Dynamic characteristics of a 1.3 μm laser diode integrated witha spot-size converter coupled to a silica planar lightwave circuit

Dynamic characteristics are clarified for a laser diode integrated with a spot-size converter coupled to a silica planar lightwave circuit. The laser was developed with a butt-jointed selectively grown spot-size converter. A maximum coupling efficiency of -0.74 dB was achieved. Bit error rate performances are estimated to clarify the allowable reflection when optical feedback from the fibre output end occurs     

A case of idiopathic plasmacytic lymphadenopathy with polyclonal hyperimmunoglobulinemia associated with chronic nephritis

A 48-year-old female who had general fatigue was admitted to our hospital. She had swelling of the axillary, inguinal, and paraaortic lymph nodes and mediastinal lesions. Laboratory examinations showed anemia, polyclonal hyperimmunoglobulinemia with IgG 5570 mg/dl, renal dysfunction and interstitial changes of the lungs. Microscopic findings of hematoxylin-eosin staining in biopsy specimens of the left inguinal and axillary lymph nodes revealed increased levels of infiltration of mature plasma cells without evidence of malignancy. Immunoperoxidase staining showed intracytoplasmic polyclonal immunoglobulin. These findings were identical to those of idiopathic plasmacytic lymphadenopathy with polyclonal hyperimmunoglobulinemia (IPL) described by Mori et al. (1980). The specimens also showed evidence of chronic nephritis with infiltration of lymph cells and a slight invasion of plasma cells. Accordingly this case was diagnosed as IPL with renal involvement, which is associated with chronic nephritis. Recently, five cases of IPL with renal dysfunction have been reported. In particular, two cases of IPL with renal dysfunction, which included our case, revealed an increased level of IL6. These findings suggest that the occurrence of renal involvement with IPL may be related to changes in IL6, which is an important factor in the pathogenesis of IPL.     

An intercomparison of measurement techniques for the determinationof the dielectric properties of solids at near millimetre wavelengths

The results of a measurement intercomparison aimed at assessing the systematic errors in near-millimetre-wavelength dielectric measurements on reasonably transparent solid specimens are presented and discussed. Various monochromatic, broad band, guided wave and free space measurement methods were intercompared in the frequency region from 30 to 900 GHz using round-robin measurements of the refractive index and absorption coefficient of seven specimens. Typically, systematic errors of up to 1% were found in the refractive index results, while very much larger systematic errors were found in the absorption results     

Clinical significance of micromegakaryocytes in de novo AML

Bone marrow specimens obtained from 54 patients with de novo AML and 7 patients with AML evolving from MDS were retrospectively examined for the presence of micromegakaryocytes defined as cells of less than 30 microns in diameter with one or two nuclei. At least 25 megakaryocytes were counted in each patient. Micromegakaryocytes were found in 17 cases (31%), M1:1/11, M2:5/18, M3:0/4, M4:5/12, M5:1/4, M6:4/4, M7:1/1. The median age of the patients was higher in de novo AML with micromegakaryocytes (57 years) than in de novo AML without micromegakaryocytes (41 years) (p = 0.014). Chromosomal analysis revealed that deletion of 5 or 5q-, 7 or 7q- was recognized only in the group of de novo AML with micromegakaryocytes and that t(15;17), t(8:21) and inv (16) were not recognized in this group. Micromegakaryocytes were identified in each bone marrow specimen obtained from 9 of 10 patients with de novo AML with trilineage myelodysplasia. The complete remission rate was significantly lower in de novo AML with micromegakaryocytes (33%) than in de novo AML without micromegakaryocytes (86%) (p = 0.001). The duration of survival of the patients with de novo AML with micromegakaryocytes was shown to be shorter than that of the patients with de novo AML without micromegakaryocytes (p = 0.017). Micromegakaryocytes were recognized in all of 7 patients with AML evolving from MDS. The presence of micromegakaryocytes in bone marrow of the patients with AML indicates a subset of AML with poor prognosis that may be closely associated with myelodysplastic syndrome.     

