Although Fetal Alcohol Syndrome (FAS) rates have been reported to be higher in American Indian populations, no screening tool has been validated for alcohol use in American Indian women. The objectives of this study were to compare the detection of prenatal alcohol use by a self-administered questionnaire to detection by clinical interview; and to ascertain whether the screening tool would increase detection of pregnant women who are abusing alcohol. The hospital records of the women were reviewed for any history of alcohol-related illnesses or injuries to compare with results obtained from the questionnaire. Seventy women attending their first prenatal clinic visit on a reservation were screened for alcohol use. There was a wide range in detection of prenatal alcohol use (20%-71% of the sample detected) depending on the method used. There was a large variation in sensitivities (7%-93%) of the individual questions in identifying patients detected as "high risk" by the clinicians. The T-ACE screening questions significantly increased detection of alcohol use compared to detection by the clinicians (p = 0.04 Fisher's exact test). Due to the large variation between different methods of detection, it is recommended that screening tools that increase detection of alcohol use should be combined with methods of higher specificity such as using questions about quantity and frequency of alcohol intake, medical chart review and clinical interview. We also found that various interpretations of the screening questions by the patients highlighted the need to tailor the wording of individual questions to the particular patient population.  相似文献

Large signal-to-background (S/B) ratios for the Fe(CN)(6)(3)(-)(/4)(-) and IrCl(6)(2)(-)(/3)(-) redox couples in KCl have been observed in cyclic voltammetric measurements made at a conductive diamond thin-film electrode without any conventional surface pretreatment. The S/B ratios were a factor of ~16 and 8 larger at diamond than at freshly polished glassy carbon (GC) for Fe(CN)(6)(3)(-)(/4)(-) and IrCl(6)(2)(-)(/3)(-), respectively. The polycrystalline diamond film, grown on a p-Si(100) substrate, possessed significant cubic {100} faceting, as evidenced by AFM images, and was of high quality, as indicated by Raman spectroscopy. The high degree of electrochemical activity without surface pretreatment, the enhanced S/B ratios, and the excellent response stability demonstrate that diamond might be an attractive new electrode material for electroanalysis.  相似文献

Synchrotron based energy dispersive X-ray diffraction has been used to profile the strains around fatigue cracks in 4140 steel test specimens. In particular strain field comparisons were made on specimens prepared: with initial constant stress intensity fatigue; with this initial fatigue followed by a single overload cycle; and with this fatigue-overload sequence followed by an additional constant stress intensity fatigue. The strain profiles behind, at and in-front-of the crack tip are discussed in detail. Selected strain profiles measurements under in situ applied tensile stress are also presented. The technique of optical surface height profiling reveals surface depression effects which can be correlated with the interior strain profiles.  相似文献

The fatigue crack growth behavior of aluminum alloy 5083-H131 has been systematically studied as a function of degree of sensitization for aging at 448?K (175?°C). Fatigue crack growth rates were measured at load ratios of 0.1 and 0.85, in vacuum, air, and a corrosive aqueous environment containing 1?pct NaCl with dilute inhibitor. Sensitization does not affect the fatigue crack growth behavior of Al 5083-H131 significantly in vacuum or air, at low- or high-load ratio. For high-load ratio, in the 1?pct NaCl+inhibitor solution, the threshold drops by nearly 50?pct during the first 200?hours of aging, then it degrades more slowly for longer aging times up to 2000?hours. The change in aging behavior at approximately 200?hours seems to be correlated with the transition from partial coverage of the grain boundaries by ?? Al3Mg2 phase, to continuous full ?? coverage. ASTM G-67 mass loss levels below approximately 30?mg/cm2 do not exhibit degraded corrosion-fatigue properties for R?=?0.85, but degradation of the threshold is rapid for higher mass loss levels.  相似文献

PWM controlled line-side power converters of modern AC traction locomotives inject harmonic currents into the feeding overhead line. This causes problems of electromagnetic interference. Passive or active filters are usually provided for a partial reduction of the line harmonics. A novel and superior approach employs a switched electronic compensator. It generates an exact replica of the harmonic current, feeding it to the high-voltage transformer to produce a harmonic counter MMF. The transformer main flux is then forced to be sinusoidal, and so is the induced voltage in the primary. The line current, being low-pass filtered by the transformer leakage inductance, assumes a pure sinusoidal waveform. The compensator operates in the low switching frequency range of the main power converters. Its installed power is only 1% of the traction power. Operation, control, and design considerations are described. Results from laboratory tests at full power level are presented  相似文献

