Sick building syndrome is the term given to a heterogeneous constellation of symptoms that affects workers in modern mechanically ventilated office buildings. Although the cause is unknown, there is evidence that the local environment of the work station is an important determinant of symptoms. In this study, investigators examined the effect of a new, individually controlled ventilation system on workers' symptoms. Investigators studied two groups of workers in one mechanically ventilated office building: (1) a control group at whose worksite no intervention was made and (2) an intervention group. The intervention consisted of installation of a device that allowed each worker control over the ventilation supplied to his or her worksite. Just before, and 4 and 16 mo after installation of this device, workers completed self-administered questionnaires regarding occurrence of symptoms. The new ventilation system resulted in higher air velocities, more variable temperatures, and higher concentrations of airborne dust and fungal spores. Four months after installation, workers with the new ventilation system reported fewer symptoms that were (a) work-related (p < .05) and that were work-related and frequent (p < .05); in addition, they reported fewer symptoms that reduced their capacity to work (p < .01). Sixteen months after installation, workers with the new device reported fewer symptoms than at baseline (although not as significantly), and they indicated that the indoor air quality improved their productivity by 11%, compared with a 4% reduction of productivity among the control group of workers (p < .001). Investigators concluded that the new ventilation system, which provided the workers with individual control over ventilation, was associated with important and sustained reduction in symptoms.  相似文献

Excretion was studied of catecholamines and diphenilamine (DOPA) in 310 patients with carcinoma of the stomach and large intestine and 43 patients with non-malignant diseases. The oncological patients showed decrease in activity of the mediator link of the sympathoadrenal system (SAS) as well as its reserves but there was no association with sex, age, location or histological structure. Three types of SAS functioning were identified, such as compensation, overstrain and emaciation. Surgical intervention led to activation of the hormonal link and exhaustion of the system's reserves. Two kinds of sympathoadrenal response to stress were described--adequate and inadequate. In the former type, phases of stress remain as they are, unchanged, as are time periods of formation thereof, while under the latter one time periods of the phases formation or formation thereof get disordered.  相似文献

The percentage of long-term survivors after intensive chemotherapy and the outcome of MDS patients who achieve partial remission (PR) with intensive chemotherapy (IC) are not known. Between 1981 and 1996 we treated 99 patients with de novo MDS who had high-risk MDS or progression to AML, with IC. 41 (41%) achieved CR, 16 (16%) achieved partial remission (PR), 26 (26%) had failure, and 16 (16%) died in aplasia. Eight of the patients who achieved CR were autografted, three were allografted and the remaining cases received moderate consolidation chemotherapy. After IC, the 16 PR patients fulfilled the criteria for RA in 15 cases and CMML in one case. Median PR duration was 17 months, and three PR were > 3 years (39, 50+, 82+ months). Median actuarial survival of patients who achieved PR and CR was 18 months and 20 months from the onset of IC, respectively (difference not significant). Of the 71 patients treated before 1993, with sufficient follow-up, 10 (14%) had survived > 4 years (long-term survivors). Four of them were alive in first CR after 49+ to 110+ months and probably cured, two were alive in PR after 50+ and 82+ months and four had died after 49-78 months. Long-term survivors were characterized by a significantly higher incidence of RAEB-T at diagnosis, and with normal or favourable cytogenetic findings. In patients with RAEB-T at diagnosis included before 1993, 8/23 (35%) cases who had no unfavourable karyotype had survived > 4 years. Our findings suggest that MDS patients who achieve PR with IC, and not only those who achieve CR, can benefit from this type of treatment. The percentage of long-term survivors remains low, however, and is almost restricted to patients with RAEB-T at diagnosis and no unfavourable karyotype.  相似文献

The treatment of cancer patients with conventional chemotherapy is sometimes associated with severe systemic toxicity and only a minimal survival benefit. Because of this, new less toxic and more efficacious treatments have been sought. 8-Chloro-cAMP (8-Cl-cAMP) is one of a new generation of anticancer drugs that act at the level of signal transduction. In preclinical models, 8-Cl-cAMP modulates protein kinase A (PKA) leading to growth inhibition and increased differentiation of cancer cells. 8-Cl-cAMP was given to 16 patients with advanced cancer as an infusion via an indwelling subclavian venous catheter. We showed that 8-Cl-cAMP had a parathyroid hormone-like effect leading to increased synthesis of renal 1,25-dihydroxyvitamin D [up to 14 times the baseline value, median 3.6 times; P = 0.00001 (Student's paired t test)]. This produced the dose-limiting toxicity of reversible hypercalcemia that could not be controlled by the administration of either pamidronate or dexamethasone. The treatment was otherwise well tolerated, and other cAMP-dependent pathways (cortisol and TSH) were not affected, emphasizing the marked differences between organs in their sensitivity to this cAMP analog. Our results have shown that 8-Cl-cAMP is biologically active, and it is feasible that if the hypercalcemia can be controlled, then this drug may have a role as a single agent, or as a short infusion between cycles of chemotherapy.  相似文献

