OBJECTIVES: An understanding of the validity and usefulness of self-reported measures (as distinct from clinically determined measures) of oral health is emerging. These self-reported measures include self-rated oral health (SROH). Three objectives were to: (1) describe self-rated oral health in dentate adults, (2) quantify associations between self-rated oral health and other measures of oral health (oral disease and tissue damage, pain and discomfort, functional limitation, and disadvantage), and (3) assess the construct validity of a model of oral health proposed herein. METHODS: The Florida Dental Care Study is a longitudinal study of oral health, which included at baseline 873 subjects who had at least one tooth, were 45 years or older, and who participated for an interview and clinical examination. RESULTS: The prevalence of self-rated oral health decrements was substantial; approximately one fourth of subjects reported their oral health as only fair or poor. Bivariate and multivariate results provided consistent evidence of the construct validity of the proposed model of oral health. Additionally, the salience of one measure of dental appearance suggests that persons may use esthetic cues when rating their oral health. CONCLUSIONS: The proposed multidimensional model of oral health has construct validity. Self-rated oral health is affected by oral disease and tissue damage, oral pain and discomfort, oral functional limitation, and oral disadvantage. These self-reported measures and the proposed model should provide useful information for dental care effectiveness research. General health status has been disaggregated into the "physical" and the "mental;" an additional separation into the "oral" aspects of health seems warranted.  相似文献

BACKGROUND: Do-not-resuscitate (DNR) orders for critically ill patients are frequently miscommunicated between attending physicians, house staff, and nurses. A computer-based system was developed to improve the communication of a procedure-specific DNR order form. METHODS: Concordance of understanding of patients' DNR status was measured with the use of unstructured DNR orders (period 1), procedure-specific DNR order forms (period 2), and procedure-specific DNR order forms administered with a computer-based communication system (period 3). The 3 components of the DNR order assessed were (1) the clinical events to which the DNR order applied, (2) whether the DNR order withheld all elements of cardiopulmonary resuscitation, and (3) whether other treatments were to be withheld. RESULTS: For the 147 patients, the computer-based system in period 3 (n = 71) improved concordance for attending physicians and nurses or residents for all 3 of the DNR components compared with period 1 (n = 40) and some of the DNR components compared with period 2 (n = 36). Concordance was "substantial" or "almost perfect" as measured by the K statistic during period 3. The proportion of agreement for the composite of all 3 components of the DNR order increased during each period (P<.001, period 3 vs period 1). Overall agreement between all caregivers for the composite DNR order also improved from period 1 (22.2%) to period 2 (47.8%) and period 3 (61.9%; P<.001 vs period 1). Errors in order entry were detected by physicians because of the computer system and corrected in 9.9% of DNR orders in period 3. Progress note documentation of DNR status did not improve during period 3. The procedures of period 3 were considered acceptable by the physician and nursing staff. CONCLUSION: A computer-based system combined with a procedure-specific DNR order form improves communication of patients' DNR status in a critical care setting.  相似文献

Natural language data from 38 47-month-olds recorded at home in unstructured observations were analyzed and comparisons made of characteristics of mental state term use in child-friend, child-sibling, and child-mother dyads. Significantly more references to mental states were made by the children in conversations with siblings and friends than with mothers. Frequent use of mental state terms by both partners was related to cooperative interaction in both child-friend and child-sibling dyads and several associations were found with measures of language fluency, gender, and maternal education, although these varied across the 2 dyads. Children's use of mental state terms in conversations with siblings and friends was correlated with their performance on two false belief measures. Results highlight the importance of extending investigations into the social implications of the development of children's "theories of mind."  相似文献

This paper examines popular concepts and "types" of organizational commitment in light of the definition of commitment and common factors that pertain to all commitments. It argues that a commitment is best conceptualized as a single, fundamental construct that may vary according to differences in focus, terms, and time-specific evaluation. Analysis of affective commitment measures indicates that certain measures contain more than one set of terms and make assumptions about evaluation that add constraints to the basic meaning of commitment. Recommendations include the argument that commitment measures address one set of terms and avoid evaluative phraseology.  相似文献

