The microstructural aspects of the superplastic phenomenon are reviewed. The experimental results of a very large number of investigations are critically analysed in the context of: grain shape and size; grain growth; grain boundary sliding and migration, grain rotation and rearrangement; diffusion and dislocation activity. It is shown, that in spite of often conflicting evidence in the literature, a common pattern of microstructural behaviour emerges for all the materials and conditions investigated to date.  相似文献

OBJECTIVE: The development of carotid atherosclerosis after neck irradiation is well documented. There has been concern about the safety and durability of carotid artery repair through a radiated field. The objective of this report is to describe the immediate and long-term results of a series of cases collected in a 13-year interval. METHODS: From 1984 to 1997, 24 patients underwent 26 carotid artery operations. All the patients had undergone prior radiation therapy at a mean interval of 17 years, with an average radiation dose of 6300 rad. Severe scarring of the skin or radiation fibrosis were present in two thirds of the patients, with 4 patients having permanent tracheostomies. The indications for carotid surgery included cerebral or monocular transient ischemic attack (58%), asymptomatic high-grade stenosis (27%), prior stroke (12%), and tumor invasion of the carotid artery (4%). General anesthesia was used with selective shunting on the basis of carotid artery back pressure or electroencephalography monitoring. Patch angioplasty closure was used in 79% of the patients. The operations included standard carotid endarterectomy (n = 20), external carotid endarterectomy (n = 2), carotid patch angioplasty alone (n = 2), aortocarotid bypass grafting (n = 1), and carotid interposition grafting (n = 1). Four patients required skin grafting or myocutaneous flaps. RESULTS: No deaths or strokes occurred within 30 days of the operations. Six patients had transient cranial nerve palsy, and two had wound infections. The patients were followed from 1 to 156 months, with six patients being followed for longer than 18 months. No strokes were seen at late follow-up examination. Duplex scan examination documented one occlusion, in a patient with primary closure, and two restenoses, one of which necessitated reoperation. The remainder of the grafts were widely patent. CONCLUSIONS: Carotid surgery after neck irradiation is safe and durable. The long-term patency rates and the protection against subsequent neurologic events are similar to the results obtained in the absence of radiation therapy. Problems of wound healing were not found in this series.  相似文献

The objective of this study was to validate retrospective caregiver interviews for diagnosing major causes of severe neonatal illness and death. A convenience sample of 149 infants aged < 28 days with one or more suspected diagnoses of interest (low birthweight/severe malnutrition, preterm birth, birth asphyxia, birth trauma, neonatal tetanus, pneumonia, meningitis, septicaemia, diarrhoea, congenital malformation or injury) was taken from patients admitted to two hospitals in Dhaka, Bangladesh. Study paediatricians performed a standardised history and physical examination and ordered laboratory and radiographic tests according to study criteria. With a median interval of 64.5 days after death or hospital discharge, caregivers of 118 (79%) infants were interviewed about their child's illness. Using reference diagnoses based on predefined clinical and laboratory criteria, the sensitivity and specificity of particular combinations of signs (algorithms) reported by the caregivers were ascertained. Sufficient numbers of children with five reference standard diagnoses were studied to validate caregiver reports. Algorithms with sensitivity and specificity > 80% were identified for neonatal tetanus, low birthweight/severe malnutrition and preterm delivery. Algorithms with specificities > 80% for birth asphyxia and pneumonia had sensitivities < 70%, or alternatively had high sensitivity with lower specificity. In settings with limited access to medical care, retrospective caregiver interviews provide a valid means of diagnosing several of the most common causes of severe neonatal illness and death.  相似文献

