It is ten years since the Cartwright report was published, three years since the code of health and disability services consumers' rights was promulgated and two years since the code came into force which makes it due for review next year. This paper reviews the issues identified by Cartwright and the effect that her investigation into the "Unfortunate Experiment" has had on the provision of health services. Issues of clinical freedom, peer supervision and informed consent are discussed in relation to the Health and Disability Commissioner Act, the Privacy Act and concepts of partnership in health care. Some comments on the present state of the relationships between consumers and providers in the health system are included.  相似文献

A detailed microstructural study has been done on Au-Pd-Zn ohmic contacts to p-type InGaAsP epitaxially grown on InP. The doping level in the InGaAsP was 1.0 × 1019 to 1.5 × 1019 cm–3 near the surface with the Zn concentration graded to a value of 7 × 1018 cm–3 at the InGaAsP-InP interface. Metal layers (10 nm Pd,3 nmZn, 25 nm Pd and 50 nm Au) were deposited sequentially by electron beam evaporation. Contact resistances less than 10–4 cm2 were achieved for all annealing temperatures studied (380–440 C) and a minimum contact resistance of 2 × 10–6 cm2 was obtained for an anneal at 400 C for 20 s. Comparisons were made to similar metallizations on p-type InP. Lower contact resistances were achieved for the quaternary material compared with the binary material, however, contact stability and uniformity were worse.  相似文献

The concern with nursing research in the future work of nurse graduates. Expectations from the viewpoint of the nurse researcher. Keeping in mind the concerns which occupy nurse researchers now and in the future, expectations are raised of the graduates of degree programmes in nursing science which are now also being established in the Federal Republic of Germany. This takes into account the structurally determined areas of self determined action in new fields of professional practice. Nurse graduates will be seen as "change agents" in their future areas of work and will have three essential aspects of involvement with nursing research: 1. they themselves will conduct research 2. they will receive, put into practice and transmit results of nursing research 3. they will initiate and commission nursing research projects. According to the broad topics of nursing research, requirements for nurses to act as professionals in their new working fields are formulated referring to the given structural conditions in Germany.  相似文献

The most interesting places can often be the most dangerous ones, and space exploration is no exception. The dynamic surfaces of comets, the turbulent atmospheres of the gas giants, and the hypothesized subsurface ocean of Europa all call for exploration, and are all very risky for on-site robotic explorers. The Casper (continuous activity, scheduling, planning, execution, and replanning) software system provides critical reasoning capabilities to robotic explorers. To show how it works, we first describe Casper's internal workings and then look at the Three Corner Sat and the Autonomous Sciencecraft Constellation missions, highlighting Casper's contributions to each.  相似文献

A review of the American Cancer Society's statistics for colorectal cancer indicates that there has been little improvements in the survival rate for this disease in the past 25 years. Although there have been advances in surgical techniques, radiation therapy, and chemotherapy, the key to improved survival rates is earlier diagnosis. A significant percentage of patients continues to present with regional or distal metastasis at the time of their initial diagnosis. Both proctosigmoidoscopy and guaiac impregnated filter slide paper methods have been productive in diagnosing this disease at an earlier stage. The "Hemoccult" test, however, is inexpensive, can be used on a routine basis, is easier for patients to perform themselves, and is aesthetically pleasing. It represents a significant cost savings compared to proctosigmoidoscopy and should be advocated for routine use in all patients over 40 years of age. Although there continues to be a role for local and regional mass screening programs, significant improvements in colorectal survival rates cannot be expected until routine application of this type of screening is practiced.  相似文献

A study of the impact of machine-paced (M/P) and self-paced (S/P) work on job satisfaction of 28 female industrial assembly workers was evaluated in which M/P work was confounded with simplified work and the S/P job was confounded with enriched tasks. Results indicated the following:

1. 1. Over three-quarters of workers were more satisfied in S/P jobs, while only less than one-quarter were more satisfied in M/P jobs.

2. 2. The 16PF personality test effectively predicts (0·88 multiple correlation) the satisfaction ratios of M/P to S/P jobs.

A new approach is presented for the optimization of steel lattice towers by combining genetic algorithms and an object-oriented approach. The purpose of this approach is to eliminate the difficulties in the handling of large size problems such as lattice towers. Improved search and rapid convergence are obtained by considering the lattice tower as a set of small objects and combining these objects into a system. This is possible with serial cantilever structures such as lattice towers. A tower consists of panel objects, which can be classified as separate objects, as they possess an independent property as well as inherent properties. This can considerably reduce the design space of the problem and enhance the result. An optimization approach for the steel lattice tower problem using objects and genetic algorithms is presented here. The paper also describes the algorithm with practical design considerations used for this approach. To demonstrate the approach, a typical tower configuration with practical constraints has been considered for discrete optimization with the new approach and compared with the results of a normal approach in which the full tower is considered.  相似文献

Multifrequency tympanometry (MFT), using probe frequencies ranging from 226-2,000 Hz, was performed on normal chinchillas to obtain normative data against which to compare results from animals with middle ear pathology. A series of validating experiments was conducted to determine the effects of anatomical alterations of the middle ear on MFT. These included artificially extending the ear canal, opening the bulla, injecting saline into the middle ear, and disrupting the ossicular chain. The results indicate that MFT characteristics of chinchilla ears are qualitatively similar to those observed in normal humans and patients with middle ear disease, and MFT provides information that is not available from the 226-Hz tympanogram.  相似文献

The taxometric method effectively distinguishes between dimensional (1-class) and taxonic (2-class) latent structure, but there is virtually no information on how it responds to polytomous (3-class) latent structure. A Monte Carlo analysis showed that the mean comparison curve fit index (CCFI; Ruscio, Haslam, & Ruscio, 2006) obtained with 3 taxometric procedures—mean above minus below a cut (MAMBAC), maximum covariance (MAXCOV), and latent mode factor analysis (L-Mode)—accurately identified 1-class (dimensional) and 2-class (taxonic) samples and produced taxonic results when applied to 3-class (polytomous) samples. From these results it is concluded that using the simulated data curve approach and averaging across procedures is an effective way of distinguishing between dimensional (1-class) and categorical (2 or more classes) latent structure. The anti-metastatic effect of Z-100, an immunomodulatory arabinomannan extracted from Mycobacterium tuberculosis, was investigated in mice bearing B16 melanoma cells. Treatment of BF10 mice implanted with high metastatic B16F10 melanoma cells with a 10 mg/kg dose of Z-100 resulted in the reduction of experimental pulmonary metastasis as compared with that of BF10 mice treated with saline. The number of pulmonary metastatic colonies in BF1 mice (mice implanted with low metastatic B16F1 melanoma cells) was greatly increased after the inoculation of CD4+ CD11b+ CD281+ TCR alphabeta+ type 2 T cells (F10-Th2 cells) derived from BF10 mice, while only a few metastatic colonies were demonstrated in lungs of BF1 mice inoculated with naive CD4+ T cells. However, the numbers of metastatic colonies in BF1 mice were not increased when they were inoculated with the F10-Th2 cell fraction derived from Z-100-treated BF10 mice and the generation of F10-Th2 cells in BF10 mice was effectively suppressed by the Z-100 treatment. These results suggest that Z-100 inhibits pulmonary metastasis of B16 melanoma through the regulation of tumor-associated Th2 cells, which are a key cell in the acceleration of tumor metastasis.  相似文献
