Introduction: Compared to traditional in‐center hemodialysis (HD), in‐center nocturnal dialysis (INHD) is characterized by longer sessions and nighttime administration, which may lead to better outcomes for some patients. Given the importance of patient choice in the decision to initiate INHD, we explored associations between patients’ psychosocial characteristics and their receipt of INHD. Methods: Among hemodialysis patients at a medium‐sized dialysis organization, we identified INHD patients as those for whom ≥80% of dialysis sessions were INHD sessions—starting at 6:30 pm or later and lasting ≥5 hours—over the 3 months (≥20 sessions total) after their first INHD session. We extracted dialysis session data from electronic medical records and psychosocial data from social worker assessments. We tested associations of patients’ psychosocial characteristics—as well as demographic and clinical characteristics—with INHD receipt among all hemodialysis patients (INHD and HD) in bivariate analyses and multivariable logistic regression models. Findings: Among 759 patients with complete data, we identified 47 (6.2%) as INHD patients. On average, these patients were more likely than HD patients to be employed (full‐time 10.6% vs. 5.2%; part‐time 17.0% vs. 4.2%; P < 0.001), and they were significantly less likely to require ambulatory assistance (14.9% vs. 39.6%, P < 0.001). In multivariable regressions, we found that part‐time employment (versus being unemployed) was associated with a 7.1 percentage‐point higher likelihood of being an INHD patient (P = 0.01), and the negative association with ambulatory assistance needs approached statistical significance (P = 0.056). No other psychosocial factors included in this main regression analysis were statistically significantly associated with INHD patient status. Discussion: Researchers comparing the outcomes of patients undergoing INHD versus other treatment modalities will need to account for differences in employment status—and other factors like requiring ambulatory assistance and age which may predict the ability to work—between INHD users and comparison patients to avoid bias in estimates.  相似文献

An important property of wildfire behaviour is rate of spread (ROS). The objectives of this study are to evaluate the uncertainty of landscape-scale ROS estimates derived from repetitive airborne thermal infrared (ATIR) georeferenced imagery and the utility of such estimates for understanding fire behaviour and controls on spread rates. Time-sequential ATIR image data were collected for the Cedar, Detwiler, and Rey Fires, which burned in California during summers of 2016 and 2017. We analyse error, uncertainty, and precision of ROS estimates associated with co-location accuracy, delineation of active fire front positions, and generation of fire spread vectors. The major sources of uncertainty influencing accuracy of ROS estimates are co-registration accuracy of sequential image pairs and procedures for delineating active fire front locations and spread vectors between them; none of these were found to be substantial. Median ROS estimates are 11 m min?1 for the Cedar Fire and 8 m min?1 for the Detwiler Fire, both of which burned through mixed shrub and tree areas of the Sierra Nevada foothills and were estimated for downslope spread events. Of the three study fires, the fastest spread rates (average spread of 25 m min?1 with maximum of 39 m min?1) are estimated for the Rey Fire, which burned on variable directional slopes through chaparral shrubland vegetation.  相似文献

The heart is one of the most important and sophisticated organ of the human body. Coronary ischemia is a condition in which the coronary muscles do not receive sufficient blood and oxygen because of blocked or tightened heart vessels. This syndrome is called cardiac vessel illness. There have been numerous attempts to detect the impact of cardiac vessel illness on the heart muscles using noninvasive experiments. Most of the effects of ischemia as well as severe cardiac conditions on the muscles of the ventricle parts can be detected using ultrasonic images. If treatment is provided to suspected cases in the early stage of cardiac vessel illness, the chance of survival is high; for this, many software‐based detection approaches have been used. In this study, we propose an approach that can automatically diagnose the cardiac artery disease by using the cardiac echo images of the four parts of the heart.  相似文献

The ability of multilamellar vesicles of phosphatidylcholine and phosphatidylethanolamine in aqueous phase to prevent access to cholesterol by a nonpolar solvent was examined. Phosphatidylethanolamine vesicles retained less sterol than phosphatidylcholine vesicles. In mixed vesicles, cholesterol was retained in proportion to the amount of phosphatidylcholine. To alter the charge and hydration of head groups, pH was adjusted from 1.2 to 12.5. Above pH 8, both phosphatidylethanolamine and phosphatidylcholine retained sterol in a 1∶1 molar ratio of phospholipid to cholesterol, regardless of acyl side chain composition. Between pH 2.0 and pH 8.0, sterol retention varied with type of head group and side chain. Lipids with 16-carbon saturated side chains retained more sterol than 18-carbon unsaturated or 12-carbon saturated side chains. Between pH 1.1 and 2.0, none of the phosphatidylethanolamines retained sterol, but long chain phosphatidylcholines, saturated or unsaturated, retained sterol in a 1∶1 molar ratio of phospholipid to sterol. Short chain phosphatidylethanolamines and phosphatidylcholines retained 0 to 20% at the low- to mid-pH range. Size of multilamellar vesicles, measured by Doppler effect light scattering analysis, had no bearing on sterol retention. Sonication of vesicles, which increases surface curvature, increases the retention of sterol. Fluorescence polarization indicated that cholesterol does not interact with DPPC or DLPC side chains. The observations can be interpreted in terms of space requirements of head groups, including charge repulsion and hydration. Other factors, such as monovalent cation replacement by protons, juxtaposition of charged groups on vesicle surfaces and length and unsaturation of acyl side chains affect the affinity of phospholipids for cholesterol.  相似文献

