BACKGROUND: Clinical manifestations and course of sickle-cell anemia are variable. Knowledge about the factors, possibly geographic, that influence prognosis are still scanty. POPULATION AND METHODS: Data of hospitalization and management of children with sickle-cell disease were studied during two years (1992-1993) in the Pediatric Unit of Libreville Hospital. They concerned 205 admissions of 171 children and 131 outpatients. RESULTS: The main causes of hospitalization were: acute anemia (36 cases before the age of 5 years); painful crisis whose frequency increased with age (23% before 5 years, 35% between 5 and 10, 42% after 10 years); infections, essentially pulmonary occurring early, and bone infections at any age. Eight children died (because a complication of their disease). Among the 131 outpatients, half were detected because pyrexia, anemia and/or more often "hand-foot syndrome". More than 60% had hepatomegaly, one third still had splenomegaly after five years of age and more than one third was icteric. More than half children older than ten years had growth disorders. Mean hemoglobin level was 7 g/dL. 21 of the 83 tested children for HBsAg were positive and only one out of 79 was positive for HIV. CONCLUSIONS: Clinical manifestations and course of sickle-cell anemia in our patients are similar to those reported in Congolese children. Genetic and environmental factors may be responsible for differences with children from other, in particular French, cohorts.  相似文献

This paper presents a new optimization method for coupled vehicle–bridge systems subjected to uneven road surface excitation. The vehicle system is simplified as a multiple rigid-body model and the single-span bridge is modeled as a simply supported Bernoulli–Euler beam. The pseudo-excitation method transforms the random surface roughness into the superposition of a series of deterministic pseudo-harmonic excitations, which enables convenient and accurate computation of first and second order sensitivity information. The precise integration method is used to compute the vertical random vibrations for both the vehicle and the bridge. The sensitivities are used to find the optimal solution, with vehicle ride comfort taken as the objective function. Optimization efficiency and computational accuracy are demonstrated numerically.  相似文献

A vortex-ring theory of the superfluid4He -transition is extended to include the dynamics of the transition. The response of the vortices to an oscillating superflow is found by solving the Fokker-Planck equation. This allows a calculation of the superfluid relaxation time, which is in agreement with Landau-Khalatnikov theory and with dynamic scaling. At high frequencies the transition becomes broadened, with both the superfluid density and the dissipation remaining finite at and above T. Comparison is made to earlier theories that use high-temperature expansions and renormalization-group expansions. Applications to other subjects such as mutual friction, high-Tc superconductors, and rapidly quenched systems are briefly discussed.  相似文献

The kinetics of reduction of nickel chloride with hydrogen were investigated in the temperature range of 533 (260 °) to 788 K (515 °). Most experiments were done with porous NiCl2 granules having ?8+10 mesh (Tyler) size. The effects of temperature, sample size, granule size and hydrogen partial pressure on the reduction kinetics were investigated. In the temperature range 533 (260 °) to 651 K (378 °) the reduction is dominated by chemical kinetic factors. At higher temperatures diffusional effects appear to become quite significant. The temperature-dependence of the chemical kinetic rate constantK is given by $$\log K = 6.744 - \frac{{22,240}}{{2.303RT}};K\,\,is\,\,in\,\,\min ^{ - 1} $$ The activation energy for the reduction was found to be 22,240 cal /mol (or 93,050 J/mol) in the chemical kinetic regime.  相似文献

Abstract

The mass spectra calculated by summing the intensities of each nominal mass over all chromatographic times, the summed ion spectra, were calculated for a set of 440 commercially available ignitable liquids that had been analyzed by gas chromatography–mass spectrometry. Each ignitable liquid is generally comprised of a mixture of chemicals, and the mixtures may be very complex in some cases, e.g., petroleum distillates, gasoline, etc. The summed ion spectra were evaluated to determine if they contained sufficient information content to allow their use for rapid and accurate identification of the ignitable liquid in a database or library. The summed ion nominal mass spectra were encoded in one bit per channel at 1% transition intensity and a set of 96,580 unique pairwise comparisons were made between spectra, resulting in an average of approximately 50 differing channels per comparison. A subset of 62 summed ion spectra were further compared by a similarity metric and found by cluster analysis to group closely along the ASTM ignitable liquid classification scheme. Receiver operator characteristic analysis of the use of the similarity index in combination with the summed ion spectra was shown to provide 99% probability of correct liquid identification and 95% probability of correct ASTM classification by primary and sub-class. Interlaboratory tests found 95% probability of correct liquid identification and 85% probability of correct ASTM classification at the primary class level. These results demonstrate the use of the summed ion spectrum as a first step in the rapid identification of an ignitable liquid by database or library searching.  相似文献
