OBJECTIVES: This study was designed to determine whether coronary vasospasm in patients with variant angina pectoris (VAP) may produce focal organic lesions at the site of vasospasm that would contribute to disease progression. BACKGROUND: Recent clinical angiographic and experimental studies have demonstrated the potential role of vasospasm in the worsening of organic coronary stenosis. METHODS: We studied histologically the coronary plaques obtained at atherectomy in 202 patients with moderate to severe coronary stenosis. This population included 22 patients with VAP, 100 patients with chronic stable angina and 80 patients with restenosis following angioplasty or atherectomy. Diagnosis of VAP was based on both the clinical feature of angina at rest associated with ST elevation and a positive response to acetylcholine provocation test. RESULTS: The most common histological appearance in 92% of patients with stable angina was hypocellular fibroatheromatous plaques, whereas neointimal hyperplasia was the characteristic feature of the plaque observed in 90% of patients with restenosis. The coronary specimens at the site of spasm in 15 of the 22 patients (68%) with VAP demonstrated intimal injuries such as neointimal hyperplasia (15), thrombus formation (2), and intimal hemorrhage (3). Neointimal hyperplasia was significantly more common in the patients with VAP as compared with those with stable angina (68% vs. 8%; p < 0.0001). A rapid progression of organic stenosis within three years was angiographically found in 5 of the 22 patients with variant angina. In all five cases, neointimal hyperplasia was the main contributor to the worsening of the organic lesion at the site of spasm. These histological findings in patients with VAP extremely resembled those in restenosis. Except for vasospasm, no factors significantly predicted the presence of neointimal formations in primary coronary lesions. CONCLUSIONS: Coronary vasospasm may provoke vascular injury that leads to the formation of neointima in VAP patients similar to that seen with restenosis. Coronary spasm may thus play a key role in the rapid coronary stenosis progression in certain patients with VAP.  相似文献

The mutual understanding of color‐normal observers (CNOs) and color‐defective observers (CDOs) is now essential because personal color information display environments have been widely adopted. However, existing tools for CDOs offer only color discrimination; they fail to support color impression (ie, saturation and contrast). Therefore, we need a novel tool that offers help in distinguishing opponent colors, while preserving color saturation. We introduce two key techniques for realizing this difficult goal. The former is the repeated sequential display of the original and processed images to support the formation of unified correct percepts that provide discrimination of both red‐green and yellow‐blue opponent colors. One image, ie, original, exhibits correct yellow‐blue but distorted red‐green information for CDOs while the other, ie, processed, provides synthesized distinguishable red‐green but confusable yellow‐blue information for CDOs; here, hue rotation (HR) is useful for advanced users whereas hue blending (HB) is suitable for general. The latter is realized by the real‐time video processing available on smartphones; our algorithms support direct processing of the digital component video signal formats (eg, Y, CR, and CB). Subjective tests suggest that the two above‐mentioned algorithms will, along with embedding a lightweight real‐time dichromatic simulation facility for CNOs, greatly help the mutual understanding of CNOs and CDOs.  相似文献

We propose a system for retrieving human locomotion patterns from tracking data captured within a large geographical area, over a long period of time. A GPS receiver continuously captures data regarding the location of the person carrying it. A constrained agglomerative hierarchical clustering algorithm segments these data according to the person's navigational behavior. Sketches made on a map displayed on a computer screen are used for specifying queries regarding locomotion patterns. Two basic sketch primitives, selected based on a user study, are combined to form five different types of queries. We implement algorithms to analyze a sketch made by a user, identify the query, and retrieve results from the collection of data. A graphical user interface combines the user interaction strategy and algorithms, and allows hierarchical querying and visualization of intermediate results. We evaluate the system using a collection of data captured during nine months. The constrained hierarchical clustering algorithm is able to segment GPS data at an overall accuracy of 94% despite the presence of location-dependent noise. A user study was conducted to evaluate the proposed user interaction strategy and the usability of the overall system. The results of this study demonstrate that the proposed user interaction strategy facilitates fast querying, and efficient and accurate retrieval, in an intuitive manner.  相似文献

