Radiographic evaluation of complications after jejunoileal bypass surgery

A retrospective analysis of 89 patients who underwent jejunoileal bypass surgery for morbid obesity disclosed 33 complications that were detected radiographically. Intestinal obstruction (10.1% of patients), cholecystitis (5.6%), renal stones (4.5%), peptic ulcer (3.4%), megacolon (6.7%), and elongation of the small intestine with hypertrophy of the mucosal folds of the jejunum (6.7%) were diagnosed solely by radiographic means.     

Automated direct antimicrobial susceptibility testing of microscopically screened urine cultures

Two screening methods for urine microbiology are proposed: one in which the Gram-stained smear is used to detect significant bacteriuria, and another in which Autobac antibiotic susceptibility tests are performed directly on positive urine samples. Results on 1,350 specimens indicated that an average of 18 bacteria per oil immersion field were observed in the urine of patients with significant bacteriuria, and an average of less than 1 bacterium per oil immersion field was found in the urine of patients without significant bacteriuria. Direct susceptibility testing by Autobac proved to be rapid (3 h versus 24 h) and reliable (0.5 to 1.2% discrepancies).     

Oral and intramuscular phenytoin

After presenting the topographical and pathological basis for the habitual luxation of the shoulder joint, our experience with the Bankart-Müller operation on 38 of our own patients was described. The results are characterized as very good, particularly two years and longer after the operation.     

The response of transplants of cultured fetal mouse ovaries in kidneys of ovariectomized adult mice

Whole ovaries from 16-day fetal mice were cultured for 6 to 20 days and then transplanted to the kidneys of ovariectomized adult mice where they remained for one to four weeks. After three weeks in the host's kidneys, many follicles developed within the transplants and became vesiculated. Many of the oocytes within these follicles had formed the first metaphase spindle of meiosis and several had completed the first polar body. Host mice bearing transplants that contained vesiculated follicles showed uterine stimulation and keratinization of their vaginae. However, ovaries that had been in culture for more than ten days before transplantation showed a limited response to the gonadotropins and never matured sufficiently to stimulate the host's reproductive tract. No ovulations occurred in any of the transplants.     

Indium-111 chloride scanning for identification of experimental abscesses

A review of all granulomatous surgical lesions in the files of the Free University Hospital revealed four cases of histoplasmoma in which Grocott-staining procedures provided histologic confirmation of the presence of Histoplasma capsulatum bodies. The case histories have been presented briefly. A short review of the literature follows.     

Optic disk vasculitis

A 44-year-old woman developed progressive loss of vision associated initially with a swollen optic disk, and later with optic atrophy and a diffuse retinal vasculopathy, which caused extensive retinal hemorrhagagic. Histopathologic examination showed hemorrhagagic infarction of the retina, as well as infarction of the anterior optic nerve. In the optic nerve, the central retinal vessels showed extensive phlebitis and occlusion of many small arterioles.     

Posthyperventilation apnea associated with severe hypoxemia

We studied a 14-year-old girl who suffered fractures of her mandible and tegmen following a fall from a balance beam. Thirteen days after hospitalization, she developed severe, protracted, recurrent episodes of hyperventilation; subsequently, she suffered posthyperventilation apnea, which at times was prolonged and association with severe hypoxemia with an arterial oxygen pressure as low as 25 mm Hg. The patient was treated with added dead space and chlorpromazine hydrochloride (Thorazine). Postulated mechanisms for her disorder are discussed. The importance of close clinical and laboratory observation in similar cases is stressed.     

Non-Mendelian inheritance of mosquito susceptibility to infection with Brugia malayi and Brugia pahangi

The mode of inheritance of susceptibility or refractoriness of insect vectors to medically important pathogens such as those causing malaria or filariasis is usually believed to follow normal Mendelian laws and to involve a single pair of alleles. In this report, experiments are described that demonstrate another mode of inheritance of mosquito susceptibility to filarial parasites. Crosses were made between susceptibile and refractory species of the Aedes Scutellaris complex, and the hybrid and backcross progeny were tested for susceptibility to infection by Brugia malayi and Brugia pahangi. The data indicate that inheritance follows a non-Mendelian pattern indicative of extrachromosomal factors inherited through the maternal parent.
