One hundred patients underwent transjugular intrahepatic portosystemic shunt (TIPS) creation for variceal bleeding (n = 94), intractable ascites (n = 3), hepatorenal syndrome (n = 2), and preoperative portal decompression (n = 1). Shunts were completed in 96 patients. Portal vein pressure was reduced from 34.5 mm Hg +/- 7.6 (standard deviation) to 24.5 mm Hg +/- 6.2; the residual portal vein-hepatic vein gradient was 10.4 mm Hg +/- 0.9. Acute variceal bleeding was controlled in 29 of 30 patients. Of the 96 patients who underwent successful TIPS creation, 26 have died and 22 have undergone liver transplantation; the remaining 48 patients have survived an average of 7.6 months. Variceal bleeding recurred in 10 patients. Fifteen patients developed shunt stenosis (n = 6) or occlusion (n = 9). Patency was reestablished in eight of the nine occluded shunts. Seventeen patients developed new or worsened encephalopathy. The authors conclude that TIPS creation is an effective and reliable means of lowering portal pressure and controlling variceal bleeding, particularly in patients with acute variceal bleeding unresponsive to sclerotherapy and patients with chronic variceal bleeding before liver transplantation.  相似文献

The multiphase alloy MP35N (35% Ni, 35% Co, 10% Mo, 20% Cr) is a high strength, high toughness alloy of choice for several safety-critical applications in aerospace, oil drilling, and biomedical industries. Several previous attempts in literature to measure the plane strain fracture toughness of commercially drawn MP35N did not produce reliable values since they violated one or more of the criteria stipulated by ASTM standards for a valid measurement of KIc. In most cases, the requirements for plane strain and small scale yielding conditions were not met, since the commercially drawn material was available only with limited cross-sectional dimensions. In this investigation specially designed specimens (modified compact tension (CT) specimens) have been used to measure the plane strain fracture toughness of MP35N in both the unaged and the aged conditions. The KIc of the commercially drawn (53% reduction level) MP35N was measured to be 126 MPa√m, while that of the commercially drawn and aged MP35N was measured to be 98 MPa√m. Both these measurements satisfied all the required criteria stipulated by ASTM standards for valid measurements of KIc. The new procedure used in this study has been verified by the measuring of fracture toughness of Al alloy, using both the modified specimen, as designed here, and the standard one. The results for plane strain fracture toughness of MP35N alloy have been verified by the standard measurement of JIc values for both the aged and the unaged alloys. Finally, on a suitably normalized plot, introduced in this paper, the toughness-strength envelop for MP35 is higher than most of structural alloys, but significantly lower than that of the TRIP steels.  相似文献

In a study of 110 Pseudomonas cepacia isolates from patients without cystic fibrosis, the in vitro potencies of three new compounds, meropenem, PD 127391, and PD 131628, were comparable to those of ceftazidime and ciprofloxacin and exceeded those of chloramphenicol and co-trimoxazole. The MICs of ceftazidime, ciprofloxacin, meropenem, and the PD compounds for 90% of strains tested were < or = 4 micrograms/ml, whereas they were 32 micrograms/ml for chloramphenicol and co-trimoxazole. Data for 20 isolates from patients with cystic fibrosis indicated that the isolates were less susceptible to all seven antibiotics tested, with the most active compounds being meropenem and PD 127391.  相似文献

Polyomavirus transforms cells in culture and induces tumors in mice without apparent interaction with or inactivation of the p53 tumor suppressor protein. In this report we investigate the ability of polyomavirus T antigens to overcome the growth suppression function of p53. A temperature sensitive p53 gene was introduced into mouse embryo fibroblasts derived from a p53 null mouse, resulting in expression of a protein with a mutant conformation at 37 degrees C and a functionally wild-type conformation at 32 degrees C. We found that expression of p53 at 32 degrees C induced the cyclin-dependent kinase inhibitor p21/WAF1 and arrested cell growth in the G1/G0 phase of the cell cycle. Only the under-phosphorylated form of the retinoblastoma tumor suppressor protein (pRB) was detected in these growth arrested cells. We introduced both polyomavirus large T (LT) and middle T (MT) antigens into this cell line and showed that LT overcame p53-dependent growth arrest, while MT did not. In cells grown at 32 degrees C, LT expession led to cell proliferation and phosphorylation of pRB in the presence of p21. A mutant LT containing a defective pRB binding domain failed to overcome the growth arrest, indicating that interaction of LT with RB proteins is required to override p53 function. Although the polyomavirus T antigens do not interact with p53 directly, our results indicate that the virus, through LT, is able to interfere with the growth suppressive activity of p53.  相似文献

Actinobacillus actinomycetemcomitans, a gram-negative, capnophilic bacterium, is associated with several human diseases and is the suspected etiologic agent in certain forms of periodontal disease. We have previously shown that this organism produces an immunosuppressive factor (ISF) which is capable of inhibiting both T- and B-cell activation. Furthermore, these effects appear to be associated with the activation of a population of suppressor cells. We now report that the ISF induces a unique population of CD4+CD8+ dual-positive T-cells. By utilizing multiparameter flow cytometric analysis, we were able to detect the presence of dual-positive cells in cultures of human T-cells treated with PHA and ISF. The cells appeared within 48 hr and their induction was dependent upon the presence of both CD4 and CD8 cells in the culture. Dual expression of CD4 and CD8 was stable in that the cells continued to express both surface proteins after being sorted and cultured for an additional 24 hr. Phenotypic analysis indicates that these cells are also CD3+, CD2+, CD5+, TCR alpha beta+, CD45RA+ (and RO+), and CD29+. The dual-positive cells express surface markers associated with T-cell activation: CD25+, CD69+, CD71+, and HLA-DR+. In contrast, the cells were negative for CD34, CD57, CD56, and CD16. Cell cycle analysis indicates that > 80% of the dual-positive cells were in the S phase. Finally, functional analysis of these cells indicates that they are capable of suppressing the proliferative response of autologous T-cells to PHA.  相似文献
