A hemodialysis patient with hepatitis C virus infection developed painful blisters on her hands that burst spontaneously. She was found to have serum porphyrin levels >2000 nmol/L. A punch biopsy revealed subepidermal blistering with festooning of dermal papillae associated with a mixed inflammatory infiltrate. Based on the clinical, biochemical, and histologic findings, a diagnosis of porphyria cutanea tarda was made. Treatment was started with twice-weekly phlebotomy and oral hydroxychloroquine and significant clinical improvement resulted.  相似文献

The emerging MPEG-4 standard encompasses a wide variety of applications, many of which are suitable for implementation on a Digital Signal Processor (DSP). In particular, consumer products with embedded multimedia capability, such as set-top boxes and wireless communicators, are suitable for DSP-based implementation. With a programmable approach, various algorithmic tradeoffs can be made, based on processing capability. For best performance, careful attention must be paid to memory allocation, data transfer, and ordering of instructions to best match the DSP architecture. We discuss implementing simple profile MPEG-4 video on the low-power TMS320C54x, core profile on the TMS320C6x, and scalable texture profile, which could be implemented on either processor family.  相似文献

Acute kidney injury is commonly encountered in critically ill patients, and is associated with worse outcomes. Fluid therapy is a key component in the management of these patients, often leading to fluid overload, especially in the setting of septic acute kidney injury. Emerging data overwhelmingly suggest that fluid overload in these patients may be associated with adverse outcomes. Management of such patients should include a strategy of early guided resuscitation, followed by careful assessment of fluid status, and early initiation of renal replacement therapy as soon as it is deemed safe, aiming for a neutral or negative fluid balance. This review will focus on the pathophysiological link between fluid overload and acute kidney injury, mechanisms of organ dysfunction in fluid overload, and strategies for management.  相似文献

The internal arrangement of the mammary gland cavity system, cisternal and alveolar milk fractions and the characteristics of milk ejection were investigated in buffaloes. Twenty-four Murrah buffaloes in three different stages of lactation and of two age groups were used. Continuous ultrasound cross-sections during milk ejection induced by exogenous oxytocin were performed to record the latency period of milk ejection. Buffaloes had small cisterns and the cavity area in the teat and gland regions were not significantly different (P > 0.05). The animals had long teat canals (3.1 +/- 0.1 cm), longer in the hind than fore quarters. Cisternal milk yield was low (0.17 +/- 0.01 kg) and cisternal fraction was only 4.9 +/- 0.1% of the total milk. The cisternal area (cm2) was 69.6 +/- 4.6, 51.61 +/- 4.8 and 26.01 +/- 4.8 while the cisternal yield (kg) was 0.32 +/- 0.05, 0.18 +/- 0.05 and 0.05 +/- 0.05 in early, mid and late lactation, respectively. A close correlation (r = 0.87, P < 0.05) existed between the ultrasound cisternal area and cisternal milk yield. The latency period of induced milk ejection was similar to that reported for cows (25 +/- 1 s) and was negatively correlated with milk yield (r = -0.75, P < 0.05). Milk ejection occurred shortly after elevated oxytocin concentrations were present. Delayed milk ejection reported earlier in this species must therefore be due to the absence of cisternal milk and delayed oxytocin release. An increase in teat length and circumference at milk ejection was also evident in the ultrasound cross sections.  相似文献

An experimental investigation has been carried out to study the heat transfer coefficient and friction factor by using artificial roughness in the form of specially prepared inverted U-shaped turbulators on the absorber surface of an air heater duct. The roughened wall is uniformly heated while the remaining three walls are insulated. These boundary conditions correspond closely to those found in solar air heaters.The experiments encompassed the Reynolds number range from 3800 to 18000; ratio of turbulator height to duct hydraulic mean diameter is varied from, e/Dh = 0.0186 to 0.03986 (Dh = 37.63 mm and e = 0.7 to 1.5 mm) and turbulator pitch to height ratio is varied from, p/e = 6.67 to 57.14 (p = 10 to 40 mm). The angle of attack of flow on turbulators, α = 90° kept constant during the whole experimentation. The heat transfer and friction factor data obtained is compared with the data obtained from smooth duct under similar geometrical and flow conditions. As compared to the smooth duct, the turbulator roughened duct enhances the heat transfer and friction factor by 2.82 and 3.72 times, respectively. The correlations have been developed for area averaged Nusselt number and friction factor for turbulator roughened duct.  相似文献

Biosorption of Pb(II) on bael leaves (Aegle marmelos) was investigated for the removal of Pb(II) from aqueous solution using different doses of adsorbent, initial pH, and contact time. The maximum Pb loading capacity of the bael leaves was 104 mg g?1 at 50 mg L?1 initial Pb(II) concentration at pH 5.1. SEM and FT-IR studies indicated that the adsorption of Pb(II) occurs inside the wall of the hollow tubes present in the bael leaves and carboxylic acid, thioester and sulphonamide groups are involved in the process. The sorption process was best described by pseudo second order kinetics. Among Freundlich and Langmuir isotherms, the latter had a better fit with the experimental data. The activation energy Ea confirmed that the nature of adsorption was physisorption. Bael leaves can selectively remove Pb(II) in the presence of other metal ions. This was demonstrated by removing Pb from the effluent of exhausted batteries.  相似文献

Two major areas of enquiry exist in the field of fibre-matrix adhesion in composite materials. One is the fundamental role that fibre-matrix adhesion plays on composite mechanical properties. The other is what is the best method used to measure fibre-matrix adhesion in composite materials. Results of an attempt to provide an experimental foundation for both areas are reported here. A well-characterized experimental system consisting of an epoxy matrix and carbon fibres was selected in which only the fibre surface chemistry was altered to produce three different degrees of adhesion. Embedded single-fibre fragmentation tests were conducted to quantify the level of fibre-matrix adhesion. Observation of the events occurring at the fibre breaks led to the documentation of three distinct failure modes coincident with the three levels of adhesion. The lowest level produced a frictional debonding, the intermediate level produced interfacial crack growth and the highest level produced radial matrix fracture. High fibre volume fraction composites made from the same material were tested for on- and off-axis, as well as fracture, properties. Results indicate that composite results can be explained if both differences in adhesion and failure mode are considered. It will be further demonstrated that fibre-matrix adhesion is an optimum condition which has to be selected for the stress state that the interface will experience. The embedded single-fibre fragmentation test is both a valuable measurement tool for quantifying fibre-matrix adhesion as well as the one method which provides fundamental information about the failure mode necessary for understanding the role of adhesion on composite mechanical properties.  相似文献
