AIMS: To establish trends in frequency of serious spinal cord injuries in rugby and rugby league over a 20 year period and to elucidate patterns of injury from retrospective analysis of cases admitted to New Zealand's two spinal injuries units. METHODS: A detailed survey of unit records with follow-up of selected patients; statistical analysis of data. RESULTS: During the 20 years 1976 to 1995, 119 rugby and 22 rugby league players (total 141) were admitted to New Zealand's two spinal injuries units suffering serious spinal injuries and 47 of these became permanently confined to wheelchairs. There was a steady increase in frequency throughout the period studied. Of the injuries 83% occurred in forwards and 17% in backs. In rugby it was the scrum which produced most injuries, and in rugby league it was the tackle. The early season month of April produced most spinal injuries. In the eighteen months since intense compulsory educational programmes on safety were introduced by the New Zealand Rugby Union there have been no serious spinal cord injuries from rugby scrums. CONCLUSION: Contrary to widespread belief, there has not been a decrease in spinal cord injuries in rugby following rule changes in the mid 1980s. The information produced by this retrospective study has been an effective educational platform to make rugby and rugby league safer.  相似文献

The first monolithic integration of a metal-semiconductor-metal (MSM) InGaAs photodetector with a field-effect transistor (FET) and resistors into a high-impedance front-end photoreceiver circuit is discussed. The sample was grown in a single step by chemical beam epitaxy, and standard processing steps for making FETs were used to fabricate the receiver circuit. Semi-insulating Fe-doped InP layers were used as the insulating gate of the FET, the barrier enhancement layer in the MSM photodetector, and the electrical isolation layer between the photodetector and the electronic circuit. A bit error rate of less than 10-9 at 200 Mb/s has been achieved with this preliminary circuit for an optical power of -17 dBm  相似文献

Evaluating performance of individual features of WiMAX technology is a topic of widespread discussion. Currently, there is no quantitative way of measuring WiMAX technology so that wireless operators can meet their design objectives. This paper outlines a set of design criteria for WiMAX and provides a decision-making aid that ranks the importance of criteria using Analytic Hierarchy Process (AHP). This ranking should sufficiently reflect market expectations of the relative importance of various design criteria. A model integrating AHP priorities with enhanced Data Envelopment Analysis (DEA) is the basis for formulating a technological value in simple,comparable format. A case study is provided to show how this technological value is used to evaluate a three year network deployment plan. In the future, this model could be extended to WiMAX equipment suppliers for the purpose of validating performance targets of individual criteria, and enhancing supplier roadmaps for future network development.  相似文献

Although the induction of fuzzy decision tree (FDT) has been a very popular learning methodology due to its advantage of comprehensibility, it is often criticized to result in poor learning accuracy. Thus, one fundamental problem is how to improve the learning accuracy while the comprehensibility is kept. This paper focuses on this problem and proposes using a hybrid neural network (HNN) to refine the FDT. This HNN, designed according to the generated FDT and trained by an algorithm derived in this paper, results in a FDT with parameters, called weighted FDT. The weighted FDT is equivalent to a set of fuzzy production rules with local weights (LW) and global weights (GW) introduced in our previous work (1998). Moreover, the weighted FDT, in which the reasoning mechanism incorporates the trained LW and GW, significantly improves the FDTs' learning accuracy while keeping the FDT comprehensibility. The improvements are verified on several selected databases. Furthermore, a brief comparison of our method with two benchmark learning algorithms, namely, fuzzy ID3 and traditional backpropagation, is made. The synergy between FDT induction and HNN training offers new insight into the construction of hybrid intelligent systems with higher learning accuracy  相似文献

PURPOSE: The relationship of the division of the diaphragm during thoracoabdominal aortic repair to prolonged ventilator support has not been studied. The purpose of this study was (1) to determine whether preservation of diaphragm integrity has a significant effect on postoperative ventilator duration and (2) to elucidate other pulmonary risk factors related to thoracoabdominal aortic surgery and to study the relationship of these factors to the intact diaphragm technique. METHODS:Between February 1991 and January 1997, we repaired 397 descending and thoracoabdominal aortic aneurysms. Descending thoracic aneurysms were not included in the study because their repair does not include the diaphragm. A total of 256 patients participated in this study. The diaphragm was divided in 150 patients and left intact in 106 patients. Examined as potential risk factors were patient demographics, history and physical findings, aneurysm extent, urgency of the procedure, acute dissection, cross-clamp time, homologous and autologous blood product consumption, and adjunctive operative techniques. FEV1 also was considered in the 197 patients for whom preoperative spirometry was available. Prolonged mechanical ventilation was defined as ventilator support for >72 hours. Data were analyzed by univariate contingency table and multiple logistic regression methods. RESULTS: Increasing age (odds ratio [OR], 1.02/y; P <.02), current smoking (OR, 2.6; P <.0008), total cross-clamp time (OR, 1.0/min; P <.008), units packed red blood cells transfused (OR, 1.06/unit; P <.008), and division of the diaphragm (OR, 2.03; P <.02) were significant, independent predictors of prolonged ventilation. Sixty-seven percent of patients (71 of 106) whose diaphragms were preserved were extubated in <72 hours compared with 52% of patients (78 of 150) who underwent diaphragm division (OR, 0.53; P <.02). CONCLUSION: Independently of well known pulmonary risk factors, an intact diaphragm during thoracoabdominal aortic repair results in a higher probability of early ventilator weaning.  相似文献
