In this report, we conducted a secondary analysis of the Treatment of SSRI-Resistant Depression in Adolescents (TORDIA) study to explore the impact of specific cognitive–behavioral therapy (CBT) treatment components on outcome. In TORDIA, 334 youths (ages 12 to 18 years) with major depressive disorder who had failed to respond to an adequate course of selective serotonin reuptake inhibitor (SSRI) medication were randomized to a medication switch (either to an alternative SSRI or venlafaxine) with or without 12 weeks of adjunctive CBT. Participants who had more than 9 CBT sessions were 2.5 times more likely to have adequate treatment response than those who had 9 or fewer sessions. CBT participants who received problem-solving and social skills treatment components, controlling for number of sessions and other confounding variables, were 2.3 and 2.6 times, respectively, more likely to have a positive response. These preliminary findings underscore the importance of receiving an adequate number of sessions to attain an adequate clinical response. Finally, social skills and problem solving may be active elements in CBT for adolescent depression and should be considered in treatment by those working with seriously depressed youths. Within 12 months of the discovery of X rays in 1895, papers appeared in the literature reporting adverse effects from high exposure. In 1925, the first International Congress of Radiology, held in London, considered the need for a protection committee, which it established at its second congress in Stockholm in 1928. This paper celebrates the 80th anniversary of ICRP by tracing the history of the development of its policies, and identifying a few of the personalities involved from its inception up to the modern era. The paper follows the progress from the early controls on worker doses to avoid deterministic effects, through the identification of stochastic effects, to the concerns about public exposure and increasing stochastic risk estimates. The key features of the recommendations made by ICRP from 1928 up to the most recent in 2007 are identified.  相似文献

We present a framework for model checking concurrent software systems which incorporates both states and events. Contrary to other state/event approaches, our work also integrates two powerful verification techniques, counterexample-guided abstraction refinement and compositional reasoning. Our specification language is a state/event extension of linear temporal logic, and allows us to express many properties of software in a concise and intuitive manner. We show how standard automata-theoretic LTL model checking algorithms can be ported to our framework at no extra cost, enabling us to directly benefit from the large body of research on efficient LTL verification. We also present an algorithm to detect deadlocks in concurrent message-passing programs. Deadlock- freedom is not only an important and desirable property in its own right, but is also a prerequisite for the soundness of our model checking algorithm. Even though deadlock is inherently non-compositional and is not preserved by classical abstractions, our iterative algorithm employs both (non-standard) abstractions and compositional reasoning to alleviate the state-space explosion problem. The resulting framework differs in key respects from other instances of the counterexample-guided abstraction refinement paradigm found in the literature. We have implemented this work in the magic verification tool for concurrent C programs and performed tests on a broad set of benchmarks. Our experiments show that this new approach not only eases the writing of specifications, but also yields important gains both in space and in time during verification. In certain cases, we even encountered specifications that could not be verified using traditional pure event-based or state-based approaches, but became tractable within our state/event framework. We also recorded substantial reductions in time and memory consumption when performing deadlock-freedom checks with our new abstractions. Finally, we report two bugs (including a deadlock) in the source code of Micro-C/OS versions 2.0 and 2.7, which we discovered during our experiments. The widespread use of personal digital assistants and smartphones gives securing these devices a high priority. Yet little attention has been placed on protecting handheld devices against viruses. Currently available antivirus software for handhelds is few in number. At this stage, the opportunity exists for the evaluation and improvement of current solutions. By pinpointing weaknesses in the current antivirus software, improvements can be made to properly protect these devices from a future tidal wave of viruses. This research evaluates four currently available antivirus solutions for handheld devices. A formal model of virus transformation that provides transformation traceability is presented. Two sets of ten tests each were administered; nine tests from each set involved the modification of source code of two known viruses for handheld devices. The testing techniques used are well established in PC testing; thus the focus of this research is solely on handheld devices. Statistical analysis of the test results show high false negative production rates for the antivirus software and an overall false negative production rate of 47.5% with a 95% confidence interval between 36.6% and 58.4%. This high rate shows that current solutions poorly identify modified versions of a virus. The virus is left undetected and capable of spreading, infecting and causing damage.  相似文献

