Cystic fibrosis (CF) is an inherited disease of epithelial cell ion transport that is associated with pathology in multiple organ systems, including lung, pancreas, and liver. As treatment of the pulmonary manifestations of CF has improved, management of CF liver disease has become increasingly important in adult patients. This report describes an approach for treating CF liver disease by somatic gene transfer. In situ hybridization and immunocytochemistry analysis of rat liver sections indicated that the endogenous CFTR (cystic fibrosis transmembrane conductance regulator) gene is primarily expressed in the intrahepatic biliary epithelial cells. To specifically target recombinant genes to the biliary epithelium in vivo, recombinant adenoviruses expressing lacZ or human CFTR were infused retrograde into the biliary tract through the common bile duct. Conditions were established for achieving recombinant gene expression in virtually all cells of the intrahepatic bile ducts in vivo. Expression persisted in the smaller bile ducts for the duration of the experiment, which was 21 days. These studies suggest that it may be feasible to prevent CF liver disease by genetically reconstituting CFTR expression in the biliary tract, using an approach that is clinically feasible.  相似文献

The purpose of this study was to examine the factors which affect the level of fatigue among patients participating in clinical trials in which this symptom had been assessed with the EORTC QLQ-C30. Data were assembled from 2390 patients in ten clinical trials in which the QLQ-C30 had been used to assess baseline and on-study quality of life. The relationship between the level of fatigue reported by the patients on the fatigue scale of this questionnaire and patient and disease characteristics was assessed in univariate and multivariate cross-sectional analyses. In addition, changes in fatigue scores were compared in a longitudinal analysis among patients on two arms of an anti-emetic trial whose emesis control was markedly different. Baseline fatigue levels differed substantially among patients taking part in the different trials. Factors associated with greater fatigue severity on univariate analysis included: female gender, presence of metastatic disease, and poorer performance status. In addition, on multivariate analyses the oldest patients were found to have less fatigue, as were patients with breast cancer, while patients with ovarian and lung cancer experienced greater fatigue. Patients on the arm of the anti-emetic trial in which emesis was better controlled showed significantly less increase in fatigue after receiving chemotherapy. The fatigue scale of the QLQ-C30 appears to provide a useful approach to assessing this important symptom. The relationships found between fatigue and patient and disease characteristics need further exploration as does the degree to which the QLQ-C30 fully captures this dimension of quality of life.  相似文献

BACKGROUND: The increase in portal vascular resistance is a significant complication of metastatic disease to the liver or locally advanced cancer, e.g., biliary cancer. PATIENTS AND METHODS: This paper describes the successful palliative treatment of two cancer patients with portal hypertension presenting with the symptoms of tense ascites, mesenteric congestion, and severe variceal bleeding. By creating a stenttract between a hepatic vein and a main branch of the portal vein and/or by placing an extendable stent into the portal vein, the transjugular intrahepatic portosystemic stent-shunt (TIPS) technique was used to decompress the portovascular system. RESULTS: The TIPS-technique offers a new, safe and effective palliation for malignant portal hypertension. In both patients, the symptoms of the portal hypertension disappeared after the procedure. This was accompanied by a significant improvement of the patients performance status allowing an early ambulation. CONCLUSION: Our findings demonstrate the feasibility and effectiveness of the TIPS procedure as a minimal invasive treatment for portal vein decompression in selected tumor patients.  相似文献

Sheet metal forming is an inherent part of todays production industry. A major goal is to increase the forming limits of classical deep-drawing processes. One possibility to achieve that is to combine the conventional quasi-static (QS) forming process with electromagnetic high-speed (HS) post-forming. This work focuses on the finite element analysis of such combined forming processes to demonstrate the improvement which can be achieved. For this purpose, a cooperation of different institutions representing different work fields has been established. The material characterization is based on flow curves and forming limit curves for low and high strain rates obtained by novel testing devices. Further experimental investigations have been performed on the process chain of a cross shaped cup, referring to both purely quasi-static and quasi-static combined with electromagnetic forming. While efficient mathematical optimization algorithms support the new viscoplastic ductile damage modelling to find the optimum parameters based on the results of experimental material characterization, the full process chain is studied by means of an electro-magneto-mechanical finite element analysis. The constitutive equations of the material model are integrated in an explicit manner and implemented as a user material subroutine into the commercial finite element package LS-DYNA.  相似文献

The most interesting places can often be the most dangerous ones, and space exploration is no exception. The dynamic surfaces of comets, the turbulent atmospheres of the gas giants, and the hypothesized subsurface ocean of Europa all call for exploration, and are all very risky for on-site robotic explorers. The Casper (continuous activity, scheduling, planning, execution, and replanning) software system provides critical reasoning capabilities to robotic explorers. To show how it works, we first describe Casper's internal workings and then look at the Three Corner Sat and the Autonomous Sciencecraft Constellation missions, highlighting Casper's contributions to each.  相似文献

Filter-driven optimization based on the extended Kalman filter concept is used here for the numerical solution of crack and flaw identification problems in elastodynamics. The mechanical modeling of the studied two-dimensional problem, which includes the effect of unilateral contact along the sides of the crack, is done with the help of the boundary element method. The effect of various dynamical test loads and the applicability of this method for crack and defect identification in disks are investigated. An inverse problem in engineering mechanics is considered where the position and the geometry of three-dimensional, ellipsoidal defects are identified by using measurements of the mechanical response under static loading on the external surface of the structure. The problem is solved by appropriate combination of genetic optimization (GO) and boundary element method (BEM) and following previously published two-dimensional problems. The three-dimensional case presents some additional difficulties. Furthermore, the function of several genetic operators and the effect of the parameters of genetic optimization on the efficiency of the solution has been numerically examined.  相似文献

This paper presents and compares methods for inspecting products using machine vision systems. This research compares the Mahalanobis Taguchi System (MTS) with a method based on principal component transformation and multi-modal overlap methods, and which is called the Principal component Feature overlap Measure (PFM). In an example application, the PFM achieves significantly higher Signal/Noise-ratios (+80 dB) and equal or better classification performance than MTS with a lower number of classification features (-75%).  相似文献

An experimental approach employing temperature and concentration gradients is presented that is suitable for determining a sequence of solvus concentrations in a single experimental cycle. The Al–Mg–Si system is used as an example. Al solid solutions (“(Al)”) with different compositions and Mg2Si precipitates were equilibrated in the temperature range between 350 and 550 °C. The equilibrium compositions of (Al) were measured by energy dispersive X‐ray spectroscopy in the transmission electron microscope. The coupled temperature‐concentration values represent a path on the ternary solvus surface. Calphad calculations using different thermodynamic data sets were carried out to relate the experimental results to phase diagram evaluations. The measured solvus path agrees well with solvus data calculated with most recent thermodynamic parameter sets. The experimental approach can be applied to multicomponent alloys irrespective of their number of components with the same experimental effort. The efficiently generated solubility data are suitable to support phase diagram evaluations of multicomponent systems by the Calphad method.  相似文献