Improvements in mental and physical status of elderly patients with dementia after institutionalization--assessment with a minimum data set

Frequency response of the glucose/maltose biosensors based on immobilized glucoamylase and glucose oxidase membranes of different thickness were investigated with the sinusoidal signal generator of substrate concentration. The results were modeled by the serial combination of the dead time and the first order response blocks, and time constants and dead times were estimated. The estimated value of time constant ranged from 4.5 to 13.2 s and that of the dead time from 3.2 to 7.7 s with the tendency that both time constant and dead time increase as membrane thickness increases and number of enzymatic reaction steps increases.     

Fenestration of the basilar artery associated with persistent primitive trigeminal artery

A 39-year-old male presented with a putaminal hemorrhage. Angiography disclosed the incidental findings of association of basilar artery fenestration and persistent primitive trigeminal artery. This is the first report of this association of congenital anomalies of the cerebral vessels.     

Improvement of longitudinal resolution by deblurring technique in helical CT of the head and neck

Patients for cataract surgery in India routinely undergo preoperative syringing to rule out chronic dacryocystitis. We determined the sensitivity and specificity of the clinical test of regurgitation on pressure over the lacrimal sac (ROPLAS) as a screening test for chronic dacryocystitis and compared it to syringing. 621 consecutive outpatients who needed syringing for various reasons (including 318 who had routine syringing prior to cataract surgery) were examined in a masked manner for regurgitation on pressure over the lacrimal sac. They then underwent syringing by a trained (masked) observer. The sensitivity and specificity of ROPLAS were 93.2% and 99.3%, respectively. Using a 6.6% prevalence of chronic dacryocystitis (the prevalence in our cataract population), the negative predictive value of the test was 99.5%. In the presence of regurgitation of pressure over the sac, the high specificity of ROPLAS confirms chronic dacryocystitis. In view of the opportunity costs, when ROPLAS is negative, preoperative syringing in cataract is perhaps unnecessary, unless the findings are equivocal or the index of suspicion for chronic dacryocystitis is very high.     

Time course of apoptosis in collagen-induced arthritis

A heterodox model of healing incorporating the basic dualism of Western philosophy--the world of concrete material substance versus that of thought, ideals, and spirit--is proposed. Such dualism is reflected in Western constructions of healing. An explanatory paradigm model with a horizontal axis representing paradigms of healing from positivism to metaphysics and a vertical axis representing activities of healing from the material to the spiritual was derived from ethnographic work with healers. A health practice model following the same philosophical patterns illustrates reflected American cultural values in the relative positioning of healing modalities from the orthodox biomedical to the most metaphysical or alternative. Examples of specific modalities of healing, many of which are resurfacing with increasing public interest in alternative therapies, are illustrated. This model places biomedicine in the same context as other philosophical approaches to healing as a standard for comparison. Implications for practice and research are discussed.     

Molecular heterogeneity of Vicia villosa-recognized perineuronal nets surrounding pyramidal and nonpyramidal neurons in the guinea pig cerebral cortex

This study examined the influence of trunk recruitment on the kinematic characteristics of pointing movements. The distribution of final positions of the hand, the extent and direction of the hand trajectory was basically preserved when trunk movement was combined with arm pointing. These effects were observed during pointing not only with but also without vision. The results imply that two functionally independent units of coordination are used in pointing regardless of visual feedback-one producing arm movement to the target (the reaching synergy) and the other coordinating trunk and arm movements diminishing the influence of the trunk on the arm endpoint trajectory (the compensatory synergy).     

Ischemic colitis arising in watershed areas of the colonic blood supply: a report of two cases

There are several weak points in the colonic blood supply, known as watershed areas, which result from incomplete anastomoses of the marginal arteries. These watershed areas are more vulnerable to ischemic injury than other parts of the colon. We report herein the cases of two patients who developed ischemic colitis well localized in the cecum, and in the rectosigmoid region at Sudeck's point, respectively. This report and our review of the literature suggest that watershed areas, including the splenic flexure, or Griffith's point, Sudeck's point, and the ileocecal region, are high-risk regions for the development of ischemic colitis.
