STUDY DESIGN: Cadaveric anatomic and in vivo survival animal studies were performed to develop a new arthrodesis technique for the lumbar spine. OBJECTIVES: To examine the feasibility, efficacy, and safety of a minimally invasive lumbar intertransverse process arthrodesis technique using an osteoinductive growth factor (rhBMP-2) delivered in a collagen sponge carrier. The technique was first developed using a rabbit model and modified for the nonhuman primate (rhesus monkey), a larger animal with the most similar bone biology to humans. SUMMARY OF BACKGROUND DATA: The morbidity of conventional posterolateral lumbar intertransverse process arthrodesis includes graft donor site morbidity; paraspinal muscle devascularization, denervation, and scarring and nonunion in up to 36% of patients. Minimally invasive anterior lumbar interbody arthrodesis techniques have been developed, but these often require a metal implant and carry risks to major vessels and development of retrograde ejaculation. A minimally invasive technique for posterolateral intertransverse process arthrodesis has not been described previously. METHODS: In Part 1, we examined seven New Zealand white rabbits and five rhesus monkeys at necropsy and during nonsurvival surgeries to determine the best access routes and to develop special instruments for video-assisted lateral intertransverse process arthrodesis. In Part 2, 38 New Zealand white rabbits underwent L4-L5 intertransverse process arthrodesis: the "OPEN" group (n = 16) underwent a standard open muscle-splitting approach using rhBMP-2 (bone morphogenetic protein) and collagen as a bone graft substitute; the "video-assisted control" group (n = 6) underwent video-assisted lateral intertransverse process arthrodesis with the collagen implant only (no growth factor); and the "video-assisted-BMP" group (n = 16) underwent video-assisted lateral intertransverse process arthrodesis with rhBMP-2 and collagen as the graft material. In Part 3, rhesus monkeys (n = 4) underwent video-assisted lateral intertransverse process arthrodesis using rhBMP-2 and collagen after laminectomy of L4-L5. RESULTS: In Part 1, we identified expedient, minimally invasive routes to the intertransverse process interval appropriate for each species: an intermuscular approach for the rabbit and a lateral, extramuscular approach for the rhesus monkey. In Part 2, all rabbits in the OPEN and video-assisted-BMP groups achieved solid intertransverse process lumbar fusions when assessed at 10 weeks. There were no neurologic impairments nor any difference between the two groups in the frequency of postoperative infection or other complications. None of the animals in the video-assisted control group showed evidence of fusion. In Part 3, exposure, decortication and grafting with rh-BMP-2 and collagen was accomplished successfully in all four monkeys through the video-assisted minimally invasive approach without complications. CONCLUSION: Video-assisted lateral intertransverse process arthrodesis is a feasible, effective, and safe method of lumbar spinal fusion in the rabbit and rhesus monkey. Use of this arthrodesis procedure will minimize the morbidity of paraspinal muscle denervation and devascularization seen with open intertransverse process fusion techniques, and the use of an osteoinductive growth factor will eliminate the problem of graft donor site morbidity and possibly increase the chances for successful fusion.  相似文献

Combined analysis of multiple data sources has increasing application interest, in particular for distinguishing shared and source-specific aspects. We extend this rationale to the generative and non-parametric clustering setting by introducing a novel non-parametric hierarchical mixture model. The lower level of the model describes each source with a flexible non-parametric mixture, and the top level combines these to describe commonalities of the sources. The lower-level clusters arise from hierarchical Dirichlet Processes, inducing an infinite-dimensional contingency table between the sources. The commonalities between the sources are modeled by an infinite component model of the contingency table, interpretable as non-negative factorization of infinite matrices, or as a prior for infinite contingency tables. With Gaussian mixture components plugged in for continuous measurements, the model is applied to two views of genes, mRNA expression and abundance of the produced proteins, to expose groups of genes that are co-regulated in either or both of the views. We discover complex relationships between the marginals (that are multimodal in both marginals) that would remain undetected by simpler models. Cluster analysis of co-expression is a standard method of screening for co-regulation, and the two-view analysis extends the approach to distinguishing between pre- and post-translational regulation.  相似文献

Stiction, the adhesion of micromachined components to each other or the substrate, decreases production yields and operational lifetimes for MEMS devices. A recent study demonstrated the feasibility of using a Nd:YAG, 1063 nm laser to repair 2 μm thick polycrystalline silicon microcantilevers with lengths up to 1000 μm which had adhered to the substrate. This investigation examines the influence of sample age at the time of laser irradiation on the repair of stiction-failed MEMS structures. The operating conditions for the 1064 nm, Nd:YAG laser included a fluence of 70 mJ/cm2, a repetition rate of 20 Hz, a pulse duration of 20 ns, and an exposure time of 60 s. For samples irradiated on the same day of release, repair yields were 100% for beams up to 500 μm in length. For 1000 μm long beams, the 10- and 30-μm-wide cantilevers had same-day repair yields of 56% and 71%, respectively. The experimental results show that increasing the amount of time the microstructures are adhered to the substrate before laser irradiation decreases the ability to repair stiction-failed devices with delays of longer than 1 month resulting in a decreased repair yield  相似文献

