Pefloxacin plus metronidazole versus netilmicin plus metronidazole in the prevention of nosocomial infections during contaminated surgery. Surgical prophylaxis is widely used in contaminated surgery, especially colorectal surgery. In this clinical trial the efficacy of pefloxacin 800 mg i.v. slow infusion associated to metronidazole 500 mg i.v. 1-2 hours before surgery and then metronidazole alone after 6 and 12 hours versus netilmicin 200 mg i.m. associated to metronidazole 500 mg i.v. 1-2 hours before surgery and then both after 6 and 12 hours were evaluated in 97 patients suffering by colorectal surgery. Efficacy of prophylaxis in patients was evaluated in terms of appearance of post-surgical infections (abdominal, urinary, respiratory and wound infections). In pefloxacin + metronidazole group (53 patients), two cases of wound infections (3.8%) and three cases of respiratory infections (5.8%) were observed. In netilmicin + metronidazole group (44 patients), two cases of wound infections (4.9%), three cases of urinary infections (7%), three cases of respiratory infections (7.5%) and one case of intra-abdominal infection were observed. Our data confirmed that in colorectal surgery, the association pefloxacin, drug with microbiological and pharmacokinetics characteristics suitable for prophylaxis + metronidazole, active against anaerobes pathogens, prevents post-surgical infections as well as a reference association (netilmicin + metronidazole), with the advantage of a single administration. 相似文献

Drinking was studied in water-deprived rats with chronic intragastric fistulas. Sham drinking, by which the ingested water was immediately lost through the open gastric fistula, was approximately four times greater than normal drinking. This proportion held across different levels of water deprivation, following 5-ml intragastric preloads of water or isotonic saline or ingestion of isotonic saline. The leaves of the mangroveCeriops tagal contained 3.2–4.1% (all percentages relate to dry weight) ofd-1-O-methyl-muco-inositol previously unreported in mangroves. They consisted of 37% aqueous acetone-water-soluble material, 18% water-insoluble polysaccharides, and ca. 50% polyphenols, which include soluble and insoluble tannins and lignin. The polysaccharide component sugars were glucose, arabinose, uronic acids, mannose, xylose, galactose, and rhamnose in the proportions 28262210752, respectively. The leaves were pectate rich, and the low level of glucan was presumed to consist mainly of cellulose. After four weeks of biodegradation, ca. 60% of the acetone-water-soluble material was lost from the leaves. Degradation processes greatly altered the polysaccharide components in the leaves. Pectates were rapidly degraded, while other polysaccharides, although reduced proportionately, resisted degradation at about the same level, and all component sugars were found in the 8-week-old leaves. Apparent lignin contents increased from 15 to >30% during biodegradation up to eight weeks. The yields of the major fractions in corresponding fecal material fromNeosarmatium smithi showed a similar trend to the diets. An enrichment of the insoluble residue was noticeable due to the digestion of dialyzable material. The fecal carbohydrate content was greatly reduced (7–11%) and the apparent lignin increased (27–39%) due to its resistance to degradation. All dietary polysaccharide component sugars were found in the fecal residues, including some uronic acids. Protein similarity estimations can be achieved using reduced dimensional representations and we describe a new application for the generation of two-dimensional maps from the three-dimensional structure. The code for the dimensionality reduction is based on the concept of pseudo-random generation of two-dimensional coordinates and Monte Carlo-like acceptance criteria for the generated coordinates. A new method for calculating protein similarity is developed by introducing a distance-dependent similarity field. Similarity of two proteins is derived from similarity field indices between amino acids based on various criteria such as hydrophobicity, residue replacement factors and conformational similarity, each showing a one factor Gaussian dependence. Results on comparisons of misfolded protein models with data sets of correctly folded structures show that discrimination between correctly folded and misfolded structures is possible. A negative temperature differential between fruits or vegetables and the water in which they are immersed theoretically enhances infiltration of water and any microorganisms it might contain into tissues. The effect of temperature differentials between cantaloupes and wash water, each at 4 and 30 degrees C, on changes in cantaloupe weight and populations of Salmonella enterica Poona recovered from rinds and stem scar tissues of Eastern and Western (shipper) types of cantaloupes was assessed. The percent weight increase in Western cantaloupes was significantly greater (P < or = 0.05) than that in Eastern cantaloupes for all cantaloupe and inoculum temperature combinations. Salmonella Poona attachment to or infiltration of Eastern but not Western cantaloupe rind is enhanced when the fruit is at 4 degrees C, compared with 30 degrees C, regardless of the temperature of the immersion suspension. The number of Salmonella Poona cells recovered from rind tissue of Western cantaloupes at 30 degrees C immersed in inoculum at 30 degrees C was significantly less (P < or = 0.05) than that recovered from rind tissues of cantaloupes at 4 or 30 degrees C that were immersed in inoculum at 4 degrees C. Salmonella Poona in immersion water can adhere to or infiltrate surface tissues of cantaloupes. The populations of Salmonella Poona recovered from stem scar tissues of Eastern and Western types of cantaloupes were not significantly (P > 0.05) affected by cantaloupe and inoculum temperature combinations. Populations of cells adhering to or infiltrating various cantaloupe tissues is not dictated entirely by temperature differentials between fruits and immersion suspensions: rather, it also apparently is influenced by structures unique to surface tissues. 相似文献