OBJECTIVE: To investigate the roles of brain angiotensin II and C-type natriuretic peptide (CNP) in the hypertensive mechanism of deoxycorticosterone acetate (DOCA)-salt hypertension. METHODS: We injected 50 microg/kg CV-11 974, an angiotensin II type-1 receptors antagonist, 30 nmol/kg CNP-22, or the vehicle (artificial cerebrospinal fluid) into the cerebral ventricle or intravenously 5 min before the intracerebroventricular infusion of 1.5 mol/I NaCl solution for 30 min into either male normotensive Wistar rats or DOCA-salt hypertensive rats anesthetized with urethane, and their arterial pressures and heart rates were continuously recorded. Blood (2 ml) was collected at the end of the infusion for the measurement of plasma concentration of arginine vasopressin. We infused 10 or 50 microg/kg per day CV-11 974, 10 or 50 nmol/kg per day CNP-22, or the vehicle (1 microl/h) into the cerebral ventricles of DOCA-salt hypertensive rats for 7 days by using osmotic minipumps, and measured their systolic arterial pressures, pulse rates, and urinary excretions of vasopressin. RESULTS: Intracerebroventricular pre-administrations of CV-11 974 and of CNP-22 inhibited increases in mean arterial pressure, heart rate, and plasma vasopressin concentration induced by intracerebroventricular infusion of 1.5 mol/l NaCl into normotensive rats; increases in hemodynamics and plasma level of vasopressin induced by intracerebroventricular infusion of 1.5 mol/l NaCl were suppressed by intracerebroventricular pre-injections of CV-11 974, but not of CNP-22, into DOCA-salt hypertensive rats. Continuous intracerebroventricular infusions of 50 microg/kg per day CV-11 974 attenuated hypertension in DOCA-salt treated rats, accompanied by a reduction in urinary excretion of vasopressin. Continuous intracerebroventricular infusions of 50 nmol/kg per day CNP-22, however, affected neither hypertension nor urinary excretion of vasopressin in DOCA-salt hypertensive rats. CONCLUSION: Brain angiotensin II could play a role in the pressor mechanism of DOCA-salt hypertension by increasing release of vasopressin via type 1 receptors. That brain CNP has an inhibitory effect on release of vasopressin in acute experiments indicates that the impairment of this inhibitory effect of brain CNP on secretion of vasopressin could be involved in the pathogenesis of DOCA-salt hypertension in rats.  相似文献

In order to study the functional development of a thymus in an experimental model, small pieces of adult rat thymic tissue were cultured for 9 days and implanted under the kidney capsule of littermates. The tissues were examined with a panel of antibodies raised against thymic and neural factors and neural crest cells at intervals from 5 to 13 days. At 5 days post-implantation, there were groups of L1+ cells within the implants that reacted with antibodies raised against neural and neural crest cell markers. L1+ cells were highly mitotic, rounded cells measuring 8.7 +/- 0.6 micrometer in diameter. Double immunostaining with different combinations of antibodies showed that 94% of the L1+ cells were also TH+, and many were HNK-1/NCAM+, PGP 9.5+, NGF+, chromogranin A+, VIP+, S100+, CGRP+, GAD+, and A2B5+. A few were also pan-cytokeratin+. These results indicate that these cells are derived from neural crest derived cells and belong to the neuroepithelial line of development. The L1+ cells were most numerous before nerves appeared (about Day 9) and reduced in number and extent as the thymus differentiated. The neural crest cells occasionally had long cytoplasmic extensions, but it was not possible to decide if they formed the nerves that appeared in the implants. Adult thymuses also contained a population of L1+ and HNK-1/NCAM+ cells, mainly in the subcapsular cortex, the septa, and the medulla. These cells could be a source of neural crest cells able to repopulate the implant. The adult thymus may always contain a reservoir of cells potentially capable of producing neuropeptides and transmitter factors required for thymic growth and regeneration.  相似文献

Pharmacological effects of acute treatment with venlafaxine (VEN), a clinically active antidepressant [a noradrenaline (NA) and 5-hydroxytryptamine (5-HT) reuptake inhibitor without any affinity for neurotransmitter receptors] were studied in mice and rats. VEN inhibited the reserpine- or apomorphine-induced hypothermia and enhanced the L-5-HTP-induced head twitches in mice. It reduced the immobility time in Porsolt's test in mice and rats, but either did not change the locomotor activity (mice) or decreased it (rats). VEN reduced the locomotor hyperactivity induced by amphetamine (AMP), apomorphine (APO) and quinpirole (QUI), as well as the APO-induced stereotypy; the stereotypy induced by AMP in rats was prolonged. VEN neither changed the clonidine-induced aggressiveness in mice nor the behavioral syndrome induced by oxotremorine in rats. The obtained results indicate that VEN, given acutely, shows a pharmacological profile similar to that of tricyclic NA and 5-HT reuptake inhibitors. In contrast to the antidepressants mentioned above, VEN does not exhibit an alpha 1-adrenolytic or a cholinolytic activity (in vivo tests).  相似文献

We previously reported early results of a new technique using a suture anchor to perform a modified Bankart reconstruction. That study included patients from two medical centers and had an average followup of only 1 year. This report includes patients from a single center with followup extended to a mean of 42 months (range, 33 to 61). Between April 1988 and August 1991, 53 patients with recurrent anterior glenohumeral instability underwent modified Bankart reconstruction with the use of a suture anchor. Thirty-two patients met inclusion criteria (identifiable Bankart lesion, open repair with suture anchors, and minimum followup of 2 years); 4 patients were lost to followup. There have been no complications as a result of this technique. Ninety-three percent of the patients in the study had objectively excellent or good results. There were 2 failures with recurrent anterior dislocation. The use of a suture anchor can simplify the Bankart reconstruction. At average followup of 3 years, 26 patients have returned to presurgery activity levels without recurrent dislocation or subluxation. However, careful attention to anchor placement at the junction of articular cartilage and the glenoid neck is necessary to avoid technical failure.  相似文献

A new preprocessing technique which removes unwanted notches and random noise generated by electronic scanners is presented. Using a 3 element by 3 element mask, the central picture element is altered from black to white, or vice versa, if the surrounding elements correspond to a predetermined pattern. A significant improvement in both subjective image quality and coding efficiency is observed.  相似文献
