BACKGROUND: The ideal urological management for the patients with cervical spinal cord injury (CSCI) is to obtain catheter free urination and to prevent urinary tract complications. We have evaluated cases that had undergone transurethral anterior sphincterotomy from the view-point of the operative indications and the efficacy. METHODS: We carried out sphincterotomy 166 times on 133 male patients with CSCI in our Center. Before the operations were performed, all patients suffered from urinary incontinence, and they were unable to catheterize themselves for low level activity of daily life. Before and after the operation, their detrusor functions with sphincter reactions were assessed by urodynamic study. In principle we have followed up these cases by cystogram combined with cystometry, cystogram and excretory-pyelography or abdominal ultrasonography. RESULTS: In post-operative evaluations, more than 80% of cases attained hypotonic detrusor contractions and residual urine was significantly decreased. In long term follow-up, 96% of patients had obtained catheter free urination and about 85% of patients had no urinary tract complications, such as bladder deformity, vesicoureteral reflux, or hydronephrosis, with the exception of common unavoidable urinary infections. About 20% of cases had to be re-operated upon, and some cases showed hypertonic detrusor contractions or detrusor-sphincter-dyssynnergia during follow-up. CONCLUSION: The operative indications of sphincterotomy should be decided when the CSCI patients is unable to perform self-catheterization, and when due to the dysfunction of the urethral sphincter, these patients suffered from voiding difficulties or autonomic dysreflexia, or when the urinary tract complications might occur. In the majority of cases the aims of the sphincterotomy were achieved but some cases underwent another operation or had recurrent dysfunction of the urethral sphincter, indicating the need for careful follow-up. 相似文献

It is important to detect predictive factors for in-hospital and delayed mortality of patients with subarachnoid hemorrhage (SAH) due to ruptured aneurysm. Forty-eight patients with initial bleeding of aneurysmal SAH were referred to our hospital from January 1982 to December 1985. In-hospital mortality was 16.7% (8 to 48), and 15% (6) of forty patients died later during the follow-up period. Using the Kaplan-Meier method we were able to conclude that, cumulatively, there was 70.8% probability that much patients would survive for 10 years. We analysed predictive factors of in-hospital and delayed mortality retrospectively. The most significant predictive factor for in-hospital mortality was SAH grading on admission, and for delayed mortality (29.2%) age on admission was the best predictive factor. In fact, two patients died with cardiac event during the follow-up period. This result suggests that, although the SAH grading on admission was the second most significant factor for delayed mortality, patients who survived in the acute phase had a survival probability similar to those in a normal control group. 相似文献

A large variety of cheeses exist in Brazil, reflecting historical and cultural aspects. Brazilian artisanal cheeses present differences in the processing, ripening time (when applied), type of milk used, texture, size, shape, color, moisture content, flavor, the use or not of starter cultures, among others. This review describes the main artisanal cheeses produced in Brazil, focusing on general and particular characteristics associated with their making process and geographical identity. Overall, the high variability of the physicochemical data and deficiency of information on sensorial properties of Brazilian artisanal cheeses were noticed. On the other hand, culture‐dependent methods were mostly used to expand the knowledge into the microbiology of these cheeses, whereas their microbial diversity has been recently discovered through the use of 16S rRNA gene sequencing‐based methods. The certification of a geographical indication for Brazilian artisanal cheeses may encompass an essential milestone for adding value to these products. Regardless of their significance in the diet, culture, and economy of producing regions, taken together, the reviewed literature discloses the need of insightful studies to generate scientific data to support the expansion of the market, while ensuring the protection of historic aspects related to the production of Brazilian artisanal cheeses. 相似文献

We have previously proposed a new type of information-theoretic method where a neuron is evaluated by itself (self-evaluation) and by its surrounding neurons (outer-evaluation). If contradiction between different types of evaluation exists, it is reduced as much as possible. In the present paper, we try to separate self- and outer-evaluation more explicitly and introduce the importance of neurons. First, we separate self- and outer-evaluation to enhance the characteristics shared by the two types of evaluation. Second, we introduce the importance of neurons in evaluation. By using a limited number of important neurons in evaluation, we expect the main characteristics in input patterns to emerge. We applied this contradiction resolution to two types of data, namely, the Senate data and the Euro-yen exchange rates. In both data sets, experimental results confirmed that improved prediction performance was obtained. Prediction performance was better than that obtained by the conventional self-organising map (SOM) and radial basis function networks. In addition, final representations obtained by contradiction resolution were easier to interpret than those given by the conventional SOM. Experimental results confirmed that improved interpretation and visualisation were accompanied by improved prediction performance. 相似文献

