In order to achieve the earliest possible benefits of the investment in the Thames Water ring main, in late 1989 it was recognized that it was necessary to establish a commissioning team. The remit for the team was to progressively commission the ring main to match the construction programme, without risking the integrity of the daily water supply.

The paper deals with the development of a simulator for approaching human errors in complex operational frameworks (e.g., plant commissioning). The aim is to integrate the quantification capabilities of the so-called ‘first-generation’ human reliability assessment (HRA) methods with a cognitive evaluation of the operator. The simulator allows analysing both error prevention and error recovery. It integrates cognitive human error analysis with standard hazard analysis methods (Hazop and event tree) by means of a ‘semi static approach’. The comparison between the results obtained through the proposed approach and those of a traditional HRA method such as human error assessment and reduction technique, shows the capability of the simulator to provide coherent and accurate analysis.  相似文献

In this paper, we present the results of the analysis of the events related to the commissioning of new nuclear power plants and reported to the IAEA International Reporting System database. These results are extracted from a study performed by the European Clearinghouse on Nuclear Power Plant Operational Feedback about the events related to the construction, the manufacturing and the commissioning of new nuclear power plants.After the initial screening of the database, we have analysed in detail 34 events in order to highlight the lessons learned specific to different components and to raise the general recommendations related to the commissioning.This paper summarizes the main lessons learned and the main recommendations concerning the commissioning-related events. These recommendations concern mainly the time of the testing, the scope of the tests, the documentation of the tests, the test acceptance criteria, the systems reconfiguration after commissioning tests and the management of the temporary devices.  相似文献

The paper deals with risk analysis and management in the realisation of process plants by engineering and contracting companies. Specifically, a new analytical methodology is developed to manage the typical risks of plant commissioning. Following a detailed study of the commissioning process and its criticalities, Hazop is selected as the most suitable approach in forecasting particular risks of this project stage. Starting from the limitations of both Hazop and Human Hazop (the extension of the technique to the field of procedures performed by humans), a new technique (Multilevel Hazop) is suggested which identifies in advance all the criticalities of each commissioning activity and defines actions to reduce risk and optimise dedicated resources. After implementation of Multilevel Hazop in a real commissioning process, a new version of the technique (Two Stage Multilevel Hazop) is proposed. This allows a significant reduction in implementation costs, justifying use of the technique both in innovative or critical cases, where the expected saving from risk reduction is high, and in general contexts.  相似文献
