Definition of the problem This paper deals with the question of adequate methodologies and methods to conduct empirical research and integrate empirical knowledge in the field of biomedical ethics. Arguments It starts with reflections about the main subject matter of ethics, the human being as a bio-psycho-social creature and its morals, and argues that social sciences are the adequate basic empirical science in the field of ethics in general. The next paragraph considers theory of science perspectives of the relationship of ethics, empirical findings, methodologies and theories and examples of the integration of empirical findings into ethical deliberations. Reasons for the tension between social science and ethics are scrutinized which are different in the yet to be defined field of empirical ethics compared to traditional applied ethics. Conclusion How a bio-psycho-social ethics that incorporates these criteria into its own body of work could look like is finally depicted.  相似文献

Definition of the problem

Establishing a good physician–patient relationship based on mutual trust and professional responsibility is a central element in physicians’ professionalism. Recently, however, there has been considerable debate in the field of medical ethics regarding trends toward an economic approach to medicine, which could potentially undermine healthy doctor–patient relationships. Part of this economic approach to medicine involves viewing the patient as an autonomous customer. But what does the customer role mean for vulnerable patients who are restricted in their autonomy and what effect does it have on the physician–patient relationship?

Arguments

In the following article, we pursue the question whether the physicians’ responsibility and the physician–patient relationship change when the (vulnerable) patient attributes the customer role to him-/herself. The study was conducted within the field of palliative care because terminally ill patients represent an especially vulnerable patient population. We base our argumentation in this article on the results of a qualitative interview study that investigated terminally ill patients’ attitudes regarding the topic “the patient as customer” as well as autonomy and decision-making.

Conclusion

The interview results show that autonomy must be understood in relational terms; furthermore, the analysis of the interviews indicates that the physician plays a crucial role in enabling the patient’s autonomy, regardless of whether patients view themselves as a patient or a customer. Subsequently, we deliberate whether the patients’ statements point to potential changes in the doctor–patient relationship and physicians’ responsibility towards patients. Based on these findings, practical implications for the doctor–patient relationship are considered in regard to the “Ethics of Care”, emphasizing the need for an individualized approach to working with patients, especially those with particularly vulnerable conditions.  相似文献

Acceptance, utility, and usability of system designs have become a focal interest in human–computer interaction (HCI) research, yet at present there is a lack detailed understandings of which system design features influence them. The purpose of the study is to examine the effects of five product design features; customization, adaptive behavior, memory load, content density, and speed on user preference through an experimental study by using conjoint analysis. In experimental study, instead of classical conjoint cards, prototypes were generated for products. Besides, desirability and market segments of product prototypes were identified. In line with the results, among the five product design features, speed is the most and customization is the least important features that affect user preference. Contrary to the expectations, customization has a relatively small importance value in this research. Subsequent design features that influence user preference after speed are minimal memory load, adaptive behavior, and content density, respectively. According to findings, interfaces that have high-speed, minimal memory load, adaptive behavior, low content density, and customization features are more preferable than those that do not.  相似文献

Aluminum nitride (AlN) hollow nanofibers were synthesized via plasma‐enhanced atomic layer deposition using sacrificial electrospun polymeric nanofiber templates having different average fiber diameters (~70, ~330, and ~740 nm). Depositions were carried out at 200°C using trimethylaluminum and ammonia precursors. AlN‐coated nanofibers were calcined subsequently at 500°C for 2 h to remove the sacrificial polymeric nanofiber template. SEM studies have shown that there is a critical wall thickness value depending on the template's average fiber diameter for AlN hollow nanofibers to preserve their shapes after the template has been removed by calcination. Best morphologies were observed for AlN hollow nanofibers prepared by depositing 800 cycles (corresponding to ~69 nm) on nanofiber templates having ~330 nm average fiber diameter. TEM images indicated uniform wall thicknesses of ~65 nm along the fiber axes for samples prepared using templates having ~70 and ~330 nm average fiber diameters. Synthesized AlN hollow nanofibers were polycrystalline with a hexagonal crystal structure as determined by high‐resolution TEM and selected area electron diffraction. Chemical compositions of coated and calcined samples were studied using X‐ray photoelectron spectroscopy (XPS). High‐resolution XPS spectra confirmed the presence of AlN.  相似文献

