BACKGROUND AND AIMS OF THE STUDY: Surgical treatment of functional tricuspid regurgitation associated with advanced valvular disease remains controversial, mainly due to the difficulty in choosing between valve replacement or reconstructive surgery. Failure to correct tricuspid regurgitation during valvular surgery carries a poor prognosis, as reoperation may represent a significant risk to the patient. Thus correct tricuspid valve surgery is vital to achieve improved early and long-term clinical results. METHODS: A total of 142 patients underwent concomitant tricuspid annuloplasty with mitral valve replacement and their clinical outcome was assessed. RESULTS: Overall hospital mortality rate was 11.3%. Seven patients died during follow up. The overall actuarial survival rate for 10 years was 74.1 +/- 14.2%. Postoperatively, 75.8% of the patients had no residual tricuspid insufficiency, while 24.2% had first- or second-degree tricuspid insufficiency and were treated medically. All patients were in NHYA functional class I or II postoperatively. CONCLUSION: Clinically, it is important to assess the severity of functional tricuspid insufficiency before and/or during the operation. Bicuspidalization annuloplasty for functional tricuspid insufficiency has provided good results, with a low incidence of reoperation and significant clinical improvement. Diffusion bonds have been produced between sheets of an Al-Li-Cu-Mg-Zr alloy using aluminium-4% copper vapour deposited metallic interlayers. Microstructural changes occurred both in the parent alloy and in the bond interface after diffusion bonding cycles and post-bonding heat treatments were analysed. Different metallographic techniques (light microscopy, scanning and transmission electron microscopy) have been used. Diffusion bonding trials were carried out using the same alloy (AA8090), both in non-superplastic (T6) and superplastic conditions. We present a structural study of two Nd3+/MgO codoped LiNbO3 waveguides fabricated by two different methods: ion-implantation and proton-exchange. Rutherford backscattering spectrometry in combination with channelling was used to study the influence of the waveguide fabrication method on the possible modification of the LiNbO3 crystal structure within the waveguiding layer. The results show that the ion-implanted waveguide mainly maintains the same properties as the virgin crystal, apart from a deterioration of a thin surface layer, whilst the proton-exchanged waveguide exhibits a quite different behaviour, although its surface is less damaged. 相似文献

The residual deformation produced by Vickers microindentations in poly(3,3-dimethyl oxetane) (PDMO) was analysed by Raman microspectroscopy. Microstructural variations were observed as the Raman spectra were recorded at different positions along and out of the microindentation. A gradual distribution of the monoclinic and orthorhombic structures arises as the spectra were scanned from the centre to the edge of the impression area. This crystalline variation in PDMO, which reflects the degree of stress present on the surface of the sample, was utilized to obtain information on the distribution of pressure in and around the microindentation. In addition to the clear variation of pressure inside the permanently deformed region, no crystalline transformation was noted out of the impression area. 1. We investigated the effect of exercise on plasma adrenomedullin, atrial natriuretic peptide (ANP) and brain natriuretic peptide (BNP) concentrations and studied the relationship between these peptides and haemodynamic parameters in nine patients with old myocardial infarction (MI) and in eight normal subjects. 2. The exercise protocol consisted of two fixed work loads (40 and 80 W) for 4 min each and venous blood samples were taken at rest, during each exercise stage and after exercise while monitoring the mean arterial pressure (MAP) and heart rate (HR). In MI, pulmonary arterial pressure (PAP), pulmonary capillary wedge pressure (PCWP), left ventricular end-diastolic pressure (LVEDP) and cardiac output (CO) were measured throughout exercise. 3. Adrenomedullin levels did not significantly increase with exercise. Adrenomedullin levels correlated with PAP and PCWP at rest (P < 0.05). Atrial natriuretic peptide levels correlated with PAP, PCWP and LVEDP throughout exercise (P < 0.05) but, on multiple regression analysis, PCWP correlated only with ANP (P < 0.01). Brain natriuretic peptide levels correlated with LVEDP throughout exercise (P < 0.01) and its increment correlated closely with basal BNP levels at rest (P < 0.01). 4. These results suggest that adrenomedullin does not respond to the acute haemodynamic changes of exercise, whereas ANP responds to it and PCWP is the major stimulus factor. Brain natriuretic peptide responds to exercise in proportion to the basal synthesis of BNP in patients with left ventricular dysfunction and LVEDP may play a role in increasing BNP during exercise. 相似文献

Sources of nutrition information of Chilean schoolers, metropolitan region, Chile, survey 1986-1987.
The purpose of the present study was to determine the relative importance of different sources of nutrition information used by Chilean schoolers to learn the basics of nutrition. In this respect, a representative sample of 4,509 elementary and high-school children was randomly selected and stratified according to grade, sex, type of school and geographic area. Schoolers were individually interviewed in order to collect data related with socioeconomic status (SES) and sociocultural conditions, as well as to learn as to what they considered to be the most important sources of nutrition information influencing their nutritional learning, gathering the necessary data through a questionnaire especially designed for this purpose. Results indicated that 87.8% schoolers mentioned their family (mainly the mother), as the most important source of nutrition information; 10.7% said the school teacher, and 1.5%, mass media (0.8% television, 0.5% books, 0.1% magazines and 0.1% radio) without significant differences by sex, age and geographic area. The role attributed to the family (mother) was important in all SES categories, but proved to be significantly higher in the high SES group (91.9%) than in the low SES group (84.6%). The role attributed to the school teacher, in contrast, was lower (6.7%) in the high SES group than in the low SES group (13.8%) (p less than 0.001). In relation to mass media, the influence attributed to books and magazines was high in the high SES group, but television and radio were more important in the low SES group. On the other hand, significant differences found by type of school were related to SES. These findings could be useful in the establishment of food and nutrition policies, as well as in educational planning focused on the Chilean school population, considering that few studies have been carried out along these lines. 相似文献
