Reliability of seizure classification using a semistructured interview

Methods for standardized classification of epileptic seizures are important for both clinical practice and epidemiologic research. In this study, we developed a strategy for standardized classification using a semistructured telephone interview and operational diagnostic criteria. We interviewed 1,957 adults with epilepsy ascertained from voluntary organizations. To confirm and expand the seizure history, we also interviewed a first-degree relative for 67% of subjects and obtained medical records for 59%. Three lay reviewers used all available information to classify seizures. To assess reliability, each reviewer classified a sample of subjects assigned to the others. In addition, an expert physician classified a sample of subjects assigned to two of the reviewers. Agreement was "moderate-substantial" for generalized-onset seizures, both for the comparisons between pairs of lay reviewers and for the neurologist versus lay reviewers. Agreement was "substantial-almost perfect" for partial-onset seizures, both for pairs of lay reviewers and for the neurologist versus lay reviewers. These results suggest that seizures can be reliably classified by lay reviewers, using operational criteria applied to symptoms ascertained in a semistructured telephone interview.     

A six-wafer combustion system for a silicon micro gas turbineengine

As part of a program to develop a micro gas turbine engine capable of producing 10-50 W of electrical power in a package less than one cubic centimeter in volume, we present the design, fabrication, packaging, and experimental test results for the 6-wafer combustion system for a silicon microengine. Comprising the main nonrotating functional components of the engine, the device described measures 2.1 cm×2.1 cm×0.38 cm and is largely fabricated by deep reactive ion etching through a total thickness of 3800 μm. Complete with a set of fuel plenums, pressure ports, fuel injectors, igniters, fluidic interconnects, and compressor and turbine static airfoils, this structure is the first demonstration of the complete hot flow path of a multilevel micro gas turbine engine. The 0.195 cm³ combustion chamber is shown to sustain a stable hydrogen flame over a range of operating mass flows and fuel-air mixture ratios and to produce exit gas temperatures in excess of 1600 K. It also serves as the first experimental demonstration of stable hydrocarbon microcombustion within the structural constraints of silicon. Combined with longevity tests at elevated temperatures for tens of hours, these results demonstrate the viability of a silicon-based combustion system for micro heat engine applications     

Konvergenz von quadratischen Interpolations- und Flächenabgleichssplines

The purpose of this paper is to show that for continuous functions the related quadratic splines converge without any assumption on the spline grid. The points of the interpolatory grid can be chosen between the corresponding points of the spline grid with a division ratio from \(\frac{{\sqrt 2 }}{2}\) to \(1 - \frac{{\sqrt 2 }}{2}\) . In the case of continuously differentiable functions the division ratio can even be taken between 0 and 1; in addition, the order of convergence is increased. For twice differentiable functions the full order of convergence is obtained. Analogous results about the convergence of histo splines are proved.     

Acute kidney failure in hypothermia

Two cases with acute renal failure after prolonged hypothermia are presented. Both patients were found in come, became rapidly uremic and required hemodilaysis treatment. Although the laboratory findings were typical of severe muscle damage, e.g. elevated levels of serum creatinine phosphokinase, serum lactic dehydrogenase and serum aldolase activities, visible "crush-injuries" were not found. Acute renal failure was characterized by extreme catabolism and severe metabolic acidosis. After 4 and 10 hemodialyses respectively, the patients became polyuric and finally were discharges with normal renal and muscle function. Hypotension with diminished renal perfusion and nontraumatic rhabdomyolysis due to prolonged hypothermia are regarded as the dominant pathogenetic factors in the acute renal failure.     

Computer simulation of the precordial QRS complex: effects of simulated changes in ventricular wall thickness and volume

Over a 4-year period 40,923 operations and 44,716 surgical admissions were monitored for both community and hospital onset infections. One thousand eight hundred sixty-five patients had 1966 surgical wound infections and 2056 remote infections including 1652 hospital onset and 404 community onset infections. One thousand one hudnred forty-four patients with multiple infections averaged 40 days in the hospital contrasted with 24 days for 721 patients with a single wound infection. The total excess cost of hospitalization for these patients was $951,150. A statistically significant reduction occurred for urinary tract infections, lower respiratory infections and clean and contaminated surgical wound infections. It is suggested that these are all inter-related and a significant reduction in surgical wound infections can be achieved through control of infections at remote sites, particularly those associated with medical devices. The coagulase positive staphylococcus is still the most important single bacterial species in the primary etiology of surgical wound infections. When the gastrointestinal tract is entered or "supra" infecting organisms appear, gram negative bacteria and mixed gram negative and gram positive infections are dominant. Reduction in remote site infections occurring in surgical patients is necessary to reduce the incidence of surgical wound infections, suggest preventive and control measures, and document the effectiveness of such measures.     

Photoinduced phenomena in nanostructured porous silicon

In this work we study the evolution of porous silicon photoluminescence under illumination. Samples were obtained by electrochemical etching of crystalline silicon wafers of different types. For the p-type samples the evolution of the spectra is explained in terms of photoinduced oxidation of nanostructures, which in turns leads to a discrete change in the photoluminescence spectra, as we reported in previous works. For the n-type material, a progressive decrease of the luminescence intensity is observed, which is attributed to the photoinduced generation of dangling bond related defect states at the surface layer surrounding the nanostructures. This model explains qualitatively well the kinetics of the evolution of the measured photoluminescence. Preliminary results of electronic paramagnetic resonance spectroscopy agree with this model.     

Conformal aluminum oxide coating of high aspect ratio structures using metalorganic chemical vapor deposition

The metalorganic chemical vapor deposition of aluminum oxide has been studied over a wide process parameter range. Electrical properties of as-grown and annealed layers have been investigated using planar aluminum/aluminum oxide/silicon capacitors. The best processing conditions resulted in a leakage current of 10 nA/cm² at an equivalent oxide thickness of 3.6 nm. In addition, the film conformality was evaluated on silicon trench structures with aspect ratios of up to 60. Excellent step coverage of over 90% (thickness at trench bottom to thickness at trench middle) was achieved at temperatures below 400 °C and a pressure of 100 Pa. After annealing the electrical properties of these layers, analyzed on planar test structures, were comparable to the results obtained at higher deposition temperature.     

On cosets of the generalized first-order reed-muller code with low PMEPR

Golay sequences are well suited for use as codewords in orthogonal frequency-division multiplexing (OFDM), since their peak-to-mean envelope power ratio (PMEPR) in q-ary phase-shift keying (PSK) modulation is at most 2. It is known that a family of polyphase Golay sequences of length 2^m organizes in m!/2 cosets of a q-ary generalization of the first-order Reed-Muller code, RM_q(1,m). In this paper, a more general construction technique for cosets of RM _q(1,m) with low PMEPR is established. These cosets contain so-called near-complementary sequences. The application of this theory is then illustrated by providing some construction examples. First, it is shown that the m!/2 cosets of RM_q(1,m) comprised of Golay sequences just arise as a special case. Second, further families of cosets of RM_q(1,m) with maximum PMEPR between 2 and 4 are presented, showing that some previously unexplained phenomena can now be understood within a unified framework. A lower bound on the PMEPR of cosets of RM_q(1,m) is proved as well, and it is demonstrated that the upper bound on the PMEPR is tight in many cases. Finally, it is shown that all upper bounds on the PMEPR of cosets of RM_q(1,m) also hold for the peak-to-average power ratio (PAPR) under the Walsh-Hadamard transform (WHT)
