine, and prednisone) regimen, a rearranged and accelerated version of the standard COPP/adriamycin, bleomycin, vinblastine, and dacarbazine (ABVD) chemotherapy, has been shown to be effective and safe in a previous pilot study for advanced stage Hodgkin's disease (HD). The present study aimed to determine a maximum practicable dose of three drugs, ie, etoposide, adriamycin, and cyclophosphamide, for which acute toxicities were acceptable and to assess the feasibility of the escalated scheme. Sixty untreated patients with advanced stage HD were enrolled in this study. Radiotherapy was given in 44 patients (73%) after chemotherapy to initial bulk lesions and residual disease. Granulocyte-colony stimulating factor (G-CSF) was given from day 8 to prevent prolonged neutrocytopenia and severe infections. The intended doses of adriamycin, etoposide, and cyclophosphamide in the BEACOPP schedule could be substantially escalated: adriamycin from 25 to 35, cyclophosphamide from 650 to 1,200, and etoposide from 100 to 200 mg/m2. The major toxicities were leukocytopenia and thrombocytopenia with considerable heterogeneity between individual patients. Of 60 patients, 56 (93%) achieved a complete remission (CR). At a median observation of 32 months, the rates of survival and freedom from treatment failure (FFTF) were estimated to be 91% (95% confidence interval 83% to 99%) and 90% (82% to 98%). These results show that a moderate dose escalation of adriamycin, cyclophosphamide, and etoposide of the baseline BEACOPP regimen is feasible. The escalated BEACOPP regimen shows very encouraging results in advanced stage HD and is now being compared in a randomized phase III study with BEACOPP at baseline dose level.  相似文献

44 patients with dementia of the Alzheimer type (DAT), 44 elderly normal control (ENC) Ss demographically matched to the DAT group, 42 patients with Huntington's disease (HD), and 42 middle-aged normal control (MNC) Ss demographically matched to the HD group were administered letter and category fluency tasks. DAT patients showed an overproportional impairment on category than on letter fluency tasks, whereas HD patients were equally impaired. Analyses based on receiver operating characteristic curves revealed that category fluency correctly classified significantly more DAT and ENC Ss than did letter fluency, whereas the 2 fluency tasks did not differ in this respect for HD and MNC Ss. Results suggest that HD patients' failures on fluency tasks are caused by impaired initiation/retrieval capacities. In contrast, DAT patients' greater category than letter fluency deficits are primarily due to a breakdown in the structure of semantic knowledge. The fatty acids of liver lipids from rats raised on a fat free diet from the 30th to the 90th day after birth were analyzed with special regard to the detection of positional isomers of mono-, di-, tri-, and tetraenoic fatty acids. The methyl esters obtained after transesterification of total lipids were separated by argentation chromatography into five fractions: I saturated, II monoenoic, III dienoic, IV dienoic nonmethylene interrupted, V triand tetraenoic fatty acid esters. After hydroxylation of the double bonds with osmium tetroxide, the analysis of the poly-O-trimethylsilyl derivatives by gas liquid chromatography on S.C.O.T. columns combined with mass spectrometry revealed the presence of 19 monoenoic, 15 dienoic, and 9 trienoic as well as 3 tetraenoic fatty acid isomers including the normally occurring representatives of the (n−3), (n−6), (n−7), and (n−9) fatty acid families. The majority of the identified isomers can be coordinated to one of these families like 7–16∶1; 11–20∶1; 6,9–18∶2; 8,11–20∶2; 5,11–20∶2; 5,8,11–20∶3; 7,10,13–22∶3 to the (n−9) family, 11–18∶1; 13–20∶1; 5,11–18∶2; 7,13–20∶2; 6,11–18∶2; 6,9–16∶2; 8, 11–18∶2; 10,13–20∶2; 5,8,11–18∶3; 7,10,13–20∶3; 4,7,10,13–20∶4 to the (n−7) family and 11,14–20∶2; 5,11,14–20∶3; 6,9,12–18∶3; 8,11,14–20∶3; 5,8,11,14–20∶4; 7,10,13,16–22∶4 to the (n−6) family. All these naturally occuring isomers can be placed into a network of desaturation and chain elongation steps which allows certain conclusions about the substrate specificity of the Δ6-, Δ5-and Δ4-desaturase systems. The great number of isomers found in the (n−7) family indicates that the members of this family are actively metabolized in partial essential fatty acid deficiency.  相似文献

Investigated the structure and development of causal–experimental thought. 260 Ss (aged 12–16 yrs) were examined by 3 test batteries. The batteries involved items addressing combinatorial, hypothesis handling, experimentation, and model construction abilities. Confirmatory factor analysis indicated that these abilities, although distinct from each other, do share a common functional core, and they are organized in a higher order causal–experimental specialized structural system. Rasch scaling indicated that these 4 kinds of abilities follow overlapping developmental trajectories. Saltus modeling suggested that development is continuous rather than discontinuous. Individual differences were found in the rate of acquisition but not in the structure of abilities.
