Extracellular vesicles (EVs) are promising therapeutic tools in the treatment of cardiovascular disorders. We have recently shown that EVs from patients with Acute Coronary Syndrome (ACS) undergoing sham pre-conditioning, before percutaneous coronary intervention (PCI) were cardio-protective, while EVs from patients experiencing remote ischemic pre-conditioning (RIPC) failed to induce protection against ischemia/reperfusion Injury (IRI). No data on EVs from ACS patients recovered after PCI are currently available. Therefore, we herein investigated the cardio-protective properties of EVs, collected after PCI from the same patients. EVs recovered from 30 patients randomly assigned (1:1) to RIPC (EV-RIPC) or sham procedures (EV-naive) () were characterized by TEM, FACS and Western blot analysis and evaluated for their mRNA content. The impact of EVs on hypoxia/reoxygenation damage and IRI, as well as the cardio-protective signaling pathways, were investigated in vitro (HMEC-1 + H9c2 co-culture) and ex vivo (isolated rat heart). Both EV-naive and EV-RIPC failed to drive cardio-protection both in vitro and ex vivo. Consistently, EV treatment failed to activate the canonical cardio-protective pathways. Specifically, PCI reduced the EV-naive Dusp6 mRNA content, found to be crucial for their cardio-protective action, and upregulated some stress- and cell-cycle-related genes in EV-RIPC. We provide the first evidence that in ACS patients, PCI reprograms the EV cargo, impairing EV-naive cardio-protective properties without improving EV-RIPC functional capability. NCT02195726相似文献

There are basically two approaches to the use of psychometric tests in the detection of "brain damage": the first is centred around a view of regional equipotentiality of brain functioning and the second around a unitary view of brain functioning. The former was explored in an earlier paper in this journal (Rubino, 1970). The latter approach is explored here in terms of the utility of three well-known tests: the Bender Gestalt, the Benton Visual Retention Test, and the Graham Kendall Memory for Designs. The ever-increasing complexity of production systems, together with the need to obtain efficient processes with limited costs, has led companies to develop custom tools for process control and management. Even for risk assessment, the traditional models often are overcome by methods that are best suited to specific needs. In this context, the aim of this paper was to propose a new model, which we call the global safety improve risk assessment (G-SIRA). This model can classify risks and identify corrective actions that allow the best risk reduction at the lowest cost. The proposed model, which is based on improvements to previous research, uses the analytic hierarchy process approach to develop a valid and simple tool for risk management. The G-SIRA method has been tested in a real-world application, i.e., it was applied to all of the processes of a textile company, and the results were compared with those obtained from the classical approach failure mode, effects, and criticality analysis. In this paper, we study the problem of how to detect the current transportation mode of the user from the smartphone sensors data, because this issue is considered crucial for the deployment of a multitude of mobility‐aware systems, ranging from trace collectors to health monitoring and urban sensing systems. Although some feasibility studies have been performed in the literature, most of the proposed systems rely on the utilization of the GPS and on computational expensive algorithms that do not take into account the limited resources of mobile phones. On the opposite, this paper focuses on the design and implementation of a feasible and efficient detection system that takes into account both the issues of accuracy of classification and of energy consumption. To this purpose, we propose the utilization of embedded sensor data (accelerometer/gyroscope) with a novel meta‐classifier based on a cascading technique, and we show that our combined approach can provide similar performance than a GPS‐based classifier, but introducing also the possibility to control the computational load based on requested confidence. We describe the implementation of the proposed system into an Android framework that can be leveraged by third‐part mobile applications to access context‐aware information in a transparent way. AbstractThe glycemia regulation is a significant challenge in the Artificial Pancreas (AP) scenario. Several control systems have been developed in the last years, many of them requiring meal announcements. Therefore, if the patients skip the meal announcement or make a mistake in the estimation of the amount of carbohydrates, the control performance will be negatively affected. In this extended version of our previous work, we present a Model Predictive Controller (MPC) for the AP in which the meal is treated as a disturbance to be estimated by an Unknown Input Observer (UIO). The MPC constraints are expressed in terms of Signal Temporal Logic (STL) specifications. Indeed, in the AP some requirements result in hard constraints (in particular, absolutely avoid hypoglycemia and absolutely avoid severe hyperglycemia) and some other in soft constraints (avoid a prolonged hyperglycemia) and STL is suitable for expressing such requirements. The achieved results are obtained using the BluSTL toolbox, which allows to synthesize model predictive controllers with STL constraints. We report simulations showing that the proposed approach, avoiding unnecessary restrictions, provides safe trajectories in correspondence of higher unknown disturbance. 相似文献

A significant reversible decrease in testosterone (T) has been associated with microgravity in male rodents and humans. Urinary T excretion increases in primates under hypergravity. Hypogonadism is somehow related to abnormally high levels of leptin (L), a hormone produced by the adipose tissue which has been found to increase under microgravity simulation conditions like head down bed rest (HDBR). The aim of this study was to assess hemodynamic and pituitary-adrenal and -gonadal adaptation to an acute HDBR test to be eventually used on a routine basis to get better prepared to next space flights. The Authors performed a 1 hour ?12° HDBR in 6 male and 6 female volunteers who underwent heart rate and blood pressure measurement together with a blood draw three times at 30 min intervals from the start to the end of the test for L, T, estradiol (E2), LH, androstenedione (A), cortisol (F), ACTH. 12 age- and sexmatched control subjects followed the same protocol except for keeping the sitting position all the time. According to the ANOVA for repeated measures, no changes occurred in L, T, E2 or LH whereas A, F and ACTH significantly decreased independently of gender. During HDBR systolic blood pressure decreased in both genders, diastolic blood pressure decreased significantly only in men and HR showed a more clear-cut decrease in women than in men. As a conclusion, such an acute steep-slope HDBR protocol may be efficiently used to testing immediate individual haemodynamic or adrenal response to microgravity but is not suitable for studies concerning gonadal adaptation. 相似文献

The emergence of psychiatric rehabilitation and the recovery movement generate new and expanded roles for psychologists in services for people with serious mental illness (SMI). However, the proportion of psychologists working in SMI services today is substantially less than previous decades. This article reviews the roles of psychologists in various mental health systems and outlines the contributions that psychologists can make in implementing evidence based approaches for people with SMI. A survey of American Psychology Association (APA)-accredited Council of University Directors of Clinical Psychology (CUDCP) Clinical Psychology doctoral programs was conducted. The results of the survey indicate an increase, since the early 1990s, in clinical faculty with SMI interests, and suggest that many graduate programs provide opportunities for SMI-relevant research and practicum training. However, the survey also indicates a lack of coursework on topics relevant to SMI and a lack of coursework relevant to assuming administrative and leadership roles in the mental health system. Despite training opportunities in graduate school, production of new PhDs who choose the SMI field is unlikely to meet the demand. According to the present study, the limiting factor is not availability of training, but student career choice. This paper proposes a new MRAS method to estimate the mechanical quantities in a double fed induction machine. In the traditional formulation the integral term, introduced by the PI MRAS regulator, is canceled in the transfer function between the estimated and the real speed. As a novelty, an additional term depending on the reference rotor flux through a constant gain g is introduced in the adjustable model of the MRAS. This innovation restores the integral term in the transfer function. As a consequence, during the closed loop operation, the estimated speed does not exhibit anymore a steady state error. The influence of the inverter non-linearities and of the non-linear rotor brush resistance on the speed estimation is also discussed. Some experimental tests validate the proposed method. 相似文献
