This study examined the relationship between cognitive and interpersonal styles and outcome among 24 clients who received time-limited cognitive therapy for depression. The authors hypothesized that this relationship would be mediated by therapeutic alliance. They found that clients' interpersonal style, particularly an underinvolved style, was predictive of treatment outcome. In a replication and extension of the Second Sheffield Psychotherapy Project (SPP2), a collaborative psychotherapy project (CPP) was carried out at 3 sites within the National Health Service of the United Kingdom. Clients (N?=?36) stratified at 3 levels of severity of depression were randomly assigned to one of 2 treatment approaches (psychodynamic-interpersonal or cognitive-behavioral) of 2 time-limited durations (8 or 16 sessions). Gains in both treatment approaches were approximately equivalent and were similar for CPP and SPP2 clients when measured at the end of treatment. However, CPP clients did not maintain their gains to the extent that the SPP2 clients did at 3-month and 1-year follow-up assessments. In the CPP, clients given 16 sessions showed a statistically significant advantage over clients given 8 sessions on some measures at some assessments; in the SPP2, similar effects were noted only among some subgroups of clients. Each of 6 counselors (aged 32–58 yrs) at college counseling centers was studied in counseling with 4 clients, of whom 2 were in initial sessions and 2 were in sessions drawn from ongoing counseling relationships. Clients perceived counselors as showing significantly greater empathy during ongoing than during initial sessions, and counselors perceived themselves as showing significantly greater empathy during initial sessions than did clients rating the same sessions. Clients rated counselors using fewer general advisements as more empathic, whereas counselors who rated themselves more empathic used more explorations and fewer reassurances. At the moment-to-moment level tapped by interpersonal process recall, exploration was the only category strongly associated with both client and counselor experiences of empathic communication in both initial and ongoing sessions. Criteria for reliable and clinically significant improvement were applied to standard and individually tailored outcome measures data from 212 depressed clients who had been randomly assigned to receive either 8 or 16 sessions of time-limited psychotherapy. The data were used to address 2 questions: (a) Is the dose-effect curve for psychological symptoms negatively accelerated? and (b) is there a differential rate of response for acute, chronic, and characterological/interpersonal components of depression? The results supported the differential rate of response of different components of depression and suggested qualifications to the acute, chronic, and characterological/interpersonal components and evidence that both supported and qualified previous suggestions that the dose effect curve is negatively accelerated. This study examined rates of improvement in psychotherapy as a function of the number of sessions attended. The clients (N = 1,868; 73.1% female; 92.4% White; average age = 40), who were seen for a variety of problems in routine primary care mental health practices, attended 1 to 12 sessions, had planned endings, and completed the Clinical Outcomes in Routine Evaluation-Outcome Measure (CORE-OM) at the beginning and end of their treatment. The percentage of clients achieving reliable and clinically significant improvement (RCSI) on the CORE-OM did not increase with number of sessions attended. Among clients who began treatment above the CORE-OM clinical cutoff (n = 1,472), the RCSI rate ranged from 88% for clients who attended 1 session down to 62% for clients who attended 12 sessions (r =-.91). Previously reported negatively accelerating aggregate curves may reflect progressive ending of treatment by clients who had achieved a good enough level of improvement. The convergent validity of the Agnew Relationship Measure (ARM) and the Working Alliance Inventory (WAI) was assessed in samples drawn from 2 comparative clinical trials of time-limited psychotherapies for depression. In 1 sample, clients (n=18) and therapists (n=4) completed self-report versions of both measures after every session (n=198). In the other sample, clients (n=39) and therapists (n=6) completed the ARM, and observers subsequently rated selected audiotaped sessions (n=78) using the WAI. In both samples, the ARM's core alliance scales (Bond, Partnership, and Confidence) were correlated with the WAI's scales (Bond, Tasks, and Goals) strongly when assessed within client and therapist perspectives and, with some qualifications, moderately when assessed between client, therapist, and observer perspectives, supporting the assumption that the ARM and the WAI measure some of the same core constructs. 104 clients completed a mailed follow-up 1 year after completing 8 or 16 sessions of treatment. Either cognitive-behavioral (CB) or psychodynamic-interpersonal (Pl) psychotherapy. Although mean scores on outcome measures at 1 year suggested that gains were, in general, well maintained, only 29% of clients were asymptomatic on all 3 occasions of testing without recourse to further treatment. However, only 11% of those asymptomatic at end of treatment experienced relapse or recurrence of depression, albeit on the limited evidence of just two follow-up assessments. The results of comparisons among treatment conditions at 1 year differed substantially from those obtained earlier. This article describes the intensive analysis of an event in psychodynamic–interpersonal therapy. The client found the event helpful because she was able to express anger; this appeared to be achieved through the technique of "staying with feelings." Observer ratings of emotion indicated that the client's experience of anger changed over the course of the event, from an atypical expression of anger to one described as normal by the pleasure–arousal theory of emotion. Experiential and interpersonal rationales for staying with feelings were compared with data describing change processes, and a possible mechanism of change was identified.
