Transected median nerves in the forearm of two male patients, 12 and 21 years of age, were treated with a chamber technique leaving a 3 to 5 mm gap between the nerve ends. The nerve ends were enclosed in a silicone tube of such a dimension that would not cause compression of the nerve. Post-operative examination including sensory evaluation and assessment of muscle contraction force was carried out after 3 years. In both cases there was excellent motor recovery of the thenar muscles. Outgrowth of sensory fibres was remarkably fast, resulting ultimately in functional sensibility allowing almost normal hand function. 2PD was < or = 6 mm (12-year-old patient) and 8 to 10 mm (21-year-old patient) respectively. In one case the silicone tube was re-explored because of minor local discomfort 2 years after the repair. The former gap was bridged by a smooth continuous nerve-like structure of the same diameter as the adjacent nerve trunk and with no signs of neuroma formation or compression of the nerve.  相似文献

Effects of butantetracarboxylic acid (BTCA) modification, choice of matrix, and fiber volume fraction on hygroexpansion of wood fiber composites have been investigated. Untreated reference wood fibers and BTCA‐modified fibers were used as reinforcement in composites with matrices composed of polylactic acid (PLA), polypropylene (PP), or a mixture thereof. The crosslinking BTCA modification reduced the out‐of‐plane hygroexpansion of PLA and PLA/PP composites, under water‐immersed and humid conditions, whereas the swelling increased when PP was used as matrix material. This is explained by difficulties for the BTCA‐modified fibers to adhere to the PP matrix. Fiber volume fraction was the most important parameter as regards out‐of‐plane hygroexpansion, with a high‐fiber fraction leading to large hygroexpansion. Fiber‐matrix wettability during processing and consolidation also showed to have a large impact on the dimensional stability and moisture uptake. POLYM. The development of the uncrossed retinofugal pathways in normally pigmented and albino rats, aged from embryonic day (E) 14.5 to E18.5, was investigated. DiI was placed into one optic tract and the retinal origin of the uncrossed component, as well as its course in the optic stalk, was studied. The results show that, as in the mouse, the uncrossed retinal projection has two components. The first component is seen at E15.5 in normally pigmented animals. It develops exclusively in the central parts of the retina and is normal in albino littermates. The second component, which arises from the peripheral parts of the ventrotemporal retina, is seen two days later at E17.5 in all animals but is significantly smaller in albinos than in their pigmented littermates. Studies of axons in the optic stalk labelled retrogradely with DiI placed in the optic tract indicate that the uncrossed axons have no preference for any position in the stalk except when they approach the chiasm, where they tend to accumulate at the caudal region of the stalk. The uncrossed axons intermingle with the crossed axons along the entire length of the stalk. In albino embryos, no obvious difference in the prechiasmatic course of uncrossed axons was seen at any age examined. It is concluded that the albino mutation in rats affects the late ventrotemporal component of the uncrossed pathway selectively. It does not act on the early central component. Further, the intermingling of crossed and uncrossed axons in the stalk and the apparently unaffected prechiasmatic course of uncrossed axons in albinos indicate that the albino gene has its primary action in the retina.  相似文献

Two cases of rare metastases from malignant neoplasms to the posterior mandible in young adults are reported. Ep-CAM, an epithelium-specific cell-cell adhesion molecule (CAM) not structurally related to the major families of CAMs, contains a cytoplasmic domain of 26 amino acids. The chemical disruption of the actin microfilaments, but not of the microtubuli or intermediate filaments, affected the localization of Ep-CAM at the cell-cell boundaries, suggesting that the molecule interacts with the actin-based cytoskeleton. Mutated forms of Ep-CAM were generated with the cytoplasmic domain truncated at various lengths. All of the mutants were transported to the cell surface in the transfectants; however, the mutant lacking the complete cytoplasmic domain was not able to localize to the cell-cell boundaries, in contrast to mutants with partial deletions. Both the disruption of the actin microfilaments and a complete truncation of the cytoplasmic tail strongly affected the ability of Ep-CAM to mediate aggregation of L cells. The capability of direct aggregation was reduced for the partially truncated mutants but remained cytochalasin D sensitive. The tail truncation did not affect the ability of the transfectants to adhere to solid-phase-adsorbed Ep-CAM, suggesting that the ability to form stable adhesions and not the ligand specificity of the molecule was affected by the truncation. The formation of intercellular adhesions mediated by Ep-CAM induced a redistribution to the cell-cell boundaries of alpha-actinin, but not of vinculin, talin, filamin, spectrin, or catenins. Coprecipitation demonstrated direct association of Ep-CAM with alpha-actinin. Binding of alpha-actinin to purified mutated and wild-type Ep-CAMs and to peptides representing different domains of the cytoplasmic tail of Ep-CAM demonstrates two binding sites for alpha-actinin at positions 289 to 296 and 304 to 314 of the amino acid sequence. The results demonstrate that the cytoplasmic domain of Ep-CAM regulates the adhesion function of the molecule through interaction with the actin cytoskeleton via alpha-actinin.  相似文献

Second trimester maternal serum screening for fetal Down's syndrome (DS), using the AFP (alpha-fetoprotein), hCG (human chorionic gonadotrophin) and uE3 (unconjugated oestriol) triple test, is as well documented procedure associated with a DS-detection rate of about 70 per cent, for an amniocentesis rate of about 7 per cent. The triple test is relatively little used in the Nordic countries, though its wider use would result in more efficient diagnosis of DS and various fetal malformtions. The maternal age indication currently used leaves gravidae under 35 years of age without prenatal diagnostics, although it is in just this age group that the majority (70 per cent) of cases of fetal DS occur. In Denmark, where 12 per cent of gravidae undergo invasive diagnostic procedures, the proportion of induced abortions due to the procedures is far too high, in relation to the DS detection rates obtained. Maternal serum screening yields a much better ratio between the risk of abortion after amniocentesis and the likelihood of DS detection than does maternal age alone. Maternal serum screening at 7-14 weeks of gestation, using pregnancy-associated plasma protein A and free hCG beta subunit concentrations, will become available within the next few years, thus reducing the incidence of some of the psychological and technical problems associated with second trimester screening, especially that of third trimester abortion. Irrespective of whether it is performed in the first or the second trimester, maternal serum screening will be the cornerstone of prenatal DS diagnosis in the future.  相似文献
