Part II: An example of the use of the comsis software: simulation of an optical network which uses wavelength multiplexing,fsk modulation format and direct detection

This second article deals with using the Comsis software for a particular application. Simulation is used to determine the maximum channel density in presence of crosstalk. In a first part, the modelling of the system is described. The simulation parameters of different optical elements are given. The results show that in the case of channels modulated at 500 Mbitls using fsk modulation format, and a direct detection operation at 1,5 μm, a minimum spacing of 6 GHz can be obtained with a 2 dB penalty. Thus if only the bandwidth of the erbium doped fibre amplifier is taken into account, the maximum number of channels which can be used is 250. However if the fsr (free spectral range) of optical filter is taken into account, the maximun number of channels is limited to 30.     

Preferred strategies for secondary infrainguinal bypass: lessons learned from 300 consecutive reoperations

PURPOSE: To determine the optimal surgical strategies in reoperative infrainguinal bypass, we reviewed our results in 300 consecutive secondary bypasses in 251 patients operated on between Jan. 1, 1975, and Nov. 1, 1993. METHODS: There were 168 men (67%) and 83 women (33%), with a mean age of 64.8 years and a typical distribution of risk factors including smoking (76.4%), diabetes (33.7%), and coronary artery disease (47.1%). The indications for surgery were limb-threatening ischemia in 83.5% and severe claudication in 16.5% of patients. The majority of conduits (n = 213) were autogenous vein and were composed of a single segment of greater saphenous vein in 121 bypasses (57%) and various alternative veins including composite, arm, and lesser saphenous vein in 92 bypasses (43%). Prosthetic conduits included 69 polytetrafluoroethylene, 16 umbilical vein, and two Dacron grafts. RESULTS: There was one perioperative death (0.3%) and a 25% total morbidity rate including a 1.7% myocardial infarction rate. There was a 28.6% early (< 30 days) graft failure and 10.7% early amputation rate for prosthetic bypass grafts compared with 13.6% early graft failure and 5.6% early amputation rates for vein grafts. Autogenous vein bypasses had higher 5-year secondary patency rates than had prosthetic grafts (51.5% +/- 4.6% vs 27.4% +/- 6.1%, p < 0.001). Results with autogenous vein bypass improved significantly from the 1975 to 1984 to the 1985 to 1993 interval with 5-year secondary patency rates increasing from 38.3% +/- 6.9% to 59.1% +/- 5.8% (p = 0.017) and 5-year limb-salvage rates increasing from 40.4% +/- 7.6% to 72.4% +/- 6.6% (p < 0.001). Vein grafts to the popliteal and tibial outflow levels had equivalent long-term results. Vein grafts completed for claudication demonstrated results superior to those for limb salvage, with a 5-year secondary patency rate of 75.8% +/- 8.1% versus 52.3% +/- 7.9% (p = 0.048). Secondary autogenous vein bypass grafting performed after early primary graft failure (< 3 months) did particularly poorly, with only a 27.2% +/- 7.7% 4-year secondary patency rate. Greater saphenous veins tended to perform better than alternative vein bypasses, with a 5-year secondary patency rate of 68.5% +/- 6.0% compared with 48.3% +/- 10.5% (p = 0.09) and a 5-year limb-salvage rate of 77.8% +/- 7.4% versus 54.2% +/- 11.8% (p = 0.046). CONCLUSIONS: When patients suffer a recurrence of limb-threatening ischemia at the time of infrainguinal graft failure, aggressive attempts at secondary revascularization with autogenous vein are warranted based on the low surgical morbidity and mortality rates and the improved patency and limb salvage rates that are currently attainable.     

Crescentic glomerulonephritis in interferon-gamma receptor deficient mice

Activation of macrophages by T cells is considered an initiating event in glomerular crescent formation. Since interferon-gamma (INF gamma) is a key mediator in T-cell-mediated activation of macrophages, we decided to test its role in a model of crescentic glomerulonephritis. An anti-glomerular basement membrane (GBM) serum was injected intravenously in presensitized wild-type or IFN gamma receptor deficient (IFN gamma(R)-/-) mice. Glomerulonephritis with glomerular crescents and tubulointerstitial inflammation developed in both strains, even though most evaluated morphological parameters and proteinuria indicated a less severe pathology in the mutant mice compared to the wild type. Thus, IFN gamma is not essential either for glomerular crescent formation or for tubulointerstitial involvement in anti-GBM glomerulonephritis in mice. In conclusion, the role of macrophages in this model might have been overestimated, or other cytokines may compensate for deficient IFN gamma signaling in the activation of macrophages.     

