The MR studies of three histologically proven spinal neurilemmomas and neurofibromas were reviewed retrospectively. There were two benign neurilemmomas (schwannomas) and one neurofibroma. The common characteristic of these cases was a central low intensity focus ("dot") seen on postcontrast T1-weighted imaging. The low intensity foci corresponded histologically to a congeries of changes including edema, microcysts, foam cells, hyalinization of blood vessels, old hemorrhage, and dystrophic calcification. 相似文献

A healthy adolescent boy was treated on two occasions for an overdose of chlorpropamide (Diabinese). Glucose therapy alone was not sufficient to control the hypoglycemia, but the administration of glucose plus diazoxide raised the blood sugar to supranormal levels. A bolus of intravenous glucagon briefly raised the blood sugar level to within normal limits, increased the blood ketones but also augmented insulin secretion. An overdose of sulfonylurea may cause prolonged and fatal hypoglycemia. Rational therapy, both in diabetic and normal persons, is glucose plus an "insulin antagonist." The administration of diazoxide was effective in our patient, substantially reducing the plasma insulin level; this agent may be the "insulin-antagonist" of choice for use in sulfonylurea-induced hypoglycemia. 相似文献

The authors tested 90 rhesus monkeys (Macaca mulatta) on a task of spatial memory, the spatial Delayed Recognition Span Test. The results showed that performance declined significantly with age, males had greater scores than females, and the rate of apparent decline with age was greater in males than in females. Both working and reference memory declined with age, but only working memory showed sex differences. The authors compared these data with that of 22 monkeys who were trained on a simpler version of the task before formal testing. Training had no effect on males but dramatically improved working memory in young females. The results confirm a male advantage in spatial working memory at a young age and confirm a greater decline with age in males than in females. We present a new theory for photogenerated carrier escape rates from single quantum wells, as a function of an applied electric field, that includes thermionic emission, direct tunneling, and tunneling via thermal occupation of upper subbands, and compare the results for GaAs/AlxGa1-xAs quantum wells with recent experiments. We account for the two dimensional (2D) density of states below the barrier, assume thermal equilibrium of carriers within the well, allow for the possibility of strain in the well and/or barrier, and include the contribution to electron thermionic emission from indirect conduction band minima. Our expressions for thermionic emission reduce, in the limit of large well width, to those derived by assuming a three-dimensional (3D) density of states. The results for electron emission from GaAs/AlxGa1-xAs quantum wells with x=0.2 and x=0.4 barriers at room temperature agree well with experiment. For wells with x=0.2 barriers, thermally assisted tunneling overtakes thermionic emission around 40 kV/cm, while for wells with x=0.4 barriers thermionic emission from the L valley conduction band minima dominates for fields less than 70 kV/cm. For holes we show that the escape rates are very sensitive to the in-plane effective masses, and results using simple expressions for the in-plane masses that do not include light/heavy-hole mixing agree poorly with experiment. The agreement with experiment is improved using in-plane masses that include light/heavy-hole mixing, particularly for wells with high barriers. A flux monitor diode is being explored as an option for measurement of the output of an X-ray tube that is used for active transmission measurements on a pipe containing UF6 gas. The measured flux can be used to correct for any instabilities in the X-ray tube or the high voltage power supply. For this measurement, we are using a silicon junction p-n photodiode, model AXUV100GX, developed by International Radiation Detectors, Inc. (IRD, Inc.). This diode has a silicon thickness of 104 μ and a thin (3-7 nm) silicon dioxide junction passivating, protective entrance window. These diodes have been extensively tested for radiation hardness in the UV range. However, we intend to operate mainly in the 10-40 keV X-ray region. We are performing radiation hardness testing over this energy range, with the energy spectrum that would pass through the diode during normal operation. A long-term measurement was performed at a high flux, which simulated over 80 years of operation. No significant degradation was seen over this time. Fluctuations were found to be within the 0.1% operationally acceptable error range. After irradiation, an I-V characterization showed a temporary irradiation effect which decayed over time. The objective of the current investigation is to characterize the dynamic strain aging (DSA) behavior in alloys 800H and 690. Constant extension rate tests were conducted at strain rates in the range of 10?4 s?1 to 10?7?s?1and temperatures between 295?K and 673?K (22?°C and 400?°C), in an argon atmosphere. Maps for the occurrence of serrated flow as a function of strain rate and temperature were built for both alloys. containing no gallium. Analysis of the 31P MAS NMR data demonstrated that addition of gallium to the sol–gel reaction increases the connectivity of the phosphate network
at the expense of hydroxyl groups. This premise is supported by the results of the elemental analysis, which showed that the
gallium-free sample contains significantly more hydrogen and by FTIR spectroscopy, which revealed a higher concentration of
–OH groups in that sample. Ga K-edge extended X-ray absorption fine structure and X-ray absorption near-edge structure data
revealed that the gallium ions are coordinated by six oxygen atoms. In agreement with the X-ray absorption data, the high-energy
XRD results also suggest that the Ga3+ ions are octahedrally coordinated with respect to oxygen. Antimicrobial studies demonstrated that the sample containing Ga3+ ions had significant activity against Staphylococcus aureus compared to the control. 相似文献

