Surgical management of congenital high vaginal atresia

In the past 15 years five neonates and children have been treated for high vaginal atresia at the Surgical Unit of Department of Pediatrics, University Medical School, Pécs, Hungary. In three of the six patients distal atresia of the vagina was found (Type III). Two of the three were neonates and had a large hydrometrocolpos and the third, a 13-year-old girl, hematometrocolpos. In the fourth patient the vaginal atresia was associated with cloacal and anorectal malformation (Type V). In the fifth there was a duplication of uterus and vagina; however, only one of the two vaginas was atretic. In three patients a pull-through of the vagina was carried out, in one of them according to Pena, in another a transvesical approach, as suggested by Monfort, was used. Following repeated surgeries in the patient with cloacal malformation the vagina was replaced with large intestine. In the case of duplication of vagina and uterus one half of the duplication was removed.     

Finite element modelling of calendering — some aspects of the effects of temperature gradients and structure inhomogeneities

The compression of a paper web in a calender nip has been simulated using the finite element method. The mechanical properties of the paper were allowed to vary in the thickness and machine directions of the web. This was done in order to model the influence of temperature gradients as well as density variations (due to the presence of fibre floes) on the deformation behaviour in the nip. Paper was assumed to be an elastic-plastic material exhibiting strain hardening. The yielding behaviour was governed by the Drucker-Prager yield condition. Simulations of the deformation behaviour when paper was subjected to a temperature gradient, clearly revealed that the deformation gradually became more concentrated towards the surface layers as the temperature of the surface increased. This is in accordance with experimental results which indicate that temperature-gradient calendering promotes the surface properties, whereas the bulk of the structure is preserved. Modelling the deformation behaviour of a structure containing density variations reveals that the paper may contain an inhomogeneous strain distribution after unloading, i.e. after passage through the nip.     

Impulse breakdown voltages of triggered multistage vacuum gap

Several sealed-off triggered vacuum gaps are connected in series to improve hold-off voltage. The characteristics of impulse breakdown voltage of these series-connected gaps are investigated experimentally. The sum hold-off voltage of series-connected gaps decreases to a unit hold-off voltage when the maximum value of voltage division ratio across the gaps increases to unity. Self-breakdown probability of the series-connected gaps is always higher than that of a single gap under the same conditions. Hence, stage efficiency of the multistage gap decreases with increasing number of stages. Its value is 90 percent with 2-stage gap and 75 percent with 5-stage gap, respectively, under the same voltage division ratio and the same gap length (2.0 mm) in each stage. Triggered breakdown voltage of 2- or 3- stage gap is several hundred volts when all gaps are triggered simultaneously at the peak of the main impulse wave and a working voltage range is nearly 100 percent in this case. The working voltage range decreases with number of stages. Its value is 45 percent with 3-stage gap and 15 percent with 5-stage gap, respectively, when one triggered gap is fired for switching.     

Zooplankton population in the river tigris: Effects of samarra impoundment

The influences of the Samarra impoundment on the ecology of the downstream sector of the river Tigris are investigated. Zooplankton were collected monthly from July 1987 to July 1988. It was found that zooplankton were most abundant during high river discharges in March. During the month of low river discharge, September, the zooplankton population was greatly reduced immediately below the barrage and remained so to the last downstream station. Several factors seem to explain such differences. The high discharge from the impoundment was found to be the dominating factor and may flush the small backwaters in which zooplankton were abundant, thus increasing the population in the river. Zooplankton species vary in their ability to sustain populations in the river, variations which are due mainly to species-specific characteristics.     

Vital-Fluorochromization of Microorganisms Using 3′,6′-Diacetylfluorescein to Determine Damages of Cell Membranes and Loss of Metabolic Activity by Ozonation

Cell suspensions of Escherichia coli as a model for bacterial populations in wastewaters were treated with ozone as a disinfectant in a continuously operated pilot plant with a plug flow reactor. Suspensions with an initial number of CFU (colony forming units) of 10⁸ mL^?1 were ozonized with ozone concentrations up to 12 mg/L. Metabolic activities and membrane functions break down with increasing ozone concentrations. The fluorochromization using 3′,6′-diacetylfluorescein (FDA) proved to be a suitable method for the detection of an alteration in permeability of the cell membranes and an inactivation of metabolic activity. By fluorescence microscopic and photometric investigations it could be clearly demonstrated that, in the case of disinfection with ozone, inactivation of the cells includes first of all a damage of the cell membranes. In contrast to the determination of the number of CFU, fluorochromization allows the detection of alteration in metabolic activities.     

Cytotoxicity versus antibacterial activity of some antiseptics in vitro

The relationship between endometriosis and infertility is observed frequently. Patients with both conditions require a conservative approach to their management. Since hormonal therapy is one of those approaches, we sought to compare the efficacy of Danazol and Gestrinone in 80 infertile patients (48 and 32, respectively). Therapy lasted 6 months in both treatment groups, and all patients studied had laboratory tests performed and were clinically evaluated and classified through laparoscopy before and after therapy. The improvement of symptoms and favorable follow-up were similar with both treatments. The reestablishment of menstrual patterns and fertility were also nearly alike in both groups. However, Gestrinone was associated with fewer secondary effects and is easier to administer than Danazol. We conclude that Gestrinone is a useful medication in the management of the infertile patient with endometriosis.