Les enjeux économiques de ľinterconnexion des réseaux

The network interconnection and its regulation consist in solving the following dilemma: increase competition and preserve the universal service through access charges, which symbolizes today institutional organization within the telecommunication industry. This paper presents some of the economic and social factors that drive the interconnection policies namely ona (open network architecture) in the United States, or onp (open network provision) in Europe. In the first section, we list some social and economical issues related to open network policies. In the second section, we study the different answers brought up by the regulator through alternative interconnection price formulas. In conclusion, we discuss the stakes and risks of interconnection, while the technological evolution of network introduces intelligent software in switching équipement and implements the tremendous abilities of the digital broadband networks.     

Reading handwritten digits: a ZIP code recognition system

A neural network algorithm-based system that reads handwritten ZIP codes appearing on real US mail is described. The system uses a recognition-based segmenter, that is a hybrid of connected-components analysis (CCA), vertical cuts, and a neural network recognizer. Connected components that are single digits are handled by CCA. CCs that are combined or dissected digits are handled by the vertical-cut segmenter. The four main stages of processing are preprocessing, in which noise is removed and the digits are deslanted, CCA segmentation and recognition, vertical-cut-point estimation and segmentation, and directly lookup. The system was trained and tested on approximately 10000 images, five- and nine-digit ZIP code fields taken from real mail     

Extending the Kernighan/Lin Heuristic for Hardware and Software Functional Partitioning

The Kernighan/Lin graph partitioning heuristic, also known as min-cut or group migration, has been extended over several decades very successfully for circuit partitioning. Those extensions customized the heuristic and its associated data structure to rapidly compute the minimum-cut metric central to circuit partitioning; as such, those extensions are not directly applicable to other problems. In this paper, we extend the heuristic for functional partitioning, which in turn can solve the much investigated codesign problem of partitioning a system's coarse-grained functions among hardware and software components. The key extension customizes the heuristic and data structure to rapidly compute execution-time and communication metrics, crucial to hardware and software partitioning, and leads to near-linear time-complexity and excellent resulting quality. Another extension uses a new criteria for terminating the heuristic, eliminating time-consuming and unnecessary fine-tuning of a partition. Our experiments demonstrate extremely fast execution times (just a few seconds) with results matched only by the slower simulated annealing heuristic, meaning that the extended Kernighan/Lin heuristic will likely prove hard to beat for hardware and software functional partitioning.     

Left ventricular outflow tract obstruction after mitral valve replacement

A method of posterior mitral annulus remodeling is presented. The posterior annulus is divided into three segments, each segment encircled by a suture that is passed in a tourniquet. Coaptation of the leaflets can be achieved by tightening the tourniquets while the ventricle is being filled. This technique is simple and quick, avoids the use of foreign material, and requires less expertise and judgment than traditional annuloplasties.     

Seroepidemiologic indicators for the evaluation of campaigns for the control of foot-and-mouth disease

A sero-epidemiological survey was conducted in two districts in Argentina between 1993 and 1995, to provide additional information on the epidemiology of foot and mouth disease (FMD) in Argentina and to assess the level of immunity in cattle populations, and the circulation of FMD virus. As part of the final stage of this survey, a comparison was made of the results obtained by the enzyme-linked immunosorbent assay (ELISA) and agar gel immunodiffusion techniques. Levels of population immunity against the four types of virus included in the vaccine increased progressively during the period of the survey until, in 1995, at the end of the vaccination period, the percentage of animals possessing adequate levels of protection was approximately 77% in yearlings, and more than 94% in cattle over one year old. During the three-year study, there was a clear tendency for viral activity to diminish, until in 1995 when between 3% and 0.6% were positive to the agar gel immunodiffusion test for the antigen associated with viral infection. By contrast, the ELISA detected antibody in about five times as many animals. The authors show how the increase in the level of population immunity was accompanied by a fall in viral activity.
