Diaphragmatic dysfunction is a common postoperative complication of cardiac surgery in children, with important effects on respiratory morbidity. Its early diagnosis, followed by prompt surgical intervention, has been shown to reduce morbidity. However, the commonest method of diagnosis, based on hemi-diaphragmatic elevation on the chest radiograph, may be less accurate than direct techniques for assessing phrenic nerve function. We have compared electrophysiological and radiological diagnoses of diaphragmatic abnormality in 100 children (aged 3 days to 17.5 yrs) undergoing cardiac surgery, looking at respiratory morbidity as assessed by the duration of ventilation, the time spent on the cardiac intensive care unit (CICU), and the requirement for reintubation. Despite showing good reproducibility, radiological diagnosis was neither sensitive nor specific in identifying patients with electrophysiological phrenic nerve damage. Analysis of the measures of outcome supported the electrophysiological technique. Patients with electrophysiological evidence of damage had a longer duration of ventilation, spent longer on the CICU, and had a greater incidence of reintubation than either radiologically abnormal or "normal" patients. Chest X-rays are not a good method for diagnosing phrenic nerve damage in the early postoperative period in children. If early diagnosis is needed, then direct assessment of phrenic nerve function, such as the measurement of phrenic latency, may be a better technique.  相似文献

Tidal breathing of adult lampreys is produced by a single phase of excitation that compresses the branchial basket for exhalation. Inhalation is due to passive recoil. Motoneurons are located primarily in the ipsilateral motor nucleus of the vagus and exhibit periodic bursts of excitatory synaptic potentials from unidentified generator cells in the medulla. Respiratory discharges continue in the isolated brain and can be recorded from the vagal roots. The frequency of bursts increases with temperature in a manner roughly similar to that of intact animals. However, the discharges of the isolated brain are not very sensitive to altered pH or oxygen content in the bathing fluid. Mechanoreceptors in the gill can initiate "cough" responses and may modulate normal breathing.  相似文献

为了缓解状态空间爆炸问题,减小模型检测过程中生成的状态空间,加快模型检测速度,引入有限精度时间自动机(finite precision timed automata,简称FPTA)作为实时系统的形式模型,并提出了一种数据结构SDS(series of delay sequence)符号化表示状态空间中的状态集.FPTA只记录时钟变量的整数值及时钟变化的先后次序,从而减小生成的状态空间.在一定的时间约束下,Alur与Dill提出的时间自动机的可达性检测可简化为FPTA的可达性检测.举例描述了状态空间的生成过程和表示方法.最后,列出部分初步的实验结果,分析了SDS的特点及不足.  相似文献

With the increased popularity of qualitative research, researchers in counseling psychology are expanding their methodologies to include mixed methods designs. These designs involve the collection, analysis, and integration of quantitative and qualitative data in a single or multiphase study. This article presents an overview of mixed methods research designs. It defines mixed methods research, discusses its origins and philosophical basis, advances steps and procedures used in these designs, and identifies 6 different types of designs. Important design features are illustrated using studies published in the counseling literature. Finally, the article ends with recommendations for designing, implementing, and reporting mixed methods studies in the literature and for discussing their viability and continued usefulness in the field of counseling psychology. We have fabricated artificial molecules consisting of three coupled quantum dots defined in the two-dimensional electron gas of a GaAs/AlGaAs heterostructure using lithographically patterned gates and trenches. The three dots are arranged in a ring structure, where each dot is coupled to the other two dots. We find that, when tuned to the Coulomb blockade regime, the triple quantum dot device acts as a charge rectifier: an electron enters the third dot where it is trapped, producing a jamming effect where no other electron may enter the first dot. Triple quantum dots coupled in a ring will allow for the study of new molecular phases using artificial molecules and may also serve as building blocks of two-dimensional arrays for quantum computation.  相似文献

A multitude of `arc tracking' tests on cables for space applications has been performed to improve the basic understanding of the phenomena responsible for the fault arc behavior. The damage in a cable bundle develops in a complex system consisting of metallic conductors, insulation materials and arc plasma. Considering the degradation effects, vacuum as one of the applied test parameters, has turned out to be an important test environment. The investigations are based on a new test method, developed to assess arc tracking properties of cables for space applications. Tests were conducted with different insulation materials at cables with different AWG at 125 V dc and nominal currents. The tests were focused on the vacuum environment, using air only for the purpose of comparison. For some cable types, vacuum has turned out to be a worst-case environmental condition with respect to fault arc effects, such as the conductor burn-off, degradation of the insulation of adjacent cables and the extension of the fault arc. A software architecture is presented that allows client application programs to interact with a DBMS server in a flexible and powerful way, using either direct, volatile messages, or messages sent via recoverable queues. Normal requests from clients to the server and replies from the server to clients can be transmitted using direct or recoverable messages. In addition, an application event notification mechanism is provided, whereby client applications running anywhere on the network can register for events, and when those events are raised, the clients are notified. A novel parameter passing mechanism allows a set of tuples to be included in an event notification. An adaptive panoramic stereo approach for two cooperative mobile platform is presented. There are four key features in the approach: 1) omnidirectional stereovision with an appropriate vertical FOV, and a simple camera calibration method; 2) cooperative mobile platforms for mutual dynamic calibration and best view planning; 3) 3D matching after meaningful object (human subject) extraction; and 4) real-time performance. The integration of omnidirectional vision with mutual awareness and dynamic calibration strategies allows intelligent cooperation between visual agents. This provides an effective way to solve the problems of limited resources, view planning, occlusion, and motion detection of movable robotic platforms. We have developed an easy-to-use and cost-effective system to construct textured 3D animated face models from videos with minimal user interaction. This is a particularly challenging task for faces due to a lack of prominent textures. We develop a robust system by following a model-based approach: we make full use of generic knowledge of faces in head motion determination, head tracking, model fitting, and multiple-view bundle adjustment. Our system first takes, with an ordinary video camera, images of a face of a person sitting in front of the camera turning their head from one side to the other. After five manual clicks on two images to indicate the position of the eye corners, nose tip and mouth corners, the system automatically generates a realistic looking 3D human head model that can be animated immediately (different poses, facial expressions and talking). A user, with a PC and a video camera, can use our system to generate his/her face model in a few minutes. The face model can then be imported in his/her favorite game, and the user sees themselves and their friends take part in the game they are playing. We have demonstrated the system on a laptop computer live at many events, and constructed face models for hundreds of people. It works robustly under various environment settings.  相似文献
