BACKGROUND: The increase in portal vascular resistance is a significant complication of metastatic disease to the liver or locally advanced cancer, e.g., biliary cancer. PATIENTS AND METHODS: This paper describes the successful palliative treatment of two cancer patients with portal hypertension presenting with the symptoms of tense ascites, mesenteric congestion, and severe variceal bleeding. By creating a stenttract between a hepatic vein and a main branch of the portal vein and/or by placing an extendable stent into the portal vein, the transjugular intrahepatic portosystemic stent-shunt (TIPS) technique was used to decompress the portovascular system. RESULTS: The TIPS-technique offers a new, safe and effective palliation for malignant portal hypertension. In both patients, the symptoms of the portal hypertension disappeared after the procedure. This was accompanied by a significant improvement of the patients performance status allowing an early ambulation. CONCLUSION: Our findings demonstrate the feasibility and effectiveness of the TIPS procedure as a minimal invasive treatment for portal vein decompression in selected tumor patients.  相似文献

So called lethal midline granuloma is of great clinical and theoretical interest. Recent evidence has shown that most lethal midline granulomas are associated with a T-cell phenotype and they are therefore referred to as nasal T-cell lymphomas (NTCL). Immunohistochemical studies, however, have shown peculiar phenotypic features such as expression of natural killer (NK)-cell-related markers and extensive T-cell antigen loss including absence of expression of alpha beta T-cell receptor (TCR). In this study, we reported genotypic and immunohistochemical features in two cases of lethal midline granuloma. The histopathological diagnosis of the biopsy specimens was polymorphic reticulosis/midline malignant reticulosis. Both cases displayed a CD2+, CD3-, CD3 epsilon+, CD4-, CD8-, CD16-, CD56+ phenotype, suggesting that these tumors may be peripheral T-cell lymphomas with extensive loss of T-cell antigens and expression of NK cell antigen (CD56), or, alternatively, NK cell neoplasias. No TCR beta gene rearrangement was detected in these cases. Monoclonal Epstein-Barr virus (EBV) genome was detected in each specimen by Southern blot hybridization. The tumor cells in one of the two cases expressed latent membrane protein (LMP). These findings support the concept that lethal midline granuloma constitutes a distinct group of lymphomas that, in addition to their peculiar clinical features, exhibits the phenotype of extensive loss of T-cell antigens and expression of the NK cell antigen, as well as harbors the EBV. In view of the LMP-transforming potential, these data suggest that EBV may play a role in the pathogenesis of lethal midline granuloma.  相似文献

People can recognize the meaning or gist of a scene from a single glance, and a few recent studies have begun to examine the sorts of information that contribute to scene gist recognition. The authors of the present study used visual masking coupled with image manipulations (randomizing phase while maintaining the Fourier amplitude spectrum; random image structure evolution [RISE]; J. Sadr & P. Sinha, 2004) to explore whether and when unlocalized Fourier amplitude information contributes to gist perception. In 4 experiments, the authors found that differences between scene categories in the Fourier amplitude spectrum are insufficient for gist recognition or gist masking. Whereas the global 1/f spatial frequency amplitude spectra of scenes plays a role in gist masking, local phase information is necessary for gist recognition and for the strongest gist masking. Moreover, the ability to recognize the gist of a target image was influenced by mask recognizability, suggesting that conceptual masking occurs even at the earliest stages of scene processing. Regarding the development of nanoparticles for polymer matrix composites the particle/agglomerate size and particle/agglomerate distribution in the composites, respectively, is often crucial. This is exemplarily shown for, e.g. optical applications with measurements of refractive index and transmittance. Classical blending techniques, where nanoparticles are dispersed in polymers or resins, are compared to a combination of a special gas-phase synthesis method with subsequent in-situ deposition of nanoparticles in high-boiling liquids. The particles/agglomerates were characterized regarding particle size and particle size distribution using transmission electron microscopy and dynamic light scattering. Additionally, important material properties like mechanical properties, relevant for application, or like viscosity, relevant for processing, are determined. It is shown, that with in-situ dispersed nanoparticles synthesized in a microwave plasma process composites with finely dispersed particles/agglomerates are attainable.  相似文献

