The epidemiological indices of multiple myeloma and concomitant nephropathies

The authors have conducted an epidemiological study for multiple myeloma morbidity and mortality among 80,000 patients of the Medical Center examined in 1988-1995. Myeloma was diagnosed in 65 cases. Standard incidence rate was 4.09, 3.18 and 3.5 per 100,000 for myeloma in males, females and both sexes, respectively. Standard mortality was 2.76 and 1.73 per 100,000 for males and females, respectively. The above rates do not differ much from those of other countries. In terminal myeloma renal insufficiency occurred in 39 of 41 cases. In 8 cases chronic pyelonephritis was an independent course of renal failure thus indicating the importance of its persistent treatment.     

Inability to work after surgical removal of mandibular third molars

This series presents six cases of a rare variant of dermatofibroma, characterized by marked clear cell change. All lesions occurred on the lower extremities of middle-aged adults (four women, two men), mostly with the clinical diagnosis of fibrohistiocytic lesion. Histological examination revealed well circumscribed, faintly stained dermal to subcutaneous lesions which were due to the overwhelming presence of clear cells (> 90%), some with prominent PAS-positive cytoplasmic granulation. Overlying epidermal hyperplasia as well as storiform arrangement of spindle cells, sclerotic collagen and some interspersed lympho-histiocytic infiltrate at the periphery of the lesion indicated the fibrohistiocytic origin. Individual histopathological peculiarities included: bizarre giant cells in two cases, perifollicular arrangement and haemangiopericytoma-like features with iron deposition in one case each. Immunohistochemically three of four lesions showed moderate reactivity for factor XIIIa and two of four with an anti-metallothionen marker E9, but were otherwise negative with a broad panel of markers. Electronmicroscopy in two cases revealed large pools of glycogen beside focal, prominent endoplasmic reticulum and lysosomes in some granular cells, but only optically translucent cells in cases of clear cells. Recognition of clear cell dermatofibroma is important as the differential diagnosis includes some entities with more serious outcome/considerations such as metastases of renal cell carcinoma, xanthogranulomatous reactions, balloon cell naevus/melanoma and clear cell sarcoma.     

Prevention and treatment of suppuration caused by neck trauma

93 cases of cervical injury are described. 39 patients developed pyonecrotic complications in the course of treatment. These were caused by both increased virulence of the microorganisms and low immunoresistance of the patients. In abscesses and pus absence the inflammatory infiltrate was opened and drained. Powder antiseptic iodopor, antibiotic epocillin, tampon ambikor, etc. were used postoperatively. Bacteriologically, the response was substantial.     

Prevalence of carbohydrate metabolism disorders in free-living females of capable age in the city of Baku

AIM: The study of prevalence of carbohydrate metabolism disorders (CHMD) in females. MATERIALS AND METHODS: A representative sample of 20-59-year-old female population of the city of Baku was examined using age-specific standardized index, WHO tests and criteria. RESULTS: CHMD occurred rather frequently. Tolerance to glucose was abnormal in 50% of the examinees. Diabetes mellitus affected 20-29-year-old women 4 times more frequently than poor glucose tolerance. An opposite trend was observed for 40-49-year-old women. CONCLUSION: Relevant prophylactic measures are required for female Baku population.     

The sequelae of the intraspecific isolation of adult rats (right-handed, left-handed and ambidextrous animals)

We studied the influence of social isolation at the age of 2 months on zoosocial behaviour of mixed-bred male rats with different paw preference. Paw preference was determined in the test of reaching for food in the horizontal tube. The level of aggression but not sociability was found to increase significantly in dextral and ambidextrous late isolants. In sinistrals the increase of sociability but not aggression was observed. Analysis of probabilistic ethological structure of aggressive behaviour showed its validity and revealed the appearance of pathological aggression as a result of isolation in all groups of animals. Being connected with a disfunction of the regulating role of the right brain hemisphere this feature of aggressive behaviour is most pronounced in dextrals and ambidextrals.
