The correlation between urodynamic and cystoscopic findings in elderly men with voiding complaints

PURPOSE: We evaluated the urethrocystoscopic findings and results of urodynamic studies in elderly men with voiding complaints. MATERIALS AND METHODS: A total of 492 consecutive patients with voiding complaints underwent a standardized screening program, including transrectal ultrasonography of the prostate, urodynamic investigations with pressure-flow study analysis and flexible urethrocystoscopy. RESULTS: A significant correlation was found between bladder trabeculation and grade of bladder outlet obstruction. Detrusor instability correlated significantly with grade of trabeculation. Grade of obstruction showed a clear correlation with prostatic occlusion of the urethra and the presence of a middle lobe at cystoscopy. CONCLUSIONS: The findings at urethrocystoscopy correlate well with those of urodynamic investigations.     

Reversible inhibition of glucose transport in human erythrocytes by a series of pyridine derivatives

A series of pyridine derivatives was chosen on the basis of structural similarity to glucose and to the hydrogen bonding capacity of this sugar. These compounds were found to reversibly inhibit glucose transport in a systematic fashion. The inhibition data can be rationalized by assuming that both competitive and noncompetitive binding sites exist on the cell membrane. The competitive site appears to have a much lower dissociation constant than the noncompetitive site for most derivatives. The effects of changes in substituents on the pyridine ring closely match, both in ring position and in the magnitude of the change in binding constant, those reported for derivatives of glucose. The stimulation of fluorodinitrobenzene inactivation of glucose transport by 2-amino-5-chloropyridine is in accord with this analysis. The strongest inhibitors bind about 100-fold more tightly than glucose itself. This series of pyridine compounds would appear to present a useful starting point for the development of site-specific irreversible inhibitors.     

Carcinosarcoma of the adrenal cortex presenting with mineralocorticoid excess

An adrenal carcinosarcoma is reported in a 79-year-old woman presenting with clinical signs of hyperaldosteronism. The tumor weighed 199 g and consisted of areas typical of adrenal carcinoma and areas of sarcoma. The sarcomatous component of the tumor showed osteogenic and chondroid differentiation. Vimentin stained both the carcinomatous and sarcomatous regions. Four months after resection, the patient developed metastases. This is the third reported case of adrenal carcinosarcoma and the only case in which hyperaldosteronism or bony differentiation was observed.
