Point-to-point functional movements involve simultaneous shoulder and elbow joint rotations. In able-bodied subjects these movements are fully automatic, and feed-forward control ensures the synergistic activity of many muscles. Synergy between joint rotations was defined and described as a scaling between joint angular velocities [19]. Similarly, subjects who can control their shoulder movements may be assisted in reaching tasks by functional electrical stimulation (FES) of elbow extensor muscles. The synergistic control paradigm can be implemented in real-time by employing a hierarchically structured production-rules method. The use of production-rules necessitates the acquisition of knowledge and the assembly of a rule-base. A nonparametric technique was designed for the identification of the rules. The identification process was divided into two phases: determination of the scaling parameters, and determination of the stimulation parameters. The scaling parameters, needed for the coordination of movements, were determined in able-bodied subjects. Those depend exclusively on the initial and target positions of the hand. The number of scalings could be reduced by dividing the workspace into 12 zones. The stimulation parameters, needed for the execution of movements, were determined in subjects with paralyzed elbow extensor muscles by identifying triplets: elbow angular velocity, elbow angular acceleration (velocity increments), and the corresponding pulse durations for various classes of movements and loads attached to the hand. 相似文献

Pituitary adenylate cyclase activating peptide and vasoactive intestinal peptide belong to the same neuropeptide family. Both peptides are present in nerve fibers in the gastric wall and are thought to be involved in the regulation of inflammatory processes. Experimental ulcers were induced in the rat gastric mucosa by local application of acetic acid. During the healing process we examined the PACAP and VIP innervation by means of immunohistochemistry and in situ hybridization. The ulcer area was examined from day 1 to day 15 after ulcer induction. There was a marked depletion of PACAP in nerve fibers at the ulcer margin from day 1 and onwards. On day 10 and day 15, PACAP-immunoreactive nerve fibers could again be visualized at the ulcer margin. In contrast, VIP immunoreactive nerve fibers were present at the ulcer margin at all time points studied. From day 10 following ulcer induction PACAP- and VIP- immunoreactive nerve fibers were increased in frequency in the smooth muscle beneath the ulcer. An upregulation of VIP and PACAP mRNA was also demonstrated in the myenteric ganglia adjacent to ulcer. The present results indicate that neuronal PACAP and VIP react differently to the inflammation at the ulcer margin but similarly in the smooth muscle during the ulcer healing. 相似文献

PURPOSE: To evaluate in a prospective study the efficacy of autologous bone marrow transplantation (BMT) in adult patients with acute myelogenous leukemia (AML) in first remission, using a single course of high-dose Cytarabine (HD Ara-C) consolidation therapy as in vivo purging. PATIENTS AND METHODS: Sixty consecutive adult patients with AML in first complete remission (CR) were treated with HD Ara-C consolidation therapy as a method of in vivo purging before marrow collection. High-dose therapy consisted of fractionated total-body irradiation (FTBI) 12 Gy, intravenous etoposide 60 mg/kg, and cyclophosphamide 75 mg/kg, followed by reinfusion of cryopreserved marrow. RESULTS: Sixty patients underwent consolidation treatment with HD Ara-C with the intent to treat with autologous BMT. Sixteen patients were unable to proceed to autologous BMT (10 patients relapsed, one died of sepsis, one developed cerebellar toxicity, two had inadequate blood counts, and two refused). Forty-four patients underwent autologous BMT and have a median follow-up time of 37 months (range, 14.7 to 68.7) for patients who are alive with no relapse. The cumulative probability of disease-free survival (DFS) at 24 months in the intent-to-treat group is 49% (95% confidence interval [CI], 37% to 62%) and in those who actually underwent autologous BMT is 61% (95% CI, 46% to 74%). The probability of relapse was 44% (95% CI, 31% to 58%) and 33% (95% CI, 20% to 49%) for the intent-to-treat and autologous BMT patients, respectively. CONCLUSION: This approach offers a relatively high DFS rate to adult patients with AML in first CR. The results of this study are similar to those achieved with allogeneic BMT. 相似文献