Arm work, by limiting movement of the chest wall and use of the respiratory muscles, may alter breathing pattern and gas exchange sufficiently to interfere with the ability to perform certain tasks. To determine the effects of arm work on breathing pattern during a well-controlled work task, depth of breathing, breathing frequency and end-expiratory lung volume (EELV) were measured at rest and during cycling exercise using an arm and a leg ergometer. Six subjects performed arm work at light, moderate and heavy intensities (30%, 60% and 90% of maximum arm work capacity respectively) and leg work at three intensities where ventilation was matched for that achieved during the arm work. This matching was necessary since the level of ventilation affects the breathing pattern. Subjects breathed on a mouthpiece and tubing that led to automated equipment for the measurement of respiratory variables. Ventilation during arm work was accomplished with a lower depth of breathing, a higher breathing frequency and a decreased EELV compared to leg work. Arm work places increased demands on the ventilatory system, including the muscles of respiration that are also recruited for task performance. The competition for using these muscles for breathing as opposed to a particular work task may result in a compromise in breathing capacity that ultimately may limit the ability to perform tasks requiring sustained heavy use of the arms. These increased demands on the upper body muscles must be considered when evaluating the ability of individuals to perform tasks that involve heavy arm work.  相似文献

Objectives

A new microfabrication method to produce low profile radio frequency (RF) resonant markers on catheter shafts was developed. A semi-active RF resonant marker incorporating a solenoid and a plate capacitor was constructed on the distal shaft of a 5 Fr guiding catheter. The resulting device can be used for interventional cardiovascular MRI procedures.

Materials and methods

Unlike current semi-active device visualization techniques that require rigid and bulky analog circuit components (capacitor and solenoid), we fabricated a low profile RF resonant marker directly on guiding the catheter surface by thin film metal deposition and electroplating processes using a modified physical vapor deposition system.

Results

The increase of the overall device profile thickness caused by the semi-active RF resonant marker (130 µm thick) was lowered by a factor of 4.6 compared with using the thinnest commercial non-magnetic and rigid circuit components (600 µm thick). Moreover, adequate visibility performance of the RF resonant marker in different orientations and overall RF safety were confirmed through in vitro experiments under MRI successfully.

Conclusion

The developed RF resonant marker on a clinical grade 5 Fr guiding catheter will enable several interventional congenital heart disease treatment procedures under MRI.

The forward characteristics of different epitazial n-n+ silicon Schottky barrier diodes have been studied up to high current densities. Modulation has been observed in these experiments, that means an increase in charge carrier density in the series resistance region, i.e. in the epitaxial n-layer. By appropriate analysis of the forward characteristics the carrier density can be determined as a function of the current density. In accordance with the predictions of Scharfetter we find that the modulation increases with the barrier height but decreases with rising donor density. Furthermore it is shown how modulation is affected by recombination centres. The modulation effect is attenuated by irradiation with 1·5 MeV electrons and in a similar manner by doping the epitaxial layer with gold.  相似文献

In this study, bacteriocins from two Lactococcus lactis subsp. lactis isolates from raw milk samples in Turkey designated OC1 and OC2, respectively, were characterized and identified. The activity spectra of the bacteriocins were determined by using different indicator bacteria including Listeria, Bacillus and Staphylococcus spp. Bacteriocins were tested for their sensitivity to different enzymes, heat treatments and pH values. Loss of bacteriocin activities after alpha-amylase treatment suggested that they form aggregates with carbohydrates. Molecular masses of the purified bacteriocins were determined by SDS-PAGE. PCR amplification was carried out with specific primers for the detection of their structural genes. As a result of these studies, the two bacteriocins were characterized as nisin and lacticin 481, respectively. Examination of plasmid contents of the isolates and the results of plasmid curing and conjugation experiments showed that in L. lactis subsp. lactis OC1 strain the 39.7-kb plasmid is responsible for nisin production, lactose fermentation and proteolytic activity, whereas the 16.0-kb plasmid is responsible for lacticin 481 production and lactose fermentation in L. lactis subsp. lactis OC2 strain.  相似文献

Influence of soft segment molecular weight and hard segment content on the morphology, thermomechanical and tensile properties of homologous polyurethaneurea copolymers based on narrow molecular weight poly(propylene oxide)glycol (PPG) oligomers were investigated. A series of polyurethaneureas with hard segment contents of 12–45% by weight and PPG number average molecular weights <Mn> of 2000 to 11,800 g/mol were synthesized and characterized structurally by SAXS and mechanically by DMA and stress strain analysis. Bis(4-isocyanatocyclohexyl)methane and 2-methyl-1,5-diaminopentane were used as the diisocyanate and the chain extender respectively. All copolymers displayed microphase separation by SAXS and DMA. The critical entanglement molecular weight (Me) of PPG is reported to be around 7700 g/mol. Our mechanical results suggest that when copolymers possess similar hard segment contents and are compared to those based on soft segments with number average molecular weights (Mn) greater than Me, they generally displayed higher tensile strengths and particularly lower hysteresis and creep than those having soft segment molecular weights below Me. These results imply that soft segment entanglements in thermoplastic polyurethaneureas may provide a critical contribution to the tensile properties of these copolymers – particularly in the range where the soft segment content is dominant.  相似文献
