The author discussed the increase in the frequency of traumatic paralyses of the ocular muscles, and reported 6 cases of inferior oblique muscle paresis caused by local blunt or sharp traumas (haematoma, contusion, rupture, incarceration) and one case of traumatic Brownian pseudo-paralysis. The localisation of the injuries in 3 cases was on the inferior temporal part and in the other 3 cases on the superior part of the bulbar conjunctiva. Atypical horizontal deviation and characteristic vertical deviation, torsion and torticollis were observed in the majority of cases. One patient recovered spontaneously. 3 patients became asymptomatic after correction by prisms and one after recession of contralateral superior rectus muscle. The symptoms remained unchanged in the case of a "blow out" fracture (in spite of operation) and a Brown's syndrome (without treatment). 相似文献
A series of 11 patients undergoing abdoninoperineal resection for "suture line recurrence" following anterior resection is presented. Five-year survival is 10%. Technically, the procedure is difficult and major problems are encountered, including large blood loss and ureteral complications. These patients had an inadequate distal margin of resection at the time of anterior resection. The survival of this group of patients underscores the importance of making the correct judgment about anterior or abdominoperineal resection at the time of the initial presentation of the patient. The phrase "suture line recurrence" is a misnomer; all of these patients had advanced pelvic malignancy. If the adequacy of the distal margin is questionable or a distal margin of 5 cm cannot be obtained safely at the time of anterior resection, abdominoperineal resection should be performed, as the opportunity for cure of a recurrence should this rule be compromised is limited. 相似文献

Emotion recognition from speech signals is an interesting research with several applications like smart healthcare, autonomous voice response systems, assessing situational seriousness by caller affective state analysis in emergency centers, and other smart affective services. In this paper, we present a study of speech emotion recognition based on the features extracted from spectrograms using a deep convolutional neural network (CNN) with rectangular kernels. Typically, CNNs have square shaped kernels and pooling operators at various layers, which are suited for 2D image data. However, in case of spectrograms, the information is encoded in a slightly different manner. Time is represented along the x-axis and y-axis shows frequency of the speech signal, whereas, the amplitude is indicated by the intensity value in the spectrogram at a particular position. To analyze speech through spectrograms, we propose rectangular kernels of varying shapes and sizes, along with max pooling in rectangular neighborhoods, to extract discriminative features. The proposed scheme effectively learns discriminative features from speech spectrograms and performs better than many state-of-the-art techniques when evaluated its performance on Emo-DB and Korean speech dataset.