OBJECTIVE: To explore the effects of contrast reduction on younger and older adults' reading behavior and to examine whether readers rely on word predictability to compensate for poor contrast. BACKGROUND: Poor contrast can degrade text and may influence reading behavior. Readers may compensate for visual degradation of text by taking advantage of word predictability. METHOD: In Experiment 1, 5 younger and 5 older adults read sentences presented with 10 levels of contrast. In Experiment 2, 40 younger and 40 older adults read high-, medium-, and low-contrast sentences that varied in target word predictability (high vs. low). RESULTS: Relative to those of younger adults, older adults' reading rates were more slowed by low contrast; comprehension was less influenced by contrast (i.e., not significantly). Older adults read high-predictability words faster and comprehended them better than low-predictability words, significantly so for high- and medium-contrast sentences. Younger adults comprehended high-predictability words significantly better than low-predictability words for high- and low-contrast sentences. CONCLUSION: Low contrast was more detrimental for older adults. High-predictability words benefited older adults by significantly reducing their reading times and benefited all readers by significantly increasing their comprehension. APPLICATION: The current findings demonstrate the benefit of adequate contrast and word predictability for optimal text design to facilitate both reading times and reading comprehension.  相似文献

The generalized Landweber iteration with a variable shaping matrix is used to solve the large linear system of equations arising in the image reconstruction problem of emission tomography. The method is based on the property that once a spatial frequency image component is almost recovered within in in the generalized Landweber iteration, this component will still stay within in during subsequent iterations with a different shaping matrix, as long as this shaping matrix satisfies the convergence criterion for the component. Two different shaping matrices are used: the first recovers low-frequency image components; and the second may be used either to accelerate the reconstruction of high-frequency image components, or to attenuate these components to filter the image. The variable shaping matrix gives results similar to truncated inverse filtering, but requires much less computation and memory, since it does not rely on the singular value decomposition.  相似文献

For successful parasitization, the female Campoletis sonorensis endoparasitic wasp injects a polydnavirus into its host, Heliothis virescens, during oviposition. Viral gene expression induces immunosuppression and alters development of the host. We report here that three abundantly expressed genes, VHv1.1, WHv1.0, and WHv1.6, describes a polydnavirus "cysteine-rich" gene family which may be important in inducing these host manifestations. These genes have a similar primary gene structure and their proteins contain cysteine motifs characteristic of snail ion-channel ligands, the omega-conotoxins. Like the omega-conotoxins, the intercysteine amino acid residues are hypervariable with only three identical amino acids in all motifs. The conservation of this domain in the three viral genes may reflect an important functional role for these viral proteins in the parasitization of H. virescens. The three genes also contain introns similar in sequence at comparable positions in their 5' untranslated leaders and coding sequences. VHv1.1 contains two cysteine motifs, and each motif is interrupted by an intron at the same position as in the cysteine motifs of WHv1.0 and WHv1.6. Intron 2 sequences of WHv1.0 and WHv1.6 are 92% identical, while the immediately flanking exon sequences encoding the cysteine motifs are only 76% identical. This provides an example of nuclear pre-mRNA introns which are more conserved than flanking exons among members of a gene family.  相似文献

This paper describes a new and objective method for tackling the problem of defining a multidisciplinary research area for bibliometric analysis. The test field was cardiovascular biology. A three stage process was adopted in setting a boundary around this research field:
  1. Appropriate sections of a hierarchical subject classification scheme, Medical Subject Headings (MeSH), were developed into a “MeSH filter” through which papers indexed in MEDLINE were screened.
  2. A panel of cardiovascular experts reviewed the core set of classification terms, identifying irrelevant and missing areas, facilitating the development of a more sophisticated “filter”.
  3. The definition was validated using publication lists from research departments with a known interest in cardiovascular research.
This iterative process resulted in a definition of the field which captured basic and clinical research papers from the international biomedical research community and which was recognisable to experts in the field of cardiovascular research. Importantly, the field boundary also excluded publications which were not relevant to cardiovascular research. The process of involving experts in shaping the field definition also yielded two intangible, but key benefits: (a) it lent credibility to subsequent analyses, the results of which were to be presented to policy-makers in cardiovascular biology, and (b) it served to shape consensus among the cardiovascular experts on the full range of scientific disciplines that are relevant to their field. Analysis of international publishing in cardiovascular research revealed that whilst the UK and US dominate in total numbers of papers, the relative emphasis on cardiovascular research in these countries (as a proportion ofall biomedical publishing) is actually quite low, and declining. Japan and Germany in contrast appear to give greater emphasis to cardiovascular research in their national portfolios of biomedical science, and between 1988–1991 Japan established a marked increase in activity.  相似文献

Object identification and tracking applications of pattern recognition at video rates is a problem of wide interest, with previous attempts limited to very simple threshold or correlation (restricted window) methods. New high-speed algorithms together with fast digital hardware have produced a system for missile and aircraft identification and tracking that possesses a degree of ``intelligence' not previously implemented in a real-time tracking system. Adaptive statistical clustering and projection-based classification algorithms are applied in real time to identify and track objects that change in appearance through complex and nonstationary background/foreground situations. Fast estimation and prediction algorithms combine linear and quadratic estimators to provide speed and sensitivity. Weights are determined to provide a measure of confidence in the data and resulting decisions. Strategies based on maximizing the probability of maintaining track are developed. This paper emphasizes the theoretical aspects of the system and discusses the techniques used to achieve real-time implementation.  相似文献
