OBJECTIVE: To propose a definition for stage IA1 cervical adenocarcinoma, based on the International Federation of Gynecology and Obstetrics (FIGO) staging system, and to determine if patients meeting criteria might be candidates for conservative surgery. METHODS: Two hundred women were diagnosed with early-stage cervical adenocarcinoma from 1982 to 1996. Histopathologic sections were reviewed by a gynecologic pathologist. Medical records were reviewed, and patients included in this study had microscopically identifiable lesions, up to 3 mm invasive depth, up to 7 mm tumor width, and negative margins if cone biopsy was performed. RESULTS: Twenty-one patients with microinvasive adenocarcinoma met criteria for FIGO stage IA1 carcinoma of the cervix. The median (range) follow-up was 76 (30-172) months and median (range) patient age was 38 (24-75) years. Definitive treatment included type II or III radical hysterectomy in 16 cases, simple abdominal or vaginal hysterectomy in four cases, and loop electrosurgical excision procedure in one case; one patient received adjuvant pelvic radiation. The histologic subtypes were endocervical adenocarcinoma in 18 cases, adenosquamous carcinoma in two cases, and clear-cell adenocarcinoma in one case. There was no evidence of parametrial invasion or lymph node metastases in any patient who had radical surgery, and there were no disease recurrences. CONCLUSION: Patients with microinvasive adenocarcinoma who met criteria for FIGO stage IA1 cervical carcinoma had disease limited to the cervix, and conservative surgery, such as cone biopsy or simple hysterectomy, might offer them definitive treatment. 相似文献

We report transmittance and conductivity measurements of aluminum-doped zinc oxide films grown by atomic layer deposition. The results show that the films have 80–90% transmittance in the visible region and good transmittance in the infrared. To our knowledge, this is the first time that the transmittance of aluminum-doped zinc oxide is reported to extend beyond 2500–5000 nm. Following annealing, an optimal sheet resistance of 25 Ω/□ was obtained for a 575 nm thick film with a carrier density of 2.4 × 1020 cm?3 without compromising the transmittance in the visible regime. 相似文献

When developing an indicator to detect temperature heterogeneities in a high pressure (HP) vessel, the temperature sensitivity and the broadness of the application window have to be weighed against each other. In this study, the potential of a Bacillus amyloliquefaciens α-amylase (BAA)-based indicator for mild pasteurization conditions was evaluated. Solvent engineering was successfully used to reach two objectives: (i) to shift the pressure,temperature-stability of BAA in the HP pasteurization range; (ii) to differentiate the pressure,temperature-range of BAA inactivation from the pressure,temperature-range of inactivation of a previously developed Bacillus subtilis α-amylase (BSA)-based indicator system (1 g/L – 0.05 M MES pH 5.0), towards milder processing conditions. The BAA-system (0.1 g/L – 0.1 M MES pH 5.0) was kinetically calibrated and modeled under static conditions. As these model parameters were inadequate to predict the residual α-amylase activity of BAA under dynamic conditions correctly, parameters were determined under dynamic conditions and subsequently successfully validated on an independent dynamic data set. Based on the kinetic model and parameters valid under dynamic conditions, isorate contour plots for different inactivation levels were constructed, demonstrating clearly the different application windows of the BAA- and BSA-systems. 相似文献

We report on operant conditioning and artificial neural network (ANN) simulations aimed at further elucidating mechanisms of black-capped chickadee chick-a-dee call note category perception. Specifically, we tested for differences in the speed of acquisition among different discrimination tasks and, in two selected discrimination groups, searched for evidence of peak shift. Earlier, unreported ANN data were instrumental in providing the motivation for the current set of studies with chickadees and are provided here. The ANNs revealed differences in the speed of learning among note-type discrimination groups that is related to the degree of perceptual similarity among the three note types tested (i.e., A, B, and C notes). In many respects, bird and network results were in agreement (i.e., in the observation of peak shift in the same group), but they also differed in important ways (i.e., all discrimination groups showed differences in speed of learning in simulations but not in chickadees). We suggest that the start, peak and end frequency of the chick-a portion of chick-a-dee call notes, which form a graded but overlapping continuum, may drive the peak shift observed. Watta and Hassoun (1996) proposed a coupled gradient neural network for mixed integer programming. In this network continuous neurons were used to represent discrete variables. For the larger temporal problem they attempted many of the solutions found were infeasible. This paper proposes an augmented Hopfield network which is similar to the coupled gradient network proposed by Watta and Hassoun. However, in this network truly discrete neurons are used. It is shown that this network can be applied to mixed integer programming. Results illustrate that feasible solutions are now obtained for the larger temporal problem. 相似文献

Glaucoma is a group of optic neuropathies that leads to irreversible vision loss. The optic nerve head (ONH) is the site of initial optic nerve damage in glaucoma. ONH-derived lamina cribrosa (LC) cells synthesize extracellular matrix (ECM) proteins; however, these cells are adversely affected in glaucoma and cause detrimental changes to the ONH. LC cells respond to mechanical strain by increasing the profibrotic cytokine transforming growth factor-beta 2 (TGFβ2) and ECM proteins. Moreover, microRNAs (miRNAs or miR) regulate ECM gene expression in different fibrotic diseases, including glaucoma. A delicate homeostatic balance between profibrotic and anti-fibrotic miRNAs may contribute to the remodeling of ONH. This study aimed to determine whether modulation of miRNAs alters the expression of ECM in human LC cells. Primary human normal and glaucoma LC cells were grown to confluency and treated with or without TGFβ2 for 24 h. Differences in expression of miRNAs were analyzed using miRNA qPCR arrays. miRNA PCR arrays showed that the miR-29 family was significantly decreased in glaucomatous LC cell strains compared to age-matched controls. TGFβ2 treatment downregulated the expression of multiple miRNAs, including miR-29c-3p, compared to controls in LC cells. Building rehabilitation is critical for numerous older urban areas, many of which have inadequate foundations to support new demands. Consequently, development of practical methods to strengthen existing foundations is crucial. In engineering practice, both subsurface grouting and helical piers have been widely used to address these issues by strengthening the foundation. If the solid shaft of a typical helical pier is replaced by a hollow shaft, then helical piers provide the ability to deliver grout. It is hypothesized that these grouted helical pier systems could address foundation strengthening needs. This paper presents findings from an exploratory research program where grouting and pier placement tools were developed and tested on the large geotechnical centrifuge at the University of California, Davis. Experimental methods and procedures developed are presented, and observations regarding the formation of grout bulbs under different conditions are analyzed. Physical observation of the test specimens indicates that average grout bulb diameters of 0.6–1.9 times the helix diameter (Dh) are attainable. For similar grout mixes, 20–50% larger grout bulbs can be attained by adding just a modest amount of injection pressure. Future research may use these results to develop load performance data. 相似文献
