BACKGROUND: Endothelial dysfunction of coronary arteries with impaired vasodilation has been reported in patients with arterial hypertension. However, the effect of dynamic exercise on coronary vasomotion of a stenotic vessel segment before and after PTCA has not yet been evaluated in these patients. METHODS AND RESULTS:Coronary vasomotion of a normal and a stenotic vessel segment was studied in 39 patients with coronary artery disease during supine bicycle exercise before and 9+/-3 months after PTCA. Luminal area changes were determined by biplane quantitative coronary arteriography. There were 21 normotensive and 18 hypertensive patients who did not differ with regard to clinical characteristics. Percent area stenosis decreased after PTCA from 90% to 39% (P<0.001) in normotensive and from 86% to 33% (P<0.001) in hypertensive patients. Exercise-induced vasomotion of the normal vessel segment was significantly different between normotensives and hypertensives before (+19% versus +1%, P<0.01) and after (+16% versus +3%, P<0.01) PTCA. In contrast, stenotic vessel segments showed vasoconstriction in both normotensive and hypertensive patients (Deltaexercise, -11% versus - 20%, P=NS), which was reversed after PTCA (+3% versus +2%, P=NS). CONCLUSIONS: Normal coronary arteries show reduced vasodilation during exercise in hypertensive patients that may be explained by the presence of endothelial dysfunction. Stenotic vessels demonstrate paradoxical vasoconstriction during exercise in both normotensive and hypertensive patients. PTCA reverses vasoconstriction by elimination of the flow-limiting stenosis and prevention of coronary stenosis narrowing during exercise in normotensive and hypertensive patients.  相似文献

The role of intramuscular metabolite changes in the adaptations following isometric strength training was examined by comparing the effect of short, intermittent contractions (IC) and longer, continuous (CC) contractions. In a parallel study, the changes in phosphate metabolites and pH were examined during the two protocols using whole-body nuclear magnetic resonance spectroscopy (NMRS). Seven subjects trained three time per week for 14 weeks. The right leg was trained using four sets of ten contractions, each lasting 3 s with a 2-s rest period between each contraction and 2 min between each set. The left leg was trained using four 30-s contractions with a 1-min rest period between each. Both protocols involved isometric contractions at 70% of a maximum voluntary isometric contraction (MVC). The MVC, length:tension and force:velocity relationships and cross-sectional area (CSA) of each leg were measured before and after training. The increase in isometric strength was significantly greater (P = 0.041) for the CC leg (median 54.7%; P = 0.022) than for IC (31.5%; P = 0.022). There were no significant differences between the two protocols for changes in the length:tension or force:velocity relationships. There were significant increases in muscle CSA for the CC leg only. NMRS demonstrated that the changes in phosphate metabolites and pH were greater for the CC protocol. These findings suggest that factors related to the greater metabolite changes during CC training results in greater increases in isometric strength and muscle CSA.  相似文献

Indices were studied of the immunological status (cell-mediated, humoral immunity) before and after the multimodality pathogenetic treatment of patients with duodenal ulcer. Noted in the course of the complex treatment were changes in the immunoregulatory index, NK-cells, IgM. The findings obtained suggest that the immune defence may be depressed, with the vascular factor, infection (Helicobacter pylori) having their part in the genesis of duodenal ulcer.  相似文献

Immunoglobulin G (IgG) for intravenous use (IVIg) selectively stimulates production of interleukin-1 receptor antagonist protein (IL-1ra) by mononuclear cells in vitro and has been proposed to stimulate IL-1ra production in vivo as part of the therapeutic effect. We tested if IVIg differed from human IgG-containing media (i.e., autologous serum (HS) and plasma (HP)) in stimulating IL-1ra release by MNC in vitro and whether IVIg induced a delayed increase in serum levels of IL-1ra. IVIg, 0.01-0.5 mg/ml, increased the IL-1ra liberation from MNC 10-15 times over that of untreated controls. HP and HS (5% v/v) had comparable effects. However, the stimulated IL-1ra liberation was reduced to less than twice the background liberation when fetal calf serum (FCS)-precoated tubes were used. Three days of high-dose IgG infusion had no significant effect on the serum levels of IL-1ra. It is concluded that therapeutic effects of IVIg cannot be ascribed to significant stimulation of IL-1ra production in vivo, as previously suggested, and that the observed stimulation of IL-1ra production in vitro is an epiphenomenon strictly dependent upon adherence of human serum and plasma constituents.  相似文献

A coded panel of 90 poliovirus isolates, 30 of each of the three known serotypes, was used to evaluate five methods for the intratypic differentiation of polioviruses: (i) an enzyme-linked immunosorbent assay with polyclonal cross-absorbed antisera (PAb-E), (ii) a neutralization assay with type-specific monoclonal antibodies (MAb-N), (iii) a restriction fragment length polymorphism (RFLP) assay, (iv) a Sabin vaccine strain-specific PCR assay, and (v) a Sabin vaccine strain-specific cRNA probe hybridization (ProHyb) assay. Sequence analysis was used for the definitive characterization of the strains. The panel was distributed to five laboratories; each laboratory analyzed the strains by at least two methods. Each method was used by three or four laboratories. The total performance scores (percentage correct results per number of tests) of the five methods were 96.7% for PAb-E, 93.9% for MAb-N, 91.9% for RFLP assay, 93.3% for Sabin vaccine strain-specific PCR, and 97.4% for Sabin vaccine strain-specific ProHyb. Consistent results were obtained by each laboratory for 88 of 90 isolates (97.8%) examined by PAb-E, 81 of 90 isolates (90.0%) examined by MAb-N, 78 of 90 isolates (86.7%) examined by RFLP assay, 81 of 90 isolates (90.0%) examined by PCR, and 89 of 90 isolates (98.9%) examined by ProHyb assay. Six strains were classified differently by different methods. It is recommended that at least two methods be used for the intratypic differentiation of poliovirus isolates, and each method should be based on a different principle (i.e., antigenic properties and nucleotide sequence composition). If two assays yield discrepant results, further characterization, preferably by partial sequence determination, will be required for correct identification.  相似文献

The wave-vector dependence of the spin-wave line-width in yttrium iron garnets has been studied for samples of grain diameter varying from 1·2μm to 12μm. Glasses in the system PbO-BaO-TiO2-B2O3-SiO2 with and without P2O5 as nucleant have been prepared. The glass samples were ceramized based ondta studies. The ferroelectric phase crystallizing out has been found to be BaTiO3 fromxrd. The optical and scanning electron micrographs show the presence of BaTiO3 as major phase. In these glass ceramic samples, dielectric constant and dissipation factor are approximately constant with temperature and frequency upto the glass transition temperatureT g and thereafter increase sharply with temperature and finally level off. The addition of P2O5 as nucleant and molar ratio of (PbO + BaO) to TiO2 has marked influence on the dielectric behaviour and composition of ferroelectric phase crystallizing out.  相似文献