Increasingly in the recent years, passengers’ services are extended into the ascent and descent flight phases on short distance flights. Trolleys containing the required meal and beverage items are used for these service operations and pushed or pulled along the aisles of the aircraft. Flight attendants reported about increased musculo-skeletal disorders and had been complaining about high physical workload from handling trolleys. In order to ensure acceptable load levels for pushing or pulling operations of trolleys, the physical capabilities of the collective “flight attendants” had been investigated by means of force measurements at maximum voluntary contraction (MVC) level and associated relevant anthropometrical and biometrical data. CEN and ISO standards as well as international and national German methods were used to derive recommended force limits for pushing/pulling operations with respect to the physical capabilities of the target group “flight attendants”. Comparing these recommended limits with the force requirements per shift under various conditions of trolley handlings (inclination of the floor, type and weight of trolley, mode of handling, frequency of operation) showed that especially for higher floor inclinations and trolley weights, flight attendants work (substantially) above recommended limits. It became also apparent that the handling of half-size trolleys is unexpectedly high demanding due to high vertical force components, caused by the unfavorable location of their center of gravity.

Relevance to industry

On short-distance flights, passengers’ services are performed by means of trolleys from the ascent to the descent flight phases. Flight attendants had been complaining about high physical workload. This study offers recommendations on maximum force limits for the handling of trolleys with respect to the muscular capacities and work situations of flight attendants on short-distance flights.  相似文献

The musculoskeletal loads from moving trolleys aboard aircraft were assessed by observation of trolley handling on planes and by physical workload analyses of pushing and pulling of trolleys in a laboratory set up. Trolley handling by a total of 15 female flight attendants was observed on 10 short- and medium-distance flights in different types of aircraft. About 25 selected flight attendants (22 females; 3 males) of five German airlines took part in the laboratory study, which comprised three-dimensional (3D) measurements of posture and hand forces during pushing and pulling of trolleys in a variety of configurations. From the on-flight observations performed, between 150 and 250 trolley movements can be projected for a work shift. The greatest physical workload is to be expected at the beginning of service: The trolleys are fully laden then, and the cabin floor can still be inclined up to 8°, as the aircraft is still climbing, particularly on short-distance flights. The laboratory investigation revealed that the musculoskeletal workload from pushing and pulling depends essentially on the trolley load and on the gradient of the cabin floor. In addition, the degree of stressing depends significantly on the trolley type, mode of handling and personal dexterity. The up/down force component perpendicular to the direction of motion often achieved considerable amplitudes-in some cases equal to or exceeding the force in the direction of movement. The posture analysis demonstrated that pulling forced the flight attendants to adopt ergonomically unfavourable postures such as pronounced flexion of the back, particularly among female subjects. The highest values for flexion of the back occurred while pulling the half-size trolley. The results demonstrate that female flight attendants are likely to overload themselves if they frequently have to move heavily laden trolleys unaided on an inclined cabin floor.

Relevance to industry

On short distance flights, flight attendants have been complaining increasingly of high physical workload from manoeuvring trolleys. On the basis of the presented data airline companies may improve the trolley handling skills of their flight attendances by practical trainings and may ergonomically optimize the general service procedures aboard aircrafts.  相似文献

The NexGen (Sonic) burner is the new burner developed by the Federal Aviation Administration, FAA, to replace old oil burners used for the required fire certification tests on power plant‐related materials, as it provides the capability to control both air and fuel flow rates. During a fire test, the burner is supposed to simulate a certain fire condition, so the flame properties should be robust and repeatable. The NexGen burner can achieve this due to the precise fuel and air controls. However, the current calibration criterion (ISO2685:1998 and AC20‐135) may not be good enough to ensure consistent flame properties. In the presented work, the sensitivity of the burner performance to air and fuel flow rate, as measured by the temperature and heat flux for calibration purposes, was studied. Additionally, the influence of the turbulator and the thermocouple size used for flame calibration was also studied. The impact of varying fuel/air ratio and thermocouple sizes was studied by conducting fire tests on aluminum samples, to show the inadequacies in the current calibration standards.  相似文献
