Relapse of the maxilla was studied retrospectively in a group of 61 patients with previously repaired cleft lip and palate who had undergone Le Fort I osteotomy. The extent of surgical move and relapse over a mean period of 28 months was measured with reference to the cranial base, and found to be similar to those in other studies. The use of direct bone plates for fixation of the maxilla is associated with a significantly more stable postoperative result.  相似文献

To study the transition of the lubricated wear of 0.53% C steel in the steady state, wear tests were carried out by rubbing the annular surfaces of two cylindrical test pieces in machine oil with no additives. The ratio of mating areas was varied to approach actual contact conditions. Three regions of variation in the coefficient of friction with contact load were determined. Fatigue wear, characterized by a friction coefficient μ ≈ 0.05 and a specific wear rate ωs ≈ 0.005 × 10?6 mm3 N?1 m?1, occurs in the first region.A transition from fatigue wear to adhesive wear, with μ ≈ 0.05?0.12 and ωs ≈ (0.005?0.05) × 10?6 mm3 N?1 m?1, takes place gradually within a specific load range. Finally, adhesive wear predominates above the load level that marks the end of the transition. The same behaviour was analysed through stepwise loading tests. The onset of transition and seizure occurs at constant mean surface temperatures. However, the end of transition is also affected by factors other than temperature. The results are compared with the transitions reported by the International Research Group on Wear of Engineering Materials of the Organization for Economic Cooperation and Development.  相似文献

BACKGROUND: Liddle's syndrome is an autosomal inheritable disorder that causes hypertension due to excess function of sodium channel. OBJECTIVE: To analyze the DNA sequence of the amiloride-sensitive epithelial sodium channel (ENaC) in three patients who had low-renin hypertension with hypokalemia. The patients included a 24-year-old woman and her 20-year-old brother whose mother was hypertensive. The third patient was a 15-year-old girl with no family history of hypertension. METHODS: The DNA sequence of the ENaC was analyzed as follows. Venous blood samples were collected from the patients and total genomic DNA was prepared by standard methods. Specific primers were used for direct polymerase chain reaction; one set of primers for amplifying the C terminus (codon 523-638) of the , subunit of ENaC, and two sets of primers for amplifying the C terminus (codons 525-587 and 568-650) of the y subunit of ENaC. Polymerase chain reaction products were purified and subjected to direct DNA sequence analysis. RESULTS: Direct sequence analysis demonstrated the presence of a single-base substitution in one segment of the 0 subunit of ENaC, a C-T transition that changed the encoded Pro (CCC) at codon 616 to Ser (TCC) in the siblings (cases 1 and 2). In case 3, we found a missense mutation of Pro (CCC) to Leu (CTC) at codon 616. Case 3 is considered to be sporadic, since DNA sequencing of the PY motif of her parents gave normal results. CONCLUSIONS: The DNA sequences of the ENaC in three patients with Liddle's syndrome were analyzed. In one family case, we found a new missense mutation of Pro (CCC) to Ser (TCC) at codon 616 in the 0 subunit of ENaC. A genetic analysis of the amiloride-sensitive epithelial sodium channel is recommended in assessing patients with low-renin, salt-sensitive hypertension whose blood pressure is not responsive to spironolactone treatment.  相似文献

The present study measures entrance surface doses of radiation administered to patients during various fluoroscopic procedures using a dose-area product meter as well as the duration of each procedure. A conversion factor for entrance skin dose to patients was calculated. The average dose to patient during the insertion of intravenous hyperalimentation was 10.2 (maximum, 74.0) mGy, during a barium meal, 58.4 (maximum, 184.0) mGy, for endoscopic retrograde cholangio-pancreatography (ERCP), 97.3 (maximum, 376.0) mGy and for a barium enema, 86.1 (maximum, 271.0) mGy. Doses tended to increase in the abdominal domain and when patients undergo not only diagnostic evaluation but also treatment requiring procedures such as drainage. The management of radiation doses determined using a dose-area product meter is very important and that of individual patients is enabled using the Radiation Information System (RIS).  相似文献

Preliminary experiments were carried out for the DNA detection targeting a complementary couple of single-stranded DNAs originating from Nitrobacter winogradskyi. The target DNAs were detected successfully with a quartz crystal unit bearing probe DNAs, based on the oscillation frequency decrease due to hybridization of the probe and target DNAs.  相似文献

OBJECTIVE: We have recently found that left antero-axillary thoracotomy provides an ideal view of aortic arch and makes the direct cannulation to superior vena cava possible for retrograde cerebral perfusion during circulatory arrest. METHOD: Twelve patients with distal aortic arch aneurysm or aortic dissection underwent the repair of aortic arch through this approach. Mean duration of retrograde cerebral perfusion was 41 min. RESULTS: Two hospital deaths occurred due to respiratory failure and stroke. The remaining patients survived without any neurological deficits. CONCLUSION: Antero-axillary thoracotomy may be an ideal approach which combines the advantages of median sternotomy and postero-lateral thoracotomy.  相似文献

In the present study, we investigated the effect of ambient pressure on [3H]-thymidine incorporation and on the production of matrix metalloproteinase 1 (tissue collagenase/proMMP-1) using human aortic endothelial cells immortalized with simian virus 40 (SE-1). Incubation of cells at ambient pressures of 50 and 100 mmHg for 24 h slightly increased [3H]-thymidine incorporation when directly compared with normal culture conditions. The amount of [3H]-thymidine incorporated in SE-1 reached a maximum at 150 mmHg, while a further increase in pressure to 200 mmHg decreased incorporation. The same ambient pressure slightly stimulated human aortic intimal smooth muscle cells (SMC) to increase [3H]-thymidine incorporation but not medial SMC. Immunoblot analysis also showed that ambient pressure, ranging from 50 to 200 mmHg, like 12-O-tetradecanoyl-phorbol-13-acetate stimulated SE-1 to produce proMMP-1, an effect not seen with either intimal or medial SMC. The amount of proMMP-1 produced also reached a maximum level at 150 mmHg. We postulate that human endothelial cells are ambient pressure sensitive and that relatively lower ambient pressures play an important role in the growth of endothelial cells, while higher pressures injure endothelial cells, resulting in the initiation of atherosclerosis. This cell line may prove useful in the investigation of both the physiological and pathological roles of blood pressure on endothelial cell function.  相似文献

In recent years, awareness of environmental problems is growing, and the price of electric power purchased by electric companies has been expensive for power plants utilizing natural energy. Thus, the introduction of wind power generation is being promoted in Japan. Generally, squirrel‐cage induction machines are widely used as generators in wind power generation systems because of their small size, light weight, and low cost. However, the induction machines do not have a source of excitation. Thus, inrush currents and instantaneous voltage drops occur when the generator is directly connected to a power grid. To reduce the inrush currents, an AC power regulator is used. Wind power generators are frequently connected to and disconnected from the power grid. However, when the inrush currents are reduced, harmonic currents are caused by phase control of the AC power regulator. And the phase control of the AC power regulator cannot control the power factor. Therefore, we propose the use of the AC power regulator to compensate for the harmonic currents and reactive power in a wind power generation system, and demonstrate the validity of this system by simulated and experimental results. © 2005 Wiley Periodicals, Inc.
