OBJECTIVES: We sought to evaluate the effects of combined administration of infra-low dose dipyridamole and low dose dobutamine on assessment of myocardial viability. BACKGROUND: Low dose pharmacologic stress echocardiography with either dobutamine or dipyridamole infusion has been proposed for the recognition of myocardial viability. METHODS: Thirty-four patients with rest wall motion dyssynergy by two-dimensional echocardiography and with angiographically proved coronary artery disease underwent in combination with two-dimensional echocardiographic monitoring: 1) low dose (5 to 10 microgram/kg per min over 3 min) dobutamine infusion; 2) infra-low dose (0.28 mg/kg over 4 min) dipyridamole infusion; 3) combination of infra-low dose dipyridamole infusion immediately followed by low dose dobutamine infusion (combined dipyridamole-dobutamine). RESULTS: Follow-up rest echocardiography was available in 30 patients. After revascularization, 82 segments showed a contractile improvement of > or = 1 grade, whereas 63 segments remained unchanged. The sensitivity of dobutamine, dipyridamole and combined dipyridamole-dobutamine for predicting recovery was 72% (95% confidence interval [CI] 60.9% to 81.3%), 67% (CI 55.8% to 77%) and 94% (CI 86.3% to 97.9%), respectively. The specificity of dipyridamole, dobutamine and combined dipyridamole-dobutamine was 95% (CI 86.7% to 99%), 92% (CI 82.4% to 97.3%) and 89% (CI 78.4% to 95.4%), respectively. The accuracy of the dobutamine, dipyridamole and combined dipyridamole-dobutamine test was 80%, 79% and 92%, respectively (combined dipyridamole-dobutamine vs. dobutamine, p < 0.05; combined dipyridamole-dobutamine vs. dipyridamole, p < 0.01). CONCLUSIONS: Infra-low dose dipyridamole added to low dose dobutamine recruits an inotropic reserve in asynergic segments that were nonresponders after either dobutamine or dipyridamole alone and destined to recover after revascularization. 相似文献

Isothermal holding tests were carried out on the Fe-Ni-Cr system in the two-phase region between the melt and crystal in order to determine the γ and δ liquidus surfaces in the iron-rich corner, to establish the position of the peritectic line running between them, and to compare the results with those contained in relevant literature. The initial contents of the 13 test series were staggered in roughly equal intervals from 29.6% Ni and 12.9% Cr to 18.7% Ni and 28.4% Cr. They were held between the temperatures of 1450 and 1530 °C at intervals of 10 °C for 40 minutes in each case, so that 93 liquidus concentrations and temperatures were determined. Both liquidus surfaces can be described very accurately with corresponding mathematical equations and mathematically intersected in order to determine the course of the peritectic line. The results enable sufficiently accurate development of the melting equilibria of the entire ternary system, building on the basis provided by the known γ and δ liquidus lines of the three binary systems. 相似文献

In the paper we present the results of our research on optical fiber sensors embedded into composite material samples. We investigate the influence of the lamination process, axial orientation of an optical fiber sensor and coating of a fiber on stress monitoring of a composite material. In the paper we present two approaches to the case of composite condition monitoring, using a polarimetric fiber optic sensor as well as fiber Bragg gratings. We also present experimental evidence that interaction between a composite material and fiber optic sensors is very significant and depends on many factors such as fiber optic axial orientation and the coating layer surrounding an optical fiber. 相似文献

ABSTRACT: Evidence exists regarding the potential role of exogenous nucleotides as regulators of the immune function in physically active humans, yet the potential use of nucleotides has been hindered by their low bioavailability after oral administration. We conducted a double-blind, placebo-controlled, randomized trial to assess the effect of sublingual nucleotides (50?mg/day) on salivary and serum immunity indicators as compared to placebo, both administered to healthy males aged 20 to 25?years for 14?days. Sublingual administration of nucleotides for 14?days increased serum immunoglobulin A, natural killer cells count and cytotoxic activity, and offset the post-exercise drop of salivary immunoglobulins and lactoferrin (P?0.05), with no adverse effects reported. No significant differences in fasting salivary antimicrobal proteins (α-amylase, lysozyme and lactoferrin) were found before or after the treatment (P?>?0.05). It seems that sublingual administration of nucleotides for two weeks considerably affected immune function in healthy males. 相似文献

The term resilience, which has been present in science for almost half a century, stands for the capacity of some system needed to overcome an amount of disturbance from the environment in order to avoid a change to another stable state. In medicine, the concept of resilience means the ability to deal with daily stress and disturbance to our homeostasis with the intention of protecting it from disturbance. With aging, the organism becomes more sensitive to environmental impacts and more susceptible to changes. Mental disturbances and a decline in psychological resilience in older people are potentiated with many social and environmental factors along with a subjective perception of decreasing health. Distinct from findings in younger age groups, mental and physical medical conditions in older people are closely associated with each other, sharing common mechanisms and potentiating each other’s development. Increased inflammation and oxidative stress have been recognized as the main driving mechanisms in the development of aging diseases. This paper aims to reveal, through a translational approach, physiological and molecular mechanisms of emotional distress and low psychological resilience in older individuals as driving mechanisms for the accelerated development of chronic aging diseases, and to systematize the available information sources on strategies for mitigation of low resilience in order to prevent chronic diseases. 相似文献