Typhoid fever is often associated with abnormal liver biochemical tests, but severe hepatic involvement with a clinical feature of acute hepatitis is a rare complication. There have been more than 150 cases of salmonella hepatitis reported from both developed and developing countries. The documented incidence varies widely from less than 1% to 26% patients with enteric fever. The possible associated factors for development of salmonella hepatitis are virulence of the organisms, delayed treatment and poor general health of the patients. The pathogenesis of severe hepatic involvement in salmonella infection may be multifactorial and includes endotoxin, local inflammatory and/or host immune reactions. Clinical jaundice in salmonella hepatitis usually occurs within the first 2 weeks of the febrile illness. Hepatomegaly and moderate elevation of transaminase levels are common findings. Extreme hepatic dysfunction with hepatic encephalopathy is a rare coexisting complication in salmonella hepatitis. A positive culture for salmonella from blood or stool is essential to differentiate salmonella hepatitis from other causes of acute hepatitis. Hepatic pathology is characterized by the presence of typhoid nodules with marked hyperplasia of reticuloendothelial cells. The prognosis is usually good as salmonella hepatitis responds well to a specific antibiotic therapy and juandice resolves with clinical improvement. The clinical course can be severe with a mortality rate as high as 20%, particularly with delayed treatment or in patients with other complications of salmonella infection. The medical management of Crohn's disease has changed in recent years, but the mainstay of treatment is still prednisone. A substantial fraction of steroid-treated patients are refractory to therapy and addition of azathioprine or methotrexate has a corticosteroid-sparing effect and increases duration of remission. Controlled ileal release budesonide (9 mg daily) induces clinical remission in 60-70% of patients with Crohn's ileitis or right-sided colitis, and continued budesonide treatment has a finite effect on the duration of remission. The efficacy of mesalazine in active Crohn's disease is limited and high doses are required (4000 mg/day). Six phosphate rocks (PRs) of varying reactivities were compared with monocalcium phosphate (MCP) in a glasshouse experiment growing perennial ryegrass (Lolium perenne cv. Nui) as the test plant on four soils of contrasting P sorption capacity and exchangeable Ca. The cumulative dry matter yield over 10 harvests showed a significant response to P application in all soils. Based on relative yield and P uptake, MCP was the most effective P fertilizer followed by the reactive phosphate rocks, which were superior to the unreactive rocks in all soils. The relative agronomic effectiveness (RAE) and substitution ratio (SR) of individual PR fertilizers, calculated with respect to MCP using the methods of vertical and horizontal comparison, respectively, were similar over a range of fertilizer rate. There was a decline or slight increase in the performance of PRs with time in the low P sorption soils but a consistent increase in the high P sorption soils. Some initial influence of exchangeable Ca content of the soils on the relative performance of PRs was also observed. The objective of this work was to develop and evaluate a soil test suitable for estimating the phosphorus status of soils whether they were fertilized with soluble or sparingly soluble P fertilizers or both. Four New Zealand soils of contrasting P sorption capacity and exchangeable Ca content were incubated alone or with monocalcium phosphate (MCP), reactive North Carolina (NC) phosphate rock or unreactive Florida (FRD) rock, at 240 mg P kg–1 soil, to allow the P sources of different solubilities to react with each soil and provide soil samples containing different amounts of extractable P, Ca and residual phosphate rock. The phosphorus in the incubated soils was fractionated into alkali soluble and acid soluble P fractions using a sequential extraction procedure to assess the extent of phosphate rock dissolution. Eight soil P tests [three moderately alkaline — Olsen (0.5M NaHCO3) modified Olsen (pretreatment with 1M NaCl) and Colwell; three acid tests — Bray 1, modified Bray 1 and Truog; and two resin tests — bicarbonate anion exchange resin (AER) and combined AER plus sodium cation exchange resin (CER)] were assessed in their ability to extract P from the incubated soils.The 0.5M NaHCO3 based alkaline tests could not differentiate between the Control and FRD treatments in any soil nor between the Control, NC and FRD treatments in the high P sorption soils. The acid extractants appeared to be affected by the P sorption capacity of the soil probably because of reabsorption of dissolved P in the acid medium. The AER test gave results similar to Olsen. Only the combined AER + CER test extracted P in amounts related to the solubility of the P sources incubated with each soil. Furthermore, when soil samples were spiked with FRD and NC and extracted immediately, the P extracted by the AER + CER test, over and above the control soils, increased with the amount and chemical reactivity of the rocks. There was no extraction of rock P by any of the alkaline extractions.Increases in the amounts of P extracted (P) by each soil test from the fertilized soils, over and above the control soils were compared with the amounts ofP dissolved from the fertilizers during incubation (measured by P fractionation). Soil P sorption capacity had least influence on the amounts of P extracted by the AER + CER and Colwell tests. However, the Colwell test was unable to differentiate between all P sources in all four soils and suffered from the disadvantage of producing coloured extracts. The AER + CER test appeared to have the potential to assess the available P status of soils better than the other tests used because of its ability to extract a representative portion of residual PR (in accordance with the amount and reactivity) and dissolved P, and thus to differentiate between fertilizer treatments in all four soils. 相似文献

Kjeldahl nitrogen (N), total N and forms of inorganic N (ammoniacal (NH4)-N, nitrate (NO3)-N and nitrite (NO2)-N) were measured in a range of animal manures. The manures include fresh samples of poultry manure, sheep manure, horse manure, dairy slurry and pig slurry and composted poultry manure. Kjeldahl N was measured by standard micro-Kjeldahl digestion. For total N measurements, NO3-N and NO2-N were recovered during Kjeldahl digestion by pretreatments with various oxidizing and reducing agents. Inorganic forms of N were measured by extraction with 2M KCl solution.Kjeldahl digestion alone allowed measurement only of organic N and NH4-N. Amongst various modifications to the Kjeldahl, pretreatment with either acidified (H2SO4) Zn-CrK(SO4)2 or acidified (H2SO4) reduced Fe achieved complete recovery of NO3-N. with KMnO4 followed by reduction to NH4-N with acidified (H2SO4) reduced Fe.More than 95% of the total N in fresh animal manure was present as organic N and NH4-N which were recovered by the standard Kjeldahl digestion. In the case of fresh manures there was no difference between the amount of total N measured by the Kjeldahl digestion and its modified methods. However composting of poultry manure or drying of poultry manure, pig slurry and dairy slurry resulted in an increase in NO3-N which was not recovered during Kjeldahl digestion alone. Under these conditions the total N could be measured by pretreating the samples with KMnO4 and reduced Fe prior to Kjeldahl digestion.Drying of animal manures caused a decrease in organic N and NH4-N, especially in poultry, pig and dairy manures. There was a slight increase in NO3-N; but most of the decrease in N content with drying was attributed to the volatilization loss of ammonia (NH3). Amongst various drying methods examined air drying caused maximum loss of N as NH3 whereas freeze drying caused minimum loss of N. This suggests that fresh animal manures can be freeze dried for analysis of N which causes minimum loss of N. 相似文献