This paper presents an interactive visualization tool to study and analyze hyperspectral images (HSI) of historical documents. This work is part of a collaborative effort with the Nationaal Archief of the Netherlands (NAN) and Art Innovation, a manufacturer of hyperspectral imaging hardware designed for old and fragile documents. The NAN is actively capturing HSI of historical documents for use in a variety of tasks related to the analysis and management of archival collections, from ink and paper analysis to monitoring the effects of environmental aging. To assist their work, we have developed a comprehensive visualization tool that offers an assortment of visualization and analysis methods, including interactive spectral selection, spectral similarity analysis, time-varying data analysis and visualization, and selective spectral band fusion. This paper describes our visualization software and how it is used to facilitate the tasks needed by our collaborators. Evaluation feedback from our collaborators on how this tool benefits their work is included.  相似文献

We present a novel approach for computing and solving the Poisson equation over the surface of a mesh. As in previous approaches, we define the Laplace-Beltrami operator by considering the derivatives of functions defined on the mesh. However, in this work, we explore a choice of functions that is decoupled from the tessellation. Specifically, we use basis functions (second-order tensor-product B-splines) defined over 3D space, and then restrict them to the surface. We show that in addition to being invariant to mesh topology, this definition of the Laplace-Beltrami operator allows a natural multiresolution structure on the function space that is independent of the mesh structure, enabling the use of a simple multigrid implementation for solving the Poisson equation.  相似文献

Motorcycle protective clothing can be uncomfortably hot during summer, and this experiment was designed to evaluate the physiological significance of that burden. Twelve males participated in four, 90-min trials (cycling 30 W) across three environments (25, 30, 35 °C [all 40% relative humidity]). Clothing was modified between full and minimal injury protection. Both ensembles were tested at 25 °C, with only the more protective ensemble investigated at 30 and 35 °C. At 35 °C, auditory canal temperature rose at 0.02 °C min?1 (SD 0.005), deviating from all other trials (p < 0.05). The thresholds for moderate (>38.5 °C) and profound hyperthermia (>40.0 °C) were predicted to occur within 105 min (SD 20.6) and 180 min (SD 33.0), respectively. Profound hyperthermia might eventuate in ~10 h at 30 °C, but should not occur at 25 °C. These outcomes demonstrate a need to enhance the heat dissipation capabilities of motorcycle clothing designed for summer use in hot climates, but without compromising impact protection.

This paper addresses the problem of matting motion blurred objects from a single image. Existing single image matting methods are designed to extract static objects that have fractional pixel occupancy. This arises because the physical scene object has a finer resolution than the discrete image pixel and therefore only occupies a fraction of the pixel. For a motion blurred object, however, fractional pixel occupancy is attributed to the object’s motion over the exposure period. While conventional matting techniques can be used to matte motion blurred objects, they are not formulated in a manner that considers the object’s motion and tend to work only when the object is on a homogeneous background. We show how to obtain better alpha mattes by introducing a regularization term in the matting formulation to account for the object’s motion. In addition, we outline a method for estimating local object motion based on local gradient statistics from the original image. For the sake of completeness, we also discuss how user markup can be used to denote the local direction in lieu of motion estimation. Improvements to alpha mattes computed with our regularization are demonstrated on a variety of examples.  相似文献

The opportunities for context-aware computing are fast expanding. Computing systems can be made aware of their environment by monitoring attributes such as their current location, the current time, the weather, or nearby equipment and users. Context-aware computing often involves retrieval of information: it introduces a new aspect to technologies for information delivery; currently these technologies are based mainly on contemporary approaches to information retrieval and information filtering. In this paper, we consider how the closely related, but distinct, topics of information retrieval and information filtering relate to context-aware retrieval. Our thesis is that context-aware retrieval is as yet a sparsely researched and sparsely understood area, and we aim in this paper to make a start towards remedying this.  相似文献
