A 4-year-old boy presented with sudden onset of fever, pyuria, and bacteriuria. Ultrasound revealed left hydronephrosis and hydroureter. A plain abdominal radiography and excretory urogram showed a giant ureteral stone measuring 9 cm causing ureteral obstruction. Underlying anatomic or metabolic abnormalities were not detected. Extraction of the stone resulted in complete disappearance of the hydronephrosis and hydroureter.  相似文献

The islets of Langerhans contain four distinct endocrine cell types producing the hormones glucagon, insulin, somatostatin and pancreatic polypeptide. These cell lineages are thought to arise from a common, multipotential progenitor cell whose identity has not been well established. The pancreatic and intestinal hormone, peptide YY, has been previously identified in glucagon-producing cells in islets; however, transgenic mice expressing Simian Virus 40 large T antigen under the control of the peptide YY gene expressed the oncoprotein in beta, delta and pancreatic polypeptide cells, and occasionally developed insulinomas, suggesting relationships between peptide YY-producing cells and several islet cell lineages. The four established pancreatic islet cell types were examined for coexpression of peptide YY in islets of normal and transgenic mice throughout development. Peptide YY immunoreactivity was identified in the earliest endocrine cells in the fetal pancreas and was coexpressed in each islet cell type during development. Peptide YY showed a high degree of co-localization with glucagon- and insulin-producing cells in early pancreatic development, but by adulthood, peptide YY was expressed in less than half of the alpha cells and was no longer expressed in beta cells. Peptide YY was also coexpressed with somatostatin and pancreatic polypeptide when these cell types first appeared, but most delta and pancreatic polypeptide cells continued to express peptide YY throughout development. The use of conditions that distinguish peptide YY from the related peptides, pancreatic polypeptide and neuropeptide Y, as well as the ability of the peptide YY gene to direct expression of a reporter gene in islets of transgenic mice, establishes expression of peptide YY in the earliest pancreatic endocrine cells.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

We have used extracorporeal membrane oxygenation (ECMO) for 28 patients (14 children and 14 adults) over a 5 year period. Nine patients improved on ECMO and 5 were long-term survivors. ECMO was used for pulmonary insufficiency in 24 patients. Initially, only moribund patients were treated, but recently the combination of open lung biopsy and pulmonary insufficiency index (PII) has been used to select patients. The best results have been obtained in newborn cases and the adult capillary leak syndromes; the major problem has been progression to fibrosis despite ECMO support. ECMO was used for cardiac failure in 4 patients. Children with postoperative cardiac failure did the best; profound shock was not reversed with venoarterial bypass. ECMO support is lifesaving in selected cases of pulmonary insufficiency. Initial trials in cardiac failure and the infant age group in this series suggest that ECMO will have an even greater role in those applications.  相似文献

Thin film ultrasonic transducers have been designed which operate over an important frequency range, 300 kHz to 10 MHz. The transducers were made using piezoelectric aluminium nitride films a few microns thick. The films would have a fundamental thickness mode resonance at 1–3 GHz if fabricated as an unsupported film, however operation at much lower frequencies has been demonstrated when the transducers are fabricated on bulk substrates. This would enables them to be used in ultrasonic non-destructive testing in circumstances where the film can be deposited directly onto the object under test. We have found that the major factors influencing the below-resonance operation of the thin film transducers are the device impedance, the spectrum of the excitation pulse, and any mechanical (mass) loading applied to the back face of the transducer. Results are presented showing that the evolution of device impedance as a function of device area could be predicted using a PSpice model of the thin film transducer. The ability of the transducer to generate longitudinal mode pulses rather than shear wave pulses was found to depend on increasing the mechanical loading at the back face of the transducer. This mechanism for pulse generation was confirmed by Finite Element Modelling using PZFlex.  相似文献

BACKGROUND: Inhibition of thrombin by either the indirect thrombin inhibitor heparin or by more potent direct thrombin inhibitors such as hirudin reduces thrombus formation after arterial injury. The present study was designed to determine if a fibrin-specific thrombin inhibitor could, by local thrombin inhibition, prevent thrombosis more effectively. METHODS AND RESULTS: We first studied antithrombotic potency in vitro, comparing fibrin-targeted hirudin (recombinant hirudin covalently linked to the Fab' fragment of the anti-fibrin monoclonal antibody 59D8) to recombinant hirudin in baboon plasma. Fibrin-targeted hirudin was nine times more effective than recombinant hirudin in inhibiting fibrin deposition on experimental clot surfaces in baboon plasma (P < .01). The potency of fibrin-targeted hirudin was then compared with that of recombinant hirudin in a baboon model of thrombus formation. 111In-labeled platelet deposition was measured in a synthetic graft segment of an extracorporeal arteriovenous shunt in control animals and in animals receiving either fibrin-targeted hirudin or hirudin. In these experiments, fibrin-targeted hirudin was 10-fold more potent than hirudin in inhibiting platelet deposition and thrombus formation (P < .05). CONCLUSIONS: These data indicate that targeting a thrombin inhibitors such as hirudin to an epitope present in thrombi results in increased antithrombotic potency.  相似文献

Filtration efficiency of Ni(II) from aqueous solution using pristine and modified MWCNTs filters was investigated as a function of Ni(II) ion concentration, pH, and filter mass. MWCNTs were synthesized by CVD method and modified using two complementary treatments, purification (using a mixture of hydrochloric acid and hydrogen peroxide) and functionalization (using nitric acid). The effect and mechanism of each treatment on the structural integrity of pristine MWCNTs has been studied. Morphology of the pristine and modified filters was investigated by Raman Spectrometry (RS), Scanning Electron Microscopy (SEM), Energy Dispersive X-ray Spectroscopy (EDS), Fourier Transform Infrared (FTIR) spectrometry and Thermogravimetric analysis. It was found from Raman spec-tra that the ratio of the intensity of D-band to that of G-band decreased by purification process, and increased by functionalization process. The adsorption mechanism of Ni(II) onto the surface functional groups of modified MWCNTs was confirmed by FTIR spectrum. The filtration results showed that the removal efficiency of Ni(II) is strongly dependent on pH and could reach 85%at pH=8. Also, modified MWCNT filters can be reused through many cycles of regeneration with high performance. Functionalized MWCNTs filters may be a promising adsor-bent candidate for heavy metal removal from wastewater.  相似文献