Intimal hyperplasia is the earliest microscopic change detectable in the arterial wall in the development of arteriosclerosis and atherosclerosis. To enable high-resolution imaging of this, a standard 1.5-T machine was modified using inserted gradient coils of 30 cm internal diameter and surface radio-frequency coils. Six rabbit aorta specimens with injury-induced intimal hyperplasia and two normal rabbit aorta specimens were imaged using a spin-echo sequence of TE 40. Pixel sizes as small as 20 µ×20 µ could be obtained and the area of intimal hyperplasia could be measured at 80 µ×40 µ. The results were compared with those made by a computer-linked microscope. In the injured aortas, there was a low-signal region on magnetic resonance imaging which corresponded to the media of the vessel wall when compared with the histology.T 1 andT 2 constants for the media were determined and compared with those of a normal specimen. TheT 1 constant was shorter for the media of the injured aorta than in the normal. This could be related to the disruption of the internal elastic lamina and migration of muscle cells to the intima which occurs following injury. The popliteal artery of four normal volunteers was imaged and the area of the wall could be measured at a pixel size of 312 µ×160 µ. This suggests thatin vivo studies of intimal hyperplasia arteriosclerosis and atherosclerosis could be made.  相似文献

While the atmospheric fate and transport of biogenic and anthropogenic hydrocarbons has been extensively studied, little is known about the behavior of first-, second-, and third generation photo-oxidation products that arise from OH radical oxidation of the parent species. The results of chamber experiments establish that *OH oxidation of biogenic and anthropogenic hydrocarbons yields carbonyls, dicarbonyls, hydroxycarbonyls, and keto-acids. However, little is known about the generation and fate of these products in the ambient atmospheric environment. This is changing because of the advent of methods that rely on 0-(2,3,4,5,6-pentafluorobenzyl)hydroxylamine (PFBHA) derivatization of carbonyls in concert with gas chromatography/ion trap mass spectrometry. Such methods provide the means to identify and quantify water-soluble organics, which historically have been difficult to measure. A limitation of existing sampling methods, however, is the use of devices that require low flow rates (0.5-1 L min(-1)). Accordingly, long sampling times (3-4 h) are needed to obtain pptv-ppbv detection limits. The mist chamber is an attractive device because of the high flow rates (25-70 L min(-1)) compatible with its use. Herein, we evaluate a mist chamber using a flow rate of 25-30 L min(-1) to provide short (10 min) sampling times and pptv limits of detection. The results establish a relationship between the Henry's law constant (KH) and the collection efficiency and demonstrate the suitability of the method to measure analytes with KH > or = 10(3) M atm(-1). Adjusting the pH, adding quaternary ammonium salts, or decreasing the temperature of the collecting solution in the mist chamber did not significantly affect the collection efficiency. We tested the method by sampling photooxidation products of isoprene (glyoxal, methylglyoxal, hydroxyacetone, and glycolaldehyde) in the Blodgett Forest, CA. This is the first report of a study the employs the mist chamberto sample hydroxycarbonyls. The accuracy and the reproducibility of the method were evaluated by the analysis of duplicate samples and field spikes. The mean recovery of field spikes was > or =80%, and the relative standard deviation was < or =22% between duplicate measurements. The detection limits were 48, 15, 7.7, and 2.7 pptv for glycolaldehyde, hydroxyacetone, methylglyoxal, and glyoxal, respectively. This work demonstrates the power of the mist chamber in concert with PFBHA derivatization and mass spectrometry to measure pptv concentrations of water-soluble organics with a sampling time of 10 min.  相似文献

An injection-molded internal supporting rib has been produced to control the flexibility of silicone rubber encapsulated electrodes designed to electrically stimulate the auditory nerve in human subjects with severe to profound hearing loss. The rib molding dies, and molds for silicone rubber encapsulation of the electrode, were designed and machined using AutoCad and MasterCam software packages in a PC environment. After molding, the prototype plastic ribs were iteratively modified based on observations of the performance of the rib/silicone composite insert in a clear plastic model of the human scala tympani cavity. The rib-based electrodes were reliably inserted farther into these models, required less insertion force and were positioned closer to the target auditory neural elements than currently available cochlear implant electrodes. With further design improvements the injection-molded rib may also function to accurately support metal stimulating contacts and wire leads during assembly to significantly increase the manufacturing efficiency of these devices. This method to reliably control the mechanical properties of miniature implantable devices with multiple electrical leads may be valuable in other areas of biomedical device design.  相似文献
