Surveillance after colorectal cancer surgery

Early diagnosis of local and distant recurrences of colorectal cancer remains difficult and there is no agreement on the effectiveness of follow-up in these patients. The aim of this study is to assess the value of our method of follow-up. We consider 239 patients with colorectal cancer and at least 2 years follow-up following radical resection. A local recurrence appeared in 26 patients (10.9%), a distant metastasis in 41 (17.1%), while in seven (2.9%) local and distant recurrences appeared simultaneously. Local recurrence was detected because of an increase in carcinoembryonic antigen (CEA) level in 15 patients (57.7%), during a scheduled endoscopy in four (15.4%) and because of symptoms in seven (26.9%). In seven patients (26.9%) a radical resection was possible. Distant metastases were detected by CEA levels in 20 patients (48.8%), by ultrasonography (U.S.) in 12 (29.3%) and by chest X-ray in five (12.2%). In 13 of 26 patients with liver metastases a resection was performed. This study shows that few patients benefit from follow-up and only CEA levels and liver U.S. performed intensively between 15 and 36 months after surgery are useful in early detection of recurrences. A modification of the follow-up to the single patient, according to the stage, location and grading of cancer, could improve the results, so lowering the costs of this expensive practice.     

Beugung am Kegel elliptischen Querschnitts und die Sommerfeldsche Lösung des Halbebenenproblems

Übersicht In dieser Arbeit wird die Beugung am Kegel elliptischen Querschnitts behandelt. Wir leiten zwei Greensche Funktionen ab, die der Dirichlet- bzw. der Neumann-Bedingung auf dem Kegel genügen und die Ausstrahlungsbedingung erfüllen. Das Randwertproblem wird zurückgeführt auf die Lösung eines zweiparametrigen Eigenwertproblems mit zwei gekoppelten Laméschen Differentialgleichungen. Wir analysieren insbesondere die Beugung einer ebenen Welle an der sektorförmigen Fläche. Im Grenzfall eines Sektoröffnungswinkels von 180° gehen die Reihenentwicklungen unter Zuhilfenahme eines speziellen Gegenbauerschen Additionstheorems und einer erzeugenden Funktion für Besselfunktionen mit ganzzahligem und halbzahligem Index über in die Sommerfeldsche Lösung des Halbebenenproblems.

Diffraction by an elliptic cone and Sommerfeld's half-plane solution

Contents In this article we examine the diffraction by an elliptic cone. We derive two Green's functions satisfying the Dirichlet-resp. Neumann-condition on the boundary of the cone. The boundary value problem is reduced to a two-parametric eigenvalue problem with two coupled Lamé equations. We analyse the diffraction of a plane wave by a plane angular sector. In the limiting case of a sector angle of 180° the series expansions pass — using a special form of Gegenbauer's addition theorem and the generating function for Bessel functions with integral and half-integral indices — into Sommerfeld's half-plane solution.

     

The effect of core modelling on the self-force and self-energy of a dislocation

A verification example is solved using a core model recently introduced in a FEM approach to dislocation dynamics. The core model is compared to the more accepted cut-off procedures. It is demonstrated that these procedures give the correct self-force if the cut-off distance is selected properly. The cut-off procedures do however have some grave weaknesses. These limit their usefulness to a restricted number of problems.     

Pediatric craniocerebral trauma. Special characteristics, therapy and prognosis

The mortality rate after traumatic brain injury in children ranges between 2.5% and 21%. Standardized diagnostic procedures and therapeutic strategies for the management of traumatic brain damage are presented in this article. Children with traumatic cerebral lesions have a better clinical outcome than head-injured adults. Optimized medical management and intensive rehabilitation may help to reduce the frequency of mental retardation and physical disability following such injuries in children.     

Inhibiting effects of serotonin and serotonin antagonists on the migration of mononuclear leucocytes

The effect of serotonin and the serotonin antagonists ketanserin, methiotepine and ICS-205-930 on the migration of leucocytes was studied by using the sealed capillary migration technique. The migration of mononuclear leucocytes was inhibited by serotonin at 10(-4) and 10(-6)-10(-10)mol/l. An inhibition of the mononuclear leucocyte migration was also caused by ICS-205-930 at 10(-4)mol/l, ketanserin at 10(-4) and 10(-8)-10(-10)mol/l and methiotepine at 10(-4) and 10(-6)-10(-8)mol/l. No inhibiting effects of serotonin or the serotonin antagonists were found on the migration of polymorphonuclear leucocytes. Thus, both serotonin and serotonin antagonists may inhibit mononuclear leucocyte migration.     

Restenosis and arterial remodelling after coronary angioplasty

Restenosis after coronary angioplasty (PTCA) is a complex process and is still the major problem, despite improvements in equipment and technique. Thrombus formation and intimal hyperplasia have been considered to be the main causes of the development of restenosis after primary successful angioplasty. As yet, pharmacological trials to prevent restenosis have failed to prevent it, despite the fact that the therapy has been aimed at reducing thrombus formation and intimal hyperplasia. Several new angioplasty devices have been developed. Series of observations and a few controlled trials have demonstrated restenosis rates similar to those obtained with conventional balloon angioplasty, except in the case of stent implantation, which appears to be promising. Intravascular ultrasound studies have provided new insight and a more complete understanding of the process leading to restenosis. Vascular remodeling is now considered as an important pathogenetic factor. It consists of a change in the cross-sectional vessel area and may involve an actual constriction of the artery. This may lead to lumen-narrowing and finally restenosis with minimal neointimal formation. In this review we summarise the literature on the restenosis process and the current status of the clinical trials aimed at preventing restenosis.     

Intrauterine transfusions influence fetal leukocyte counts and subsets

Histamine is an important mediator in allergic reactions, gastric acid secretions, and neurotransmission in the central nervous system. Basophils and mast cells are the main sources of histamine, which is formed from L-histidine by histidine decarboxylase (HDC). However, the regulatory mechanism of HDC in these cells remains unclear. We examined the regulation of HDC activity and gene expression using a unique human mast cell line, HMC-1, after stimulation with phorbol 12-myristate 13-acetate (PMA) or ionomycin. HDC activity was increased from 52.1+/-0.4 (mean+/-standard deviation) to 154+/-6.9, or 105.6+/-6.2 pmol/min/mg protein (n = 3), 4 hours after stimulation with PMA (10 ng/mL) or ionomycin (10[-6] M). Although actinomycin D had no effect on this increase, cycloheximide completely inhibited the increase caused by these stimuli. The population of HMC-1 cells containing HDC protein was increased after stimulation with either PMA or ionomycin as evaluated by immunocytochemical analysis with anti-HDC antibody as a marker. HMC-1 constitutively expressed HDC mRNA, and its level was not increased with these stimuli. These results suggest that the increase of HDC activity in HMC-1 induced by PMA or ionomycin is regulated at the translational level.
