We have previously documented the safety of 1 unit (50 gram) of human polymerized hemoglobin (Poly SFH-P) in healthy volunteers. This report describes the first patient trial to assess the therapeutic benefit of Poly SFH-P in acute blood loss. Thirty-nine patients received 1 (n = 14), 2 (n = 2), 3 (n = 15), or 6 (n = 8) units of Poly SFH-P instead of red cells as part of their blood replacement after trauma and urgent surgery. There were no safety issues related to the infusion of Poly SFH-P. The plasma hemoglobin concentration ([Hb]) after the infusion of 6 units (300 gram) of Poly SFH-P was 4.8 +/- 0.8 g/dL (mean +/- SD). Although the red cell [Hb] fell to 2.9 +/- 1.2 g/dL, the total [Hb] was maintained at 7.5 +/- 1.2 g/dL. Poly SFH-P maintained total [Hb], despite the marked fall in red cell [Hb] due to blood loss. The utilization of O2 (extraction ratio) was 27 +/- 16% from the red cells and 37 +/- 13% from the Poly SFH-P. Twenty-three patients (59%) avoided allogeneic transfusions during the first 24 hours after blood loss. Poly SFH-P effectively loads and unloads O2 and maintains total hemoglobin in lieu of red cells after acute blood loss, thereby reducing allogeneic transfusions. Poly SFH-P seems to be a clinically useful blood substitute.  相似文献

Barley β‐amlyase was immobilized on two polymeric materials; poly(acrylamide–acrylic acid) resin [P(AM‐AAc)] and poly(acrylamide–acrylic acid–diallylamine–HCl) resin [P(P(AM‐AAc‐DAA‐HCl) using two different methods: covalent and cross‐linking immobilization. Thionyl chloride, used to activate the polymers for covalent immobilization, has the advantage that it is able to react with a number of surface groups of protein under very mild conditions. Cross‐linking with glutaraldehyde gave a higher coupling yield (approximately 70%) than covalent immobilization (approximately 20%). The activity and stability of the resulting biopolymers have been compared with those of free β‐amylase. The specific activity of the immobilized enzyme was significantly influenced by the amount of enzyme loaded onto the polymers, the optimal level being 3.5 mg g?1 polymer. It was found that the immobilized β‐amylase stored at 4°C retained approximately 90% of its original activity after 30 days, whereas free β‐amylase stored in solution at 4°C retained only 47% of its activity after same period. The difference in long term stability was more significant when the enzyme was stored at room temperature; the immobilized enzyme maintained 40% of its activity after 30 days, whereas the residual activity of free enzyme was only 10%. Nitric oxide (NO) is produced by a variety of cells within the respiratory tract, including inflammatory epithelial cells. NO has been detected in the exhaled air of normal human subjects, and its concentration is raised in asthmatic patients. To study whether exhaled NO arises from the respiratory tract, we administered a NO synthase (NOS) inhibitor, NG-monomethyl-L-arginine (L-NMMA), by inhalation (490 mg) in a double-blind randomized manner in nine normal and six asthmatic subjects. Because exhaled NO may arise from an inducible isoform of NO synthase that may be inhibited by glucocorticosteroids, we also studied the effects of oral prednisolone (30 mg orally for 3 d) in seven normal and six asthmatic subjects in a separate double-blind crossover study with matched placebo. After nebulized L-NMMA, there was a significant fall in peak exhaled NO compared with saline control values, with a mean fall of 43.6 +/- 5.6% in normal subjects (p < 0.01) and of 39.7 +/- 6.5% (p < 0.01) in asthmatic subjects, which persisted for 4 h. There were no effects of L-NMMA inhalation on heart rate, blood pressure, or FEV1 in either normal or asthmatic patients. Administration of oral prednisolone (30 mg) resulted in a fall in exhaled NO concentrations in asthmatic subjects by 21.6 +/- 5.0% at 48 h (p < 0.01) but no significant change in normal subjects. These data suggest that NOS inhibitors may be safely given in normal and asthmatic patients and that the increased exhaled NO seen in asthmatic patients is likely to be caused by induction of inducible NOS.  相似文献

An electrophysiological probe technique was used to ascertain whether the same attentional mechanisms are employed for both the detection of simple visual features and the discrimination of conjunctions of features. Visual search arrays containing 14 grey items and 2 colored items were presented; one color was designated relevant for each trial block. Subjects were required to report the presence or absence of the relevant color (feature detection condition) or the shape of the relevantly-colored item (conjunction discrimination condition). Shortly after the onset of the search array, a task-irrelevant probe stimulus was flashed at the location of the relevant or irrelevant color and the event-related potential (ERP) produced by this stimulus was used to assess sensory processing at the probed location. Probes presented at the location of the relevant color were found to elicit enhanced ERP components and probes presented on the opposite side of the display from the relevant color elicited suppressed components. These effects were observed in both the detection and discrimination conditions, indicating that spatially restricted attentional processes are used for both the detection of simple features and the discrimination of conjunctions. However, one ERP component (the PI wave) exhibited these effects in the discrimination condition but not in the detection condition, indicating that conjunction discrimination utilizes additional attentional processes beyond those required for feature detection.  相似文献

Histological and histochemical techniques have been employed to determine the structure and autonomic innervation of paraganglia located in the human pelvis. In foetal and early postnatal tissues, paraganglia formed rounded cellular masses which were frequently in company with the autonomic nerves and ganglia of the urinary bladder and other pelvic viscera. The constituent cells contained only small amounts of cholinesterase and were unrelated to enzyme positive autonomic nerves; acetylcholinesterase containing nerves were occasionally observed in the capsule and the fibrous septa of the pelvic paraganglia. In early postnatal specimens, pelvic paraganglia frequently contained single or multiple "pacinian-like" corpuscles, each possessing a central region which was rich in both acetyl and pseudocholinesterase. These structures were rarely observed within autonomic ganglia and were absent 4 1/2 years post partum. By means of a histochemical technique, pelvic paraganglia were found to contain catecholamine which was attributed to the presence of relatively large quantities of noradrenaline. These observations have been discussed with particular reference to the results of other studies on the autonomic innervation of paraganglia.  相似文献

在重复率脉冲固体激光器中,由于储能电容需要频繁的充放电,通常采用谐振充电电路以适应其负载的大范围变化。为此研制了开关频率为20kHz,充电电流为2.1A,最高充电电压为1.86kV的半桥串联谐振充电电路。该电路工作于电流断续模式,开关管的开通和关断均为软开关。通过对周期电压、电流递推公式的分析,表明该工作模式中每个周期的平均充电电流均为恒定。应用递推公式计算稳态和暂态谐振电容电压和电流,结果显示暂态最大工作电压是稳态最大电压的2倍,暂态最大电流为稳态最大峰值电流的1.5倍,为谐振电容和开关管的选取提供了依据。  相似文献

为了适应联机分析处理(OLAP)系统中实时数据高性能分析需求不断提高的需求,提出一种能够适合Spark环境并结合多维Bloom Filter(MDBF)的星型连接算法SMDBFSJ。首先,根据多个维表构建MDBF,利用其占用空间小的特点,广播到所有节点;然后,在本地节点完成事实表过滤操作,事实表不需要在节点间移动数据;最后,过滤后的事实表与维表采用重划分方式进行连接,进而得到最终结果。SMDBFSJ算法避免了事实表数据移动,通过MDBF减小了需要广播的数据量,充分结合了广播连接和重划分连接的优势。实验结果表明了该算法的有效性,在单机和集群环境下,该算法相比重划分连接均获得了3倍左右的性能提升。  相似文献
