Histone-histone interactions. I. An electrophoretic study

Whole histone extracted from chromatin by either acid or protamine displacement was found by gel electrophoresis at pH7 to contain only two histone complexes, H2A-H2B and H3-H4, and uncomplexed histone H1. Although both complexes are dissociated at low pH or with high urea concentrations, removal of the denaturant resulted in complete complex reformation within minutes at the most. A syntematic investigation of binary, ternary and quaternary histone mixtures revealed that interactions also occur between histones H2B-H4 and H2A-H4. No evidence however was found for the formation of ternary and quaternary histone complexes.     

Toxicity following methoxyflurane anaesthesia. IV. The role of obesity and the effect of low dose anaesthesia on fluoride metabolism and renal function

Seven obese and five normal weight patients were studied before, during and after one hour of methoxyflurane-nitrous oxide anaesthesia during peripheral surgical operations and compared with eight patients of normal weight anaesthetized with nitrous oxide-meperidine and d-tubocurare. Estimates were made of renal function, including serum and urinary electrolytes, osmolarity, uric acid, urea and creatinine. Renal clearances for the latter three substances were also calculated. Serum and urinary inorganic and organic fluoride concentrations were measured, as were renal clearances. This low dose methoxyflurane anaesthesia resulted only in a decrease in uric acid clearance among all the measures, when compared to the meperidine-nitrous oxide controls. The clearance of uric acid remained depressed for longer in the obese patients, but otherwise they did not differ from the normal weight patients. It is possible but not proven that depressed uric acid clearance may be related to the organic fluoride metabolite and an early indicator of methoxyflurane renal toxicity. The previously documented biotransformation of methoxyflurane was seen in this study. A double peak in serum inorganic fluoride was shown in all patients but one. Rather large differences in peak levels of serum inorganic fluoride occurred. The only significant difference between the obese and normal weight patients as far as fluoride metabolism was concerned was a greater variability in the serum inorganic fluoride levels in the obese patients. It would appear that the obese patient metabolizes methoxyflurane in a quantitatively if not qualitatively different fashion than the normal weight patient, perhaps because of fatty infiltration of the liver. Caution is advised in the use of methoxyflurane for more than 90 minutes of low concentration administration in view of the unpredictability of the biotransformation.     

X-linked mental retardation with a fragile site in Xq and an inversion of chromosome no. 9

The second black male with X-linked mental retardation and a fragile site at Xq27 was ascertained by screening for macroorchidism. GTG banding revealed the presence of an inv(9)(p13q21). This inversion is thought to be a chance occurrence and is probably of no clinical significance. More cases having a marker Xq and a chromosome abnormality are expected; some of these abnormalities will be clinically significant.     

Schwannoma of the brain stem: case report

Management of diabetes demands that informed patients cooperate with knowledgeable and concerned health professionals in a planned system of health care. It is imperative that each of the numerous health professionals caring for these patients has an understanding of his/her specific role and responsibilities as it relates to the entire health care team and to the individual patient. Acute, in-depth, and continuing phases of diabetes education are briefly described and the importance of altering patient behavior by contracting is documented.     

Fracture Toughness and Microcracks on Natural Apophyllite Crystals

Results on fracture toughness of natural apophyllite crystals when subjected to impact are presented. The morphology of the cracks and fracture has been studied. It is found that the crack formation does not show any anisotropic property and remains the same whatever may be the orientation of the crystal. The average values of the fracture toughness KC and brittleness index, as determined from measurements of crack length, are estimated to be 0.107 gμm-3/2and 4.19μm-1/2 respectively for the a(100) plane. The yield strength forα(100), p(111) and c(001) planes of apophyllite crystals for lower loads is 2250. 2160 and 1840 MPa and for higher loads is 1510, 1600 and 1430 MPa respectively     

Adverse drug reactions in pediatrics with a study of in-hospital intensive surveillance

A two-part prospective study of adverse drug reactions (ADRs) in Indian children was carried out at a teaching general hospital. Using an in-hospital intensive surveillance scheme (IISS) for the detection of ADRs, indoor patients of one of the two units in the pediatric ward were monitored daily for 6 months, with the other unit serving as a control group. A total of 347 patients were monitored, 2781 daily orders written and 24,474 doses of 96 different drugs given. Six patients suffered from ADRs (1.73%), and 1 reaction proved fatal (0.29%), while the control group reported only 1 ADR in the same time period. The frequency of ADRs (p < 0.001) and their resultant mortality in Indian children was less than that in a western prototype study. Though IISS showed a marked increase in ADR reporting, it was too cumbersome for routine use in our country. In the second part of the study, 40 cases of ADRs seen over 2 years were analyzed. Antimicrobials, especially sulphonamides, accounted for a high percentage of cases mostly as skin rashes and fairly severe reactions were common. Patients on anti-tuberculous and anti-convulsant drugs required prolonged supervision for late onset reactions.