Protein translocation to the extracellular space is essential for the invasion, colonization, and survival of pathogenic gram-negative bacteria within a host organism. In addition to the N-terminal signal sequence-dependent secretion system, which is specific for protein transport to the periplasmic space, there are five major systems (type I, II, III, IV, and V) that are known to be involved in protein secretion into the extracellular space. Of the systems, the type I pathway, which is composed of three membrane components including an ATP-binding cassette (ABC) protein, translocates proteins into the extracellular space from the cytosol by directly using the energy generated from ATP hydrolysis, and therefore, the system is a member of the ABC transporter family and is also known as the ABC exporter. To date, ABC exporters have been discovered to be involved in the secretion of a wide variety of exoproteins including RTX (repeats-in-toxin) toxins, cell surface layer proteins, proteases, lipases, bacteriocins, heme-acquisition proteins, and nodulation-related proteins such as the exoglucanases of gram-negative bacteria. A secretory protein and its associated specific ABC exporter are encoded in the same gene cluster in most cases, and ABC exporters show substrate specificity for secretion. Consequently, ABC exporters are present based primarily on the number of secretory protein genes. A secretion signal is situated in the C-terminal region of secretory proteins, however, the characteristics of the secretion signal are not fully understood. The detonation propagation phenomena in curved channels were experimentally studied in order to determine the stable propagation condition. A stoichiometric ethylene–oxygen mixture gas and five types of rectangular-cross-section curved channels with different inner radii of curvature were employed. The detonation waves propagating through the curved channels were visualized using a high-speed video camera. Multi-frame short-time open-shutter photography (MSOP) was developed in the present study to simultaneously observe the front shock shape of the detonation wave and the trajectories of triple points on the detonation wave. The detonation wave became more stable under the conditions of a higher filling pressure of the mixture gas and/or a larger inner radius of curvature of the curved channel. The critical condition under which the propagation mode of the detonation wave transitioned from unstable to stable was having an inner radius of curvature of the curved channel (ri) equivalent to 21–32 times the normal detonation cell width (λ). In the stable propagation mode, the normal detonation velocity (Dn) increased with the distance from the inner wall of the curved channel and approached the velocity of the planar detonation propagating through the straight section of the curved channel (Dstr). The smallest Dn was observed on the inner wall and decreased with decreasing ri/λ. The distribution of Dn on the detonation wave in the stable mode was approximately formulated. The approximated Dn given by the formula agreed well with the experimental results. The front shock shape of the detonation wave could be reconstructed accurately using the formula. The local curvature of the detonation wave (κ) nondimensionalized by λ decreased with increasing distance from the inner wall. The largest λκ was observed on the inner wall and increased with increasing ri/λ. Dn/Dstr decreased with increasing λκ. This nondimensionalized Dnκ relation was nearly independent of ri/λ.  相似文献

Ethanol metabolism-associated oxidative stress contributes to the pathogenesis of alcoholic liver disease. We examined the effect of a Mate tea extract on ethanol-induced liver injury in vitro and in vivo models. Isolated hepatocytes were incubated with ethanol. An extract of Yerba-Mate tea (EMT) was added to the cultures simultaneously with ethanol. EMT treatment suppressed the ethanol-induced increase in cell death by inhibiting cytochrome p450 2E1 (CYP2E1) activity, which is related to the production of reactive oxygen species. Furthermore, we examined the effects of EMT on serum transaminase activity, and the progression of liver fibrosis in rats treated with ethanol and CCl4. Rats were fed a diet that included 0.005% or 0.02% EMT or no EMT. For a period of 3 weeks, the animals were provided drinking water containing 5% ethanol and were also treated with carbon tetrachloride (CCl4) (0.1 ml/kg of body weight). EMT treatment suppressed plasma ALT and AST activities in the ethanol- and CCl4-treated rats. EMT treatment also decreased CYP2E1 expression and increased ADH expression in the ethanol- and CCl4-treated rats. EMT treatment fully protected the rats against ethanol- and CCl4-induced liver injury. These results suggest that EMT may serve as a candidate for preventing ethanol-induced liver injury.  相似文献
