Hypoproteinemia can result in many adverse consequences, including hypovolemia and the formation of edema. An understanding of the normal forces governing plasma will help the clinician tailor appropriate fluid therapy in these patients. BACKGROUND: It is our habit to employ an open drainage after thyroid surgery in our department. We have also found a large number of surgical infections in these patients (5.8% vs 2.5). Aim of the study is to evaluate prospectively if contamination happens during surgical procedure or in a later time according to the presence of the open drainage. METHODS: From October 1995 to November 1996, 113 patients who underwent a subtotal thyroidectomy were randomized into two groups: group A with antibiotic prophylaxis (57 patients) and group B without it (56 patients). RESULTS: One case (1.7%) of sepsis among 57 patients of group A and 2 cases (3.4%) among 56 patients of group B were observed. CONCLUSIONS: No statistical difference was found between the two groups despite antibiotic prophylaxis covering surgical procedure. It is personal opinion that sepsis arose after surgical procedure, due to the presence of the open drainage.  相似文献

Atractylon, a main sesquiterpenic constituent of Atractylodes rhizomes, was studied for the mechanism of its inhibitory effects on the tert-butyl hydroperoxide (t-BHP)-induced cytotoxicity and lipid peroxidation in primary culture of rat hepatocytes. In the preliminary study, atractylon showed an effective antioxidant property tested by its capacity for quenching 1,1-diphenyl-2-picrylhydrazyl radical (DPPH). Further investigations showed that atractylon at the concentrations of 0.01, 0.1 and 1.0 mg/ml decreased the formation of malondialdehyde (MDA), leakage of lactate dehydrogenase (LDH) and alanine aminotransferase (ALT) and repair synthesis of DNA induced by 30-min treatment of t-BHP (1.5 mM) in primary cultured rat hepatocytes. Addition of atractylon also attenuated the genotoxicity of t-BHP evaluated by unscheduled DNA synthesis. The sum of the results suggested that the protective effect of atractylon against oxidative stress induced by t-BHP is via its ability to quench free radicals.  相似文献

In a murine model it was shown that during a T help-1 cell dependent immune reaction, i.e. delayed type hypersensitivity (DTH), directed against the small molecular weight compound picryl chloride (PCI), altered lung functions are induced. Skin sensitization with PCI followed by intranasal hapten challenge resulted in an increment of pulmonary resistance and airway hyperresponsiveness which are general features of asthma. Whether this is also true for low molecular weight compounds, such as toluene diisocyanate, that can induce asthmatic complaints in humans, in addition to T help-2 cell dependent immune responses, remains to be clarified in future studies.  相似文献

Recent evidence suggests that bisphosphonates (BPs) may inhibit bone resorption by mechanisms that lead to osteoclast apoptosis. We have previously shown that BPs also reduce cell viability and induce apoptosis in the macrophage-like cell line J774. To determine whether BPs inhibit osteoclast-mediated bone resorption and affect J774 macrophages by the same molecular mechanism, we examined the potency to reduce J774 cell viability of pairs of nitrogen-containing BPs that differ slightly in the structure of the heterocycle-containing side chain but that differ markedly in antiresorptive potency. In all cases, the most potent antiresorptive BP of each pair also caused the greatest loss of J774 viability, while the less potent antiresorptive BPs were also less potent at reducing J774 cell viability. Similarly, the bisphosphinate, phosphonoalkylphosphinate and monophosphonate analogs of BPs (in which one or both phosphonate groups are modified, giving rise to much less potent or inactive antiresorptive agents) were much less potent or inactive at reducing J774 cell viability. Thus, the structure-activity relationships of BPs for inhibiting bone resorption match those for causing loss of cell viability in J774 cells, indicating that BPs inhibit osteoclast-mediated bone resorption and reduce J774 macrophage viability by the same molecular mechanism. Loss of J774 cell viability after treatment with BPs was associated with a parallel increase in apoptotic cell death. We have recently proposed that nitrogen-containing BPs reduce cell viability and cause J774 apoptosis as a consequence of inhibition of enzymes of the mevalonate pathway and hence loss of prenylated proteins. In this study, the BPs that were potent inducers of J774 apoptosis and potent antiresorptive agents were also found to be effective inhibitors of protein prenylation in J774 macrophages, whereas the less potent BP analogs did not inhibit protein prenylation. This provides strong evidence that BPs with a heterocyclic, nitrogen-containing side chain, such as risedronate, inhibit osteoclast-mediated bone resorption and induce J774 apoptosis by preventing protein prenylation.  相似文献

Data indicate that the synovial lining of the temporomandibular joint (TMJ) in some respects differs from other joints. The normal variation in morphology of the synovial lining of the TMJ is quite great, whereas the variation in pattern of pathologic changes appears to be relatively small (ie, synovial inflammation is not of the severity as that in other joints). In the current review, a system for histologic grading of synovial inflammation is proposed. The system is based on semiquantitative evaluation of the following set of parameters: 1) synovial lining cell layers; 2) vascularity (number or size of vascular profiles); and 3) Inflammatory cell infiltrate (commonly lymphocytes).  相似文献

In this study, four components of the Stroop effect were examined for manual word and vocal responses. The components were lexical, semantic relatedness, semantic relevance, and response set membership. The results showed that all four components were present in the vocal response task. However, in the manual word response task, the only component that produced significant interference on its own was response set membership. These results do not support predictions made by recent translation models (see W. R. Glaser & M. O. Glaser [1989] and Sugg & McDonald [1994]). A possible solution was suggested that located two sites for Stroop interference. The lexical, semantic relatedness, and semantic relevance effects were located in the lexical system, whereas the response set membership effect was located at a response selection stage.  相似文献

An experiment with a multiple Latin square design using 36 midlactation Friesian cows was carried out to determine the effect of fish meal or soybean meal in a supplement fed at two amounts (3.5 or 7 kg/d) with grass silage. The diets were also fed to four ruminally and duodenally cannulated cows to determine digestibility, ruminal fermentation, and nutrient flows. Dry matter intake, milk production, milk constituent yields, and concentrations of protein and lactose were significantly increased by the supplements when provided at 7 kg/d. The supplement containing fish meal increased milk production, protein and lactose yields, and milk protein concentration, but the increases were smaller than those obtained by increasing the amount of supplement fed. Neither the type nor the amount of supplement fed affected digestibility of dry matter or organic matter, and few significant effects on ruminal fermentation were observed. Supplements fed at the higher allowance significantly increased the flow of dry matter, organic matter, all nitrogenous components, and amino acids to the duodenum. The supplement containing fish meal significantly increased the duodenal flow of nonammonia nonmicrobial N and some amino acids. Results indicated that an increase in the concentration of supplement in the diet is more effective in increasing milk production, protein concentration and yield, and flow of amino acids to the duodenum than is increasing the concentration of ruminally undegradable protein in the supplement.  相似文献