In megawatt-rated inverter-fed induction motor drive systems, the gate-turn-off switching losses account for a considerable amount of the total losses, hence, the switching frequency in such systems must be kept at a low value of only a few hundred hertz. To avoid undesired torque harmonics under such operating conditions, the pulse control of the inverter is made dependent on the orientation of the rotor flux of the drive machine. This method transfers a major portion of the unavoidable current distortions into the field axis where they have no influence on the machine torque. An optimal trajectory-oriented control is presented that achieves low switching frequency at low torque and current harmonics and exhibits a very fast dynamic response. Exposure of mammalian cells to ultraviolet (UV) light elicits a cellular response and can also lead to apoptotic cell death. In this report, we show that a 36-kDa myelin basic protein (MBP) kinase detected by an in-gel kinase assay can be dramatically activated during the early stages of UV irradiation-triggered apoptosis of A431 cells. Immunoblot analysis revealed that this 36-kDa MBP kinase could be recognized by an antibody against the C-terminal regions of a family of p21Cdc42/Rac-activated kinases (PAKs). By using this antibody and a PAK2-specific antibody against the N-terminal region of PAK2 as studying tools, we further demonstrated that UV irradiation caused cleavage of PAK2 to generate a 36-kDa C-terminal catalytic fragment and a 30-kDa N-terminal fragment in A431 cells. The appearance of the 36-kDa C-terminal catalytic fragment of PAK2 matched exactly with the activation of the 36-kDa MBP kinase in A431 cells upon UV irradiation. In addition, UV irradiation also led to activation of CPP32/caspase-3, but not ICH-1L/caspase-2 and ICE/caspase-1, in A431 cells and the kinetics of activation of CPP32/caspase-3 appeared to correlate well with that of DNA fragmentation and of cleavage/activation of PAK2, respectively. Moreover, blockage of activation of CPP32/caspase-3 by pretreating the cells with two specific tetrapeptidic inhibitors for caspases (Ac-DEVD-cho and Ac-YVAD-cmk) could significantly attenuate the extent of cleavage/activation of PAK2 induced by UV irradiation. Collectively, the results demonstrate that cleavage and activation of PAK2 can be induced during the early stages of UV irradiation-triggered apoptosis and indicate the involvement of CPP32/caspase-3 in this process.  相似文献

Fifty consecutive patients with surgical obstructive jaundice were evaluated prospectively with ultrasonography (US), computed tomographic scans (CT scan) and cholangiography-percutaneous transhepatic cholangiography (PTC) or endoscopic retrograde cholangio-pancreaticography (ERCP). The diagnostic accuracy of ultrasound in defining the level of obstruction was 86% as compared to 86% and 94.8% for CT scan and cholangiography, respectively. To measure the etiology of the obstruction, the accuracy of ultrasound, CT scan and cholangiography were 84%, 86% and 75%, respectively. The sensitivity of CT scans and cholangiography in the diagnosis of choledocholithiasis was 100%, 81.8% and 90%, respectively, whereas specificity was 97%, 100% and 100%, respectively. Sensitivity for a diagnosis of malignant disease was 100% for both US and CT scans whereas specificity was 90% and 81%, respectively. Ultrasonography as a single radiological investigation is sufficient in the evaluation of the majority of patients with surgical obstructive jaundice. CT scan and cholangiography should be done only when US gives equivocal findings or if concomitant therapeutic procedures like basketing and stenting are also planned.  相似文献

Detergent-based vaginal microbicides, in addition to their high contraceptive failure rates, cause mucosal erosion and local inflammation that might increase the risk of heterosexual human immunodeficiency virus (HIV) transmission. In a systematic effort to identify a microbicide contraceptive potentially capable of preventing the sexual transmission of HIV as well as providing fertility control, a series of novel aryl phosphate derivatives of 5-bromo-6-methoxy-3'-azido-3'-deoxythymidine (AZT; zidovudine) were synthesized and examined for dual anti-HIV and sperm-immobilizing activity (SIA). Whereas AZT displayed potent anti-HIV activity (IC50 = 0.006 microM) but lacked SIA (EC50 > 300 microM), two 5-bromo-6-methoxy-aryl phosphate derivatives of AZT, compounds WHI-05 and WHI-07, exhibited potent anti-HIV activity as well as SIA. The IC50 (HIV) and EC50 (SIA) values for WHI-07 were 439-fold and 13.5-fold lower, respectively, than those for the detergent-based virucidal spermicide, nonoxynol-9 (N-9). Sperm motion kinematics using computer-assisted sperm motion analysis combined with confocal laser scanning microscopy, high-resolution low-voltage scanning, and transmission electron microscopy demonstrated that both WHI-05 and WHI-07 cause a complete and irreversible loss of sperm motility in a concentration- and time-dependent fashion without concomitantly affecting the sperm acrosomal membrane integrity. In experiments designed to assess the fertilizing capacity of treated sperm, preincubation of sperm with either compound resulted in a concentration-dependent loss of the ability to adhere to and penetrate zona-free hamster eggs as well as inhibition of binding to human zona. WHI-07 applied intravaginally prior to artificial insemination of epididymal sperm drastically reduced fertility in hormonally primed CD-1 mice. Unlike the intravaginal application of N-9, repetitive intravaginal application of WHI-07 did not damage the vaginal epithelium or cause local inflammation. Structure-function relationship analyses showed that the addition of bromo-methoxy functional groups to AZT was essential for, and the aryl phosphate derivatization contributory to, the SIA of both compounds. Compounds WHI-05 and WHI-07 may be useful as dual-function vaginal contraceptives for women who are at high risk for acquiring HIV/acquired immunodeficiency virus syndrome by heterosexual vaginal transmission.  相似文献