Early stages of insulin aggregation, which involve the transient formation of oligomeric aggregates, are an important aspect in the progression of Type II diabetes and in the quality control of pharmaceutical insulin production. This study is the first to utilize capillary electrophoresis (CE) with ultraviolet (UV) detection to monitor insulin oligomer formation at pH 8.0 and physiological ionic strength. The lag time to formation of the first detected species in the aggregation process was evaluated by UV-CE and thioflavin T (ThT) binding for salt concentrations from 100 mM to 250 mM. UV-CE had a significantly shorter (5-8 h) lag time than ThT binding (15-19 h). In addition, the lag time to detection of the first aggregated species via UV-CE was unaffected by salt concentration, while a trend toward an increased lag time with increased salt concentration was observed with ThT binding. This result indicates that solution ionic strength impacts early stages of aggregation and β-sheet aggregate formation differently. To observe whether CE may be applied for the analysis of biological samples containing low insulin concentrations, the limit of detection using UV and laser induced fluorescence (LIF) detection modes was determined. The limit of detection using LIF-CE, 48.4 pM, was lower than the physiological insulin concentration, verifying the utility of this technique for monitoring biological samples. LIF-CE was subsequently used to analyze the time course for fluorescein isothiocyanate (FITC)-labeled insulin oligomer formation. This study is the first to report that the FITC label prevented incorporation of insulin into oligomers, cautioning against the use of this fluorescent label as a tag for following early stages of insulin aggregation. 相似文献

CONTEXT: Many groups have developed guidelines to shorten hospital length of stay in pneumonia in order to decrease costs, but the length of time until a patient hospitalized with pneumonia becomes clinically stable has not been established. OBJECTIVE: To describe the time to resolution of abnormalities in vital signs, ability to eat, and mental status in patients with community-acquired pneumonia and assess clinical outcomes after achieving stability. DESIGN: Prospective, multicenter, observational cohort study. SETTING: Three university and 1 community teaching hospital in Boston, Mass, Pittsburgh, Pa, and Halifax, Nova Scotia. PATIENTS: Six hundred eighty-six adults hospitalized with community-acquired pneumonia. MAIN OUTCOME MEASURES: Time to resolution of vital signs, ability to eat, mental status, hospital length of stay, and admission to an intensive care, coronary care, or telemetry unit. RESULTS: The median time to stability was 2 days for heart rate (< or =100 beats/min) and systolic blood pressure (> or =90 mm Hg), and 3 days for respiratory rate (< or =24 breaths/min), oxygen saturation (> or =90%), and temperature (< or =37.2 degrees C [99 degrees F]). The median time to overall clinical stability was 3 days for the most lenient definition of stability and 7 days for the most conservative definition. Patients with more severe cases of pneumonia at presentation took longer to reach stability. Once stability was achieved, clinical deterioration requiring intensive care, coronary care, or telemetry monitoring occurred in 1% of cases or fewer. Between 65% to 86% of patients stayed in the hospital more than 1 day after reaching stability, and fewer than 29% to 46% were converted to oral antibiotics within 1 day of stability, depending on the definition of stability. CONCLUSIONS: Our estimates of time to stability in pneumonia and explicit criteria for defining stability can provide an evidence-based estimate of optimal length of stay, and outline a clinically sensible approach to improving the efficiency of inpatient management. 相似文献

BACKGROUND: The dysregulation of both myc gene expression and retinoid signaling pathways commonly occurs in small cell lung carcinoma (SCLC). Because preclinical data showed that all-trans-retinoic acid (RA) inhibited SCLC growth, altered myc expression, and blocked transition to a treatment-resistant phenotype, a Phase II trial was designed to determine the effects of the combination of RA, cisplatin, and etoposide in patients with SCLC. METHODS: Patients with untreated, extensive stage SCLC were treated with up to 8 cycles of cisplatin, 60 mg/m2, intravenously (i.v.) on Day 1 and etoposide, 120 mg/m2, i.v. on Days 1-3 in addition to up to 1 year of oral RA, 150 mg/m2/day. RESULTS: Of 22 assessable patients 1 had a complete response and 9 had a partial response, for an overall response rate of 45% (95% confidence interval, 24-68%). The median survival was 10.9 months and the 1-year survival was 41%. The median duration of chemotherapy was 6 cycles and the median duration of RA treatment was 2.8 months. Thirteen patients discontinued RA prematurely due to toxicity and only 4 responders were receiving RA at the time of recurrence. Toxicity-limiting RA treatment mainly was comprised of mucocutaneous changes and headaches. CONCLUSIONS: RA at a dose of 150 mg/m2/day was tolerated poorly in combination with cisplatin plus etoposide, leading to early discontinuation of RA in the majority of patients. The hematologic toxicity, response rate, and survival were similar to those associated with cisplatin and etoposide in prior trials. Further studies with more active and less toxic agents will be required to determine the role of retinoids in the treatment of SCLC. 相似文献