BACKGROUND: Chronic myelogenous leukemia (CML) is an indolent but ultimately fatal disease. Because the natural history of CML varies and quality of life with CML may be excellent until shortly before death, deciding whether and when to pursue unrelated donor bone marrow transplantation is often difficult. OBJECTIVE: To compare early transplantation, delayed transplantation, and no transplantation for patients with chronic-phase CML on the basis of discounted, quality-adjusted life expectancy. DESIGN: A markov model comparing different strategies was constructed. This model considers patient age, quality of life, risk aversion, and the competing risks for CML progression and transplant toxicity. SETTING: Therapeutic decision at the time of diagnosis of CML. PATIENTS: The base case is a 35-year-old patient with intermediate-prognosis CML. Younger and older patients with better and worse prognoses are also evaluated. INTERVENTION: Early transplantation, delayed transplantation, and no transplantation. MEASUREMENTS: Quality-adjusted, discounted life expectancy. RESULTS: For patients with newly diagnosed CML, transplantation within the first year provides the greatest quality-adjusted expected survival, although this benefit decreases with increasing patient age. For a 35-year-old patient with intermediate-prognosis CML, transplantation within the first year results in 53 more discounted, quality-adjusted years of life expectancy than does no transplantation. This finding is robust even with varying baseline assumptions. CONCLUSIONS: These results support the use of early unrelated donor bone marrow transplantation for most patients with CML.  相似文献

The effect of grain size and composition on the fracture toughness of Ti–Al–Nb alloys in β solution-treated condition was investigated. The fracture toughness of the alloys was found to increase with an increase in grain size initially, reach a maximum and subsequently decrease with further increase in grain size. This trend was attributed primarily to the effect of grain size on the enhancement of fracture toughness due to stress-induced martensitic transformation (SIMT) at the crack tip, which in turn can be related to the effect of grain size on trigger stress for SIMT. Alloys containing higher Al and Nb showed a higher toughness for the same grain size, which was also explained in terms of effect of composition on the trigger stress.  相似文献

Euthanasia, informed consent, the conflict of interests of the individual versus those of society and racism were as prominent themes of Nazi medicine as they are today. Research in the field of TiO2-based photocatalysis has gained wide attention to address important energy and environmental problem. Lately, the use of conducting polymers as photosensitizers has proven to immensely enhance photodegradation by exhibiting excellent photocatalytic activity under both ultraviolet light and natural sunlight irradiation which is not possible using semiconductors alone. Considering the unique performance of conducting polymer-based nanocomposites in photocatalysis, the present review provides the recent advances in the development of ultraviolet and visible light-responsive conducting polymer-based TiO2 nanocomposites for their potential application in environmental remediation. This review ends with a summary focusing on the challenges and new dimensions in this still emerging area of research.  相似文献

The steady state solution of the double-ended queue involving taxis and customers at a taxi stand has been considered under the assumption that there is limited waiting space both for taxis and for customers. Taxis arrive according to mixed Erlangian input, the mean times of stay in the various phases which comprise the arrival channel not being the same. The arrivals of customers are assumed to be Poisson distributed. We obtain an expression for the probabilities that there aren units in the queue. The cases of exponential and 2-Erlang arrival distributions for taxis have been considered as particular cases. For the 2-Erlang arrival distribution for taxis, we have evaluated the probabilities that the waiting space for customers or taxis is full; their values are also shown by means of graphs. Grid applications with stringent security requirements introduce challenging concerns because the schedule devised by nonsecurity‐aware scheduling algorithms may suffer in scheduling security constraints tasks. To make security‐aware scheduling, estimation and quantification of security overhead is necessary. The proposed model quantifies security, in the form of security levels, on the basis of the negotiated cipher suite between task and the grid‐node and incorporates it into existing heuristics MinMin and MaxMin to make it security‐aware MinMin(SA) and MaxMin(SA). It also proposes SPMaxMin (Security Prioritized MinMin) and its comparison with three heuristics MinMin(SA), MaxMin(SA), and SPMinMin on heterogeneous grid/task environment. Extensive computer simulation results reveal that the performance of the various heuristics varies with the variation in computational and security heterogeneity. Its analysis over nine heterogeneous grid/task workload situations indicates that an algorithm that performs better for one workload degrades in another. It is conspicuous that for a particular workload one algorithm gives better makespan while another gives better response time. Finally, a security‐aware scheduling model is proposed, which adapts itself to the dynamic nature of the grid and picks the best suited algorithm among the four analyzed heuristics on the basis of job characteristics, grid characteristics, and desired performance metric.
