Clients are leaving the hospital "quicker and sicker," and they frequently have acute care needs that must be met by resources outside the hospital setting. Community resources are diverse, vary widely from place to place, and have no central administration. Thus, using them can be challenging for both the nurse, client, and family. Reimbursement mechanisms underlie a person's ability to use resources and receive health care. By presenting two actual case scenarios where clients have mobility problems commonly seen by orthopaedic nurses, the authors discuss the community resources available, avenues of access to them, and their reimbursement mechanisms.  相似文献

An integrated remote sensing/field ecology project linked the use of synthetic aperture radar (SAR) and aerial photography to studies of landscape spatial heterogeneity and bird community ecology. P-, L-, and C-band SAR data, collected over a section of Kakadu National Park in Australia's Northern Territory during the Joint NASA/Australia DC-8 data acquisition campaign, were analyzed in light of field data integrating vegetation structure and floristics with bird abundances across a heterogeneous study site. Results indicate that SAR data are able to discern structural differences relevant to bird habitat quality within floristically homogeneous stands, while multispectral sensors successfully identified floristic differences among habitat types. Simplifying indices of bird diversity showed ambiguous changes across the site; however, the abundances of individual species were observed to change significantly across both floristic and structural gradients. These results suggest that efforts to map bird diversity should focus on species-specific habitat relationships and that some measure of vegetation structure is needed to understand bird habitat. The approach employed here advances the use of SAR data in the three-dimensional mapping of animal habitats from remotely sensed data, and extends current capabilities for mapping and modeling large-scale patterns in the distribution of biological diversity.  相似文献

Observers consciously prefer Mondrian's paintings in their original orientation compared with a rotated position—the “oblique effect” (Latto, Brain, & Kelly, 2000). However, this finding's premise, that all vertical–horizontal orientations of the thick black lines in Mondrian's oeuvre are preferred, overlooks the fact that the overall balance of these images is also altered when they are reoriented. Thus, balance may regulate the oblique effect, which might influence conscious aesthetic preferences. To address this issue, we explore Hess's (1965, 1972) claim that observers will unconsciously increase their pupil diameter to pleasing images and constrict it to unpleasant images. We overcame Hess's methodological limitation of not keeping his images' luminances and contrast constant across conditions by presenting eight Mondrian paintings (1921–1944) to 30 observers on a CRT for 20 s each in either their original or seven rotated positions. Simultaneously, we measured their pupil size while asking them to report how (dis)pleasing they found each image. We found both evidence for the oblique effect (where image rotation hampers preference) and a correlation between this consciously reported aesthetic preference and unconsciously derived pupil size. To measure exploratory play at 1 yr of age, 41 middle-class infants were observed in a free-play setting. In a separate session, 11 structured tasks assessed the infants' persistence in mastering tasks. In a 3rd session, the Bayley Scales of Infant Development were given. As part of another study, home observations were available for 40 of the infants at 1 yr of age and for 23 of the infants at 6 mo of age. Measures of quantitative aspects of exploratory play showed no relationship to persistence or cognitive development; however, measures indexing the quality of exploratory play did. In addition, environmental measures at both 6 and 12 mo were related to exploratory play at 1 yr, particularly to the production of perceptual effects; but some of these findings were opposite to those expected. Results suggest that the quality of exploratory play, rather than the quantity, is a better index of underlying mastery motivation. In addition, the widely accepted assumption of a link between the amount of general exploration and cognitive ability needs to be reexamined. Antibodies to DNA (anti-DNA) are the serological hallmark of systemic lupus erythematosus, a prototypic autoimmune disease. These antibodies bind to conserved sites on single-stranded and double-stranded DNA and display variable region somatic mutations consistent with antigen selection. Nevertheless, the interaction of anti-DNA with DNA has unconventional features. Anti-DNA antibodies bind by a mechanism called monogamous bivalency, in which stable interaction requires contact of both Fab sites with determinants on the same extended DNA molecule; the size of this DNA can be hundreds to thousands of bases, especially in solid phase assays. This binding also requires the presence of the Fc portion of IgG, a binding mechanism known as Fc-dependent monogamous bivalency. As shown by the effects of ionic strength in association and dissociation assays, anti-DNA binding is primarily electrostatic. Like anti-DNA autoantibodies, anti-DNA antibodies that bind specifically to non-conserved sites on bacterial DNA, a type of anti-DNA found in otherwise healthy individuals, also interact by monogamous bivalency. The unconventional features of anti-DNA antibodies may reflect the highly charged and polymeric nature of DNA and the need for molecular rearrangements to facilitate monogamous bivalency; the Fc portion contributes to binding in an as yet unknown way.  相似文献