Composite membranes consisting of biodegradable poly(butylene adipate‐co‐terephthalate (PBAT) and zeolite Y (0–10 wt %) were produced by extrusion. Zeolite Y is well dispersed in the membrane up to 5 wt %, but tends to agglomerate at higher contents. The presence of zeolite Y in the composite resulted in an improvement of the thermal stability, mechanical properties, and increased the barrier properties. The interaction of the composite membranes with rhynchophorol was investigated by different techniques, showing that the semiochemical progressively lixiviates PBAT monomers, causing thermal and mechanical properties to decrease. However, no interaction seemed to occur between the rhynchophorol and the zeolite. Studies of diffusion of pheromone through membranes have shown that the addition of the zeolite Y has not contributed significantly to a decrease in the release rate of rhynchophorol, but the presence of the zeolite Y helps to increase chemical, mechanical, and thermal stabilities of the membranes. © 2017 Wiley Periodicals, Inc. Decabromodiphenyl ether (BDE-209) isthe major component in the flame-retardant formulation DecaBDE which is incorporated into numerous consumer goods ranging from upholsteries to electronics. Because of the high volume of DecaBDE produced, its presence in consumer products and the environment, and the finding of BDE-209 in the blood of exposed workers, the extent of bioavailability, persistence, and potential debromination are important issues. To measure the bioconcentration, distribution, reductive debromination, and whole-body half-lives of BDE-209 after multiple low doses in an animal model, we dosed rats with a commercial DecaBDE (0.3 microg/g of diet) for 21 days and measured tissue polybrominated diphenyl ether levels during a 21 day withdrawal period. BDE-209, three nona-BDEs, and four octa-BDEs accumulated in the rats and distributed proportionately throughout the body. Only 5% of the total BDE-209 dose was present as parent compound in the rats after 21 days of dosing and <4% in the feces, suggesting extensive metabolism. A nona-BDE (BDE-207) and two octa-BDEs (BDEs-201 and -197) appeared to form via meta-debromination(s) of BDE-209 to a minimal extent (1% of the total BDE-209 dose). The wholebody half-lives tended to increase with decreasing bromination; however, two octa-BDEs, presumably forming from debromination, increased in the rats after 21 days of withdrawal and demonstrated the potential for BDE-209 to form more persistent lipophilic compounds in vivo.  相似文献

It is believed that the endowment of primordial follicles in mammalian ovaries is finite. Once follicles are depleted, infertility ensues. Thus, the size of the initial endowment has consequences for fertility and reproductive longevity. Follicular endowment is comprised of various processes that culminate with the incorporation of meiosis-arrested oocytes into primordial follicles. Apoptosis is prominent during follicular endowment, and apoptosis regulatory genes are involved in its regulation. Conflicting data exist with regard to the role of the proapoptotic Bcl-2 associated X protein (BAX) in follicular endowment. Therefore, we investigated the role of BAX during follicular endowment in embryonic and neonatal ovaries. We found that BAX is involved in regulating follicular endowment in mice. Deletion of Bax yields increased oocyte numbers in embryonic ovaries and increased follicle numbers in neonatal ovaries when compared with wild-type ovaries. Increased follicular endowment in Bax -/- ovaries is not due to enhanced germ cell viability. Further, it is not due to an increased primordial germ cell (PGC) allotment, a delay in the onset of meiosis, or altered proliferative activity of oogonia. Instead, our data suggest that the regulatory activity of BAX in follicular endowment likely occurs during PGC migration, prior to PGC colonization of the gonad.  相似文献

We combine CdSe semiconductor nanocrystals (or quantum dots) and single-crystal ZnO nanowires to demonstrate a new type of quantum-dot-sensitized solar cell. An array of ZnO nanowires was grown vertically from a fluorine-doped tin oxide conducting substrate. CdSe quantum dots, capped with mercaptopropionic acid, were attached to the surface of the nanowires. When illuminated with visible light, the excited CdSe quantum dots injected electrons across the quantum dot-nanowire interface. The morphology of the nanowires then provided the photoinjected electrons with a direct electrical pathway to the photoanode. With a liquid electrolyte as the hole transport medium, quantum-dot-sensitized nanowire solar cells exhibited short-circuit currents ranging from 1 to 2 mA/cm2 and open-circuit voltages of 0.5-0.6 V when illuminated with 100 mW/cm2 simulated AM1.5 spectrum. Internal quantum efficiencies as high as 50-60% were also obtained.  相似文献
