BACKGROUND: Foot complications in diabetes incur huge human and medical costs. There is a high incidence of complicated diabetes mellitus in Far North Queensland, particularly in the indigenous communities. METHODS: An audit of 51 patients admitted to Cairns Base Hospital from July 1992 to December 1994 with diabetic foot complications requiring surgical intervention was performed. RESULTS: The majority of surgical procedures were for serious infections in neuropathic feet precipitated by minor trauma. Surgical procedures included debridement/drainage, minor amputation, major amputation and arterial bypass. Half of the patients required more than one procedure and/or multiple admissions. Average length of stay was 48 days. Indigenous patients were overrepresented in this audited group. CONCLUSIONS: Prevention or minimization of diabetic foot complications can be achieved only through improvement in education and vigilance at all levels of the treatment process from community to hospital. Special attention to the needs of indigenous communities with a high incidence of diabetes must be a feature of any future initiatives.  相似文献

In this study, white organic electroluminescent devices with microcavity structures were developed. A flexible high‐resolution active‐matrix organic light‐emitting diode display with low power consumption using red, green, blue, and white sub‐pixels formed by a color‐filter method was fabricated. The microstructure development during plastic deformation was reviewed for iron and steel which were subjected to cold rolling or mechanical milling (MM) treatment, and the change in strengthening mechanism caused by the severe plastic deformation (SPD) was also discussed in terms of ultra grain refinement behavior. The microstructure of cold-rolled iron is characterized by a typical dislocation cell structure, where the strength can be explained by dislocation strengthening. It was confirmed that the increase in dislocation density by cold working is limited at 1016m−2, which means the maximum hardness obtained by dislocation strengthening is HV3.7 GPa. However, the iron is abnormally work-hardened over the maximum dislocation strengthening by SPD of MM because of the ultra grain refinement caused by the SPD. In addition, impurity of carbon plays an important role in such grain refinement: the carbon addition leads to the formation of nano-crystallized structure in iron.  相似文献

In this paper, fuzzy rule-based systems are applied to a point-to-point car racing game. In the point-to-point car racing game, two car agents compete with each other for taking waypoints. There are three waypoints in the car racing field, each of which is assigned a number that indicates the order to take. The control process of car agents is modeled as a non-holonomic system where there are two input variables (acceleration and steering) for controlling the position, angle and velocity of the car agents. Fuzzy rule-based systems are used to make a high-level decision where the target waypoint to take is determined. Since a fuzzy rule-based system for the high-level decision making is generated in the manner of supervised learning, a set of training patterns should be given for the construction of the fuzzy rule-based systems. In this paper we examine two methods to obtain such a set of training patterns. We also examine two representations of input vectors for the fuzzy rule-based systems. We discuss the effect of obtained training patterns and the input representation on the performance of the fuzzy rule-based systems. After discussing and analyzing the experimental results, we present an adaptive framework of fuzzy rule-based systems. The performance of adaptive fuzzy rule-based systems is then examined based on the results of their non-adaptive version. A series of computational experiments are performed to show the learning ability of the adaptive fuzzy rule-based systems.  相似文献

We consider a production system in a general configuration with a new control strategy: the push mechanism for the part transport and the kanban technique for the work-in-process (WIP). The production system is composed of many stations such as an entrance station, a set of work stations, a central station, and an exit station, that are arranged in a general configuration. The push mechanism is followed for transporting a part from a station to a destination station. The kanban technique is adopted for controlling the WIP in a work station. The production system is modeled by a closed queuing network in a general configuration with a Markov part sending mechanism and a machine no blocking (MNB) technique. An optimal part sending policy that maximizes the expected system throughput is formulated into a long run average semi-Markov decision process. Three solution approaches are developed for obtaining optimal or suboptimal solutions. Numerical examples are given to evaluate the quality of the solutions obtained by the solution approaches  相似文献

Fracture toughness of silicon crystals has been investigated using indentation methods, and their surface energies have been calculated by molecular dynamics (MD). In order to determine the most preferential fracture plane at room temperature among the crystallographic planes containing the 〈001〉, 〈110〉 and 〈111〉 directions, a conical indenter was forced into (001), (110) and (111) silicon wafers at room temperature. Dominant {110}, {111} and {110} cracks were introduced from the indents on (001), (011) and (111) wafers, respectively. Fracture occurs most easily along {110}, {111} and {110} planes among the crystallographic planes containing the 〈001〉, 〈011〉 and 〈111〉 directions, respectively. A series of surface energies of those planes were calculated by MD to confirm the orientation dependence of fracture toughness. The surface energy of the {110} plane is the minimum of 1.50 Jm−2 among planes containing the 〈001〉 and 〈111〉 directions, respectively, and that of the {111} plane is the minimum of 1.19 Jm−2 among the planes containing the 〈011〉 direction. Fracture toughness of those planes was also derived from the calculated surface energies. It was shown that the K IC value of the {110} crack plane was the minimum among those for the planes containing the 〈001〉 and 〈111〉 directions, respectively, and that K IC value of the {111} crack plane was the minimum among those for the planes containing the 〈011〉 direction. These results are in good agreement with that obtained conical indentation.  相似文献

No Heading The new technique for embedding microimpurities into solid helium has been developed. A He crystal was grown in a vertical cylindrical cell placed in the bath of pumped LHe cryostat. The He solid was continuously moved downward by exhaust of helium from the cell bottom and was simultaneously restored from its upper edge by sedimentation of a jet of gas helium mixed with impurity. A guest particle density of 3· 1019 per cc and stable during an hour doped crystal growth with the rate of 1 mm per 20 s were achieved. The solid Deuterium clusters isolated in solid Helium have been investigated at T = 1.3K and P = 3 MPa by CARS technique. The strong effect of Raman scattering sensitivity to the molecular environment nuclear spin has been found in CARS - the ratio of probabilities for the scattering by para (J=1) and ortho (J=0) deuterium being equal to 1 in a gas is as high as 10,000 in nearly pure o-D2, whereas it is 50 in common Raman scattering. In this study, albumin or γ-globulin, both of which are included in natural synovial fluid, was used as an additive into lubricants to investigate the ability to reduce the friction for poly(vinyl alcohol) hydrogel in mixed lubrication. It was found from a measurement in circular dichroism that albumin contains a large amount of α-helix structure and γ-globulin contains a large amount of β-sheet structure. The lubricant containing only albumin showed low friction compared to the lubricant containing only γ-globulin. The effect of protein boundary film was clarified by changing lubricant. Albumin kept the friction low after changing from γ-globulin used at initial rubbing, but γ-globulin increased the friction after changing from albumin at initial rubbing. From a sliding distance of 600 m, albumin showed lower friction but γ-globulin showed higher friction. Therefore, in the case of friction decreasing, γ-globulin forms a tight adsorbed layer and subsequently albumin forms a layer with low shearing strength. Hence, it is important to apply the adsorption layer of γ-globulin at the bottom and make a layered structure composed of albumin and γ-globulin to reduce both friction and wear.  相似文献