This work evaluated the effect of pressure and temperature on yield and characteristic flavour intensity of Brazilian cherry (Eugenia uniflora L.) extracts obtained by supercritical CO2 using response surface analysis, which is a simple and efficient method for first inquiries. A complete central composite 22 factorial experimental design was applied using temperature (ranging from 40 to 60 °C) and pressure (from 150 to 250 bar) as independent variables. A second order model proved to be predictive (p ≤ 0.05) for the extract yield as affected by pressure and temperature, with better results being achieved at the central point (200 bar and 50 °C). For the flavour intensity, a first order model proved to be predictive (p ≤ 0.05) showing the influence of temperature. Greater characteristic flavour intensity in extracts was obtained for relatively high temperature (> 50 °C). Therefore, as far as Brazilian cherry is concerned, optimum conditions for achieving higher extract yield do not necessarily coincide to those for obtaining richer flavour intensity.Industrial relevanceSupercritical fluid extraction (SFE) is an emerging clean technology through which one may obtain extracts free from organic solvents. Extract yields from natural products for applications in food, pharmaceutical and cosmetic industries have been widely disseminated in the literature. Accordingly, two lines of research have industrial relevance, namely, (i) operational optimization studies for high SFE yields and (ii) investigation on important properties extracts are expected to present (so as to define their prospective industrial application). Peierls stresses (τP) of dislocations of 66 slip systems in 52 crystals were estimated from experimental data either by direct extrapolation of the critical resolved shear stress (τc) vs. temperature curve to absolute zero temperature, or from the T0 value at which the temperature dependence of τc vanishes, based on the kink-pair formation enthalpy, which is a function of τP, described by the line tension model of the dislocation. The normalized τP/G (G is the shear modulus) values are distributed over four orders of magnitude, but τP/G values for a group of crystals with the same crystal structure are within an order of magnitude, indicating a homologous nature of τP in crystals. In order to compare the results with the Peierls–Nabarro (P–N) formula generalized to any dislocation character, log(τP/G) values were correlated with crystal parameters. In this generalized P–N plot, most of the plots deviate downwards from the Huntington relation (a revised, original P–N relation) and the results of the discretized P–N models of Ohsawa et al. (Ohsawa K, Koizumi H, Kirchner HOK, Suzuki T. Philos Mag A 1994;69:171), with the deviation becoming larger at large h/δ value, where h is the lattice spacing of the glide plane, and δ is the period of the lattice in the direction of the dislocation glide. In the plot, there is a tendency that the stronger the covalent character, the higher the τP/G value, reflecting the general tendency of the normalized theoretical shear strength of crystals. 相似文献

Juice and integument of leaves of 3 Aloe species, Aloe vera, A. ferox and A. africana, are not allowed to be used as food according to the Pharmaceutical Affairs Law in Japan. On the other hand, whole leaves of A. arborescens can be used as food. The present study was designed to distinguish Aloe species by random amplified polymorphic DNA (RAPD) analysis. DNA was isolated from fresh and dried leaves of the 4 Aloe species. Five out of 32 different 10-mer primers examined were useful for analysis. By comparison of the characteristic bands of PCR products on agarose gel, it was possible to distinguish the 4 species. Thus, the botanical species of Aloe in commercial food products can be identified by RAPD analysis. 相似文献

This paper concerns the development of an entirely new sensor for measuring forces using a gyroscope (called gyroscopic force measuring system, or simply GFMS) for measuring a force vectorially. In a previous paper [S. Kurosu, M. Adachi, K. Kamimura, Dynamical characteristics of gyroscopic weight measuring device, ASME J. Dynamic System Measurement, and Control 119 (1997) 346–350], the dynamics of the GFMS for measuring a vertical force were investigated and the principal characteristics were examined theoretically and experimentally. The results of this work are directly applicable to measurement of a vectorial force in three-dimensional space. The principle and the dynamical characteristics of the GFMS for measuring a force vector are analyzed theoretically. To measure a force vectorially, two auxiliary turntables (driven by servomechanisms) are installed around the gyroscope, in which turntable outputs are required to follow some angles of incidence of a force vector. Some unfavorable errors caused by various factors and disturbances are analyzed. Two types of compensation methods are proposed as a device both for accurate force measurement and disturbance suppression. The feasibility of the proposed GFMS is confirmed by numerical simulations. 相似文献
