The Diagnostic Performance of Ultrasonography in the Evaluation of Extrathyroidal Extension in Papillary Thyroid Carcinoma: A Systematic Review and Meta-Analysis

Extrathyroidal extension (ETE) in patients with papillary thyroid carcinoma (PTC) is an indication of disease progression and can influence treatment aggressiveness. This meta-analysis assesses the diagnostic accuracy of ultrasonography (US) in detecting ETE. A systematic review and meta-analysis were performed by searching PubMed, Embase, and Cochrane for studies published up to April 2022. The pooled sensitivity, specificity, and diagnostic odds ratio (DOR) were calculated. The areas under the curve (AUC) for summary receiver operating curves were compared. A total of 11 studies analyzed ETE in 3795 patients with PTC. The sensitivity of ETE detection was 76% (95%CI = 74–78%). The specificity of ETE detection was 51% (95%CI = 49–54%). The DOR of detecting ETE by US was 5.32 (95%CI = 2.54–11.14). The AUC of ETE detection was determined to be 0.6874 ± 0.0841. We report an up-to-date analysis elucidating the diagnostic accuracy of ETE detection by US. Our work suggests the diagnostic accuracy of US in detecting ETE is adequate. Considering the importance of ETE detection on preoperative assessment, ancillary studies such as adjunct imaging studies and genetic testing should be considered.     

Circulating MicroRNA Profiling Identifies Distinct MicroRNA Signatures in Acute Ischemic Stroke and Transient Ischemic Attack Patients

Transient ischemic attack (TIA) refers to a momentary neurologic deficit caused by focal cerebral, spinal or retinal ischemic insult. TIA is associated with a high risk of impending acute ischemic stroke (AIS), a neurologic dysfunction characterized by focal cerebral, spinal or retinal infarction. Understanding the differences in molecular pathways in AIS and TIA has merit for deciphering the underlying cause for neuronal deficits with long-term effects and high risks of morbidity and mortality. In this study, we performed comprehensive investigations into the circulating microRNA (miRNA) profiles of AIS (n = 191) and TIA (n = 61) patients. We performed RNA-Seq on serum samples collected within 24 hrs of clinical diagnosis and randomly divided the study populations into discovery and validation cohorts. We identified a panel of 11 differentially regulated miRNAs at FDR < 0.05. Hsa-miR-548c-5p, -20a-5p, -18a-5p, -484, -652-3p, -486-3p, -24-3p, -181a-5p and -222-3p were upregulated, while hsa-miR-500a-3p and -206 were downregulated in AIS patients compared to TIA patients. We also probed the previously validated gene targets of our identified miRNA panel to highlight the molecular pathways affected in AIS. Moreover, we developed a multivariate classifier with potential utilization as a discriminative biomarker for AIS and TIA patients. The underlying molecular pathways in AIS compared to TIA may be explored further in functional studies for therapeutic targeting in clinical translation.     

Dexmedetomidine Attenuates Methotrexate-Induced Neurotoxicity and Memory Deficits in Rats through Improving Hippocampal Neurogenesis: The Role of miR-15a/ROCK-1/ERK1/2/CREB/BDNF Pathway Modulation

Methotrexate (MTX) is a widely used neurotoxic drug with broad antineoplastic and immunosuppressant spectra. However, the exact molecular mechanisms by which MTX inhibits hippocampal neurogenesis are yet unclear. Dexmedetomidine (Dex), an α2-adrenergic receptor agonist, has recently shown neuroprotective effects; however, its full mechanism is unexplored. This study investigated the potential of Dex to mitigate MTX-induced neurotoxicity and memory impairment in rats and the possible role of the miR-15a/ROCK-1/ERK1/2/CREB/BDNF pathway. Notably, no former studies have linked this pathway to MTX-induced neurotoxicity. Male Sprague Dawley rats were placed into four groups. Group 1 received saline i.p. daily and i.v. on days 8 and 15. Group 2 received Dex at 10 μg/kg/day i.p. for 30 days. Group 3 received MTX at 75 mg/kg i.v. on days 8 and 15, followed by four i.p. doses of leucovorin at 6 mg/kg after 18 h and 3 mg/kg after 26, 42, and 50 h. Group 4 received MTX and leucovorin as in group 3 and Dex daily dosages as in group 2. Bioinformatic analysis identified the association of miR-15a with ROCK-1/ERK1/2/CREB/BDNF and neurogenesis. MTX lowered hippocampal doublecortin and Ki-67, two markers of neurogenesis. This was associated with the downregulation of miR-15a, upregulation of its target ROCK-1, and reduction in the downstream ERK1/2/CREB/BDNF pathway, along with disturbed hippocampal redox state. Novel object recognition and Morris water maze tests demonstrated the MTX-induced memory deficiencies. Dex co-treatment reversed the MTX-induced behavioral, biochemical, and histological alterations in the rats. These neuroprotective actions could be partly mediated through modulating the miR-15a/ROCK-1/ERK1/2/CREB/BDNF pathway, which enhances hippocampal neurogenesis.     

Microstructural evolution,dislocation density and tensile properties of Al-6.5Si-2.1Cu-0.35Mg alloy produced by different casting processes

Al-Si-Cu-Mg foundry alloys are used in casting process technologies.However,their strength proper-ties remain low due to their microstructural characteristics and porosity.In this work,the microstruc-tural characteristics,dislocation densities,and mechanical properties of Al-Si-Cu-Mg cast alloys prepared through different casting methods were studied experimentally.Four casting processes,namely,gravity casting (GC),rheocasting (RC),thixoforming (Thixo),and Thixo with heat treatment,were used.The GC and RC samples had mainly dendritic α-Al phase microstructures and exhibited coarse Si particles and intermetallic compounds in their interdendritic regions.By contrast,the Thixo and heat-treated Thixo(HT-Thixo) samples exhibited microstructural refinement with uniformly distributed α-Al globules,fine fibrous Si particles,and fragmented intermetallic compounds among α-Al globules.The accumulation of dislocation densities increased in the Thixo sample as the strain was increased due to plastic deforma-tion.Furthermore,the ultimate tensile strength and yield strength of the HT-Thixo sample increased by 87％ and 63％,respectively,relative to those of the GC sample.The cleavage fracture displayed by the GC and RC samples led to brittle failure.Meanwhile,the Thixo and HT-Thixo samples presented dimple-based ductile fracture.     

Effect of surface mechanical finishes on charging ability of electron irradiated PMMA in a scanning electron microscope

Charging of Polymethyl Methacrylate insulators (PMMA), in a scanning electron microscope (SEM) is studied owing to a time resolved current method. This method allows the evolution of trapped charge versus time and the charging time constant to be deduced. The effect of surface roughness change on the ability of PMMA to trapped charge is highlighted. The results show that the trapped charge at the steady state decreases when the roughness increases in the micrometer range while the time constant of charging increases with surface roughness. This behaviour is due to the increase of leakage current and/or enhanced secondary electron emission (SEE). On the one hand, surface mechanical finishes allows, the build up charge in insulators submitted to an electron bombardment to be lowered. On the other hand this treatment allows the secondary electron emission to be raised for some specific applications.
