We present the case of a patient in whom hypotension, sudden cardiopulmonary arrest, and coma developed after a massive dose of a seemingly harmless cathartic agent. The diagnosis of hypermagnesemia was made 9 hours after the patient's admission, when the serum magnesium concentration was 21.7 mg/dL (8.9 mmol/L). The patient's condition improved with IV calcium, saline solution infusion, and cardiorespiratory support. The elimination half-life of magnesium in this case was 27.7 hours. Few cases have been reported in which patients have survived with serum levels greater than 18 mg/dL (7.4 mmol/L). This case provides evidence that hypermagnesemia may occur in patients with normal kidney function. The diagnosis of hypermagnesemia should be considered in patients who present with symptoms of hyporeflexia, lethargy, refractory hypotension, shock, prolonged QT interval, respiratory depression, or cardiac arrest. 相似文献

The constituents in the fruit of Anthriscus sylvestris Hoffm. were investigated, and four lignans [deoxypodophyllotoxin, morelensin, (-)-deoxypodorhizone, and (-)-hinokinin], one phenylpropanoid [1-(3',4'-dimethoxyphenyl)-1 xi-hydroxy-2-propene], two phenylpropanoid esters [3',4'-dimethoxycinnamyl (Z)-2-angeloyloxymethyl-2-butenoate and 3',4'-dimethoxycinnamyl (Z)-2-tigloyloxymethyl-2-butenoate], and one polyacetylenic compound (falcarindiol) were isolated. Physical optics (PO) is an approximation method for high‐frequency scattering and diffraction problems. But PO fields are inaccurate in the shadow region where the source is screened by the scatter. Two key factors of the error for PO are (i) edge diffraction coefficients and (ii) existence of fictitious penetrating rays. The correction with respect to the former has been extensively investigated by many authors using various ray techniques such as GTD, UTD, and UAT. On the other hand, the latter was identified recently by the authors. This paper proposes novel PO‐based calculation procedures termed PO‐AF and PTD‐AF, where PO and Aperture Field Integration Method (AFIM) are merged with the help of special elementary diffraction coefficients. These can uniformly cover the whole angular region and only the error factor (ii) is removed in PO‐AF while both (i) and (ii) are removed in PTD‐AF. The theoretical backgrounds of PO currents are discussed in terms of field equivalence theorem and visualization of EM waves. Next, new methods are proposed and their validity is numerically demonstrated for 2D scattering problems of a strip, a corner reflector, and a cylinder of circular arc. © 2004 Wiley Periodicals, Inc. Heterocyclic amines (HCAs), formed during the cooking of meats and fish, are thought to be the genotoxic carcinogens associated with important types of human cancer in meat-eating populations, such as cancer of the breast, colon or pancreas. We studied the effect of black or green tea, and of the tea polyphenols theaflavine gallate (TFG, black tea) and epigallocatechin gallate (EGCG, green tea) on the formation of typical HCAs, 2-amino-3,8-dimethylimidazo[4,5-f]quinoxaline (MeIQx), and 2-amino-1-methyl-6-phenylimidazo[4,5-b]pyridine (PhIP), using the model in vitro systems of J?gerstad. Green tea and black tea and solutions of TFG and EGCG lower the formation of PhIP by 62-85% during 1 h heating at 160 degrees C of 1 mmol creatinine, 1 mmol phenylalanine and 0.5 mmol glucose in 3.3 ml diethylene glycol-water (10:1), where the inhibitors replaced 0.3 ml of the water. The production of MeIQx and related HCAs, in the same system but with 1 mmol glycine instead of phenylalanine was likewise reduced, determined by amounts of mutagens formed. In the latter systems, the teas were not, or less effective, but the polyphenols were inhibitory. Thus, the tea products represent another approach to lower the formation of HCAs. 相似文献

Beneficial effects of dietary phytic acid (myo-inositol hexaphosphate; IP6) have often been explained by its strong iron ion-chelating ability, which possibly suppresses iron ion-induced oxidative
damage in the gastrointestinal tract. Because phytic acid is hydrolyzed during digestion, this work aimed to know whether
its hydrolysis products (IP2′ IP3′, IP4′ and IP5) could still prevent iron ion-induced lipid peroxidation. Studies using liposomal membranes demonstrated that hydrolysis
products containing three or more phosphate groups are able to inhibit iron ion-induced lipid peroxidation although their
effectiveness decreased with dephosphorylation. Similarly, they also prevented iron ion-induced decomposition of phosphatidylcholine
hydroperoxide. These results demonstrate that intermediate products of phytic acid hydrolysis still possess iron ion-chelating
ability, and thus they can probably prevent iron ion-induced lipid peroxidation in biological systems. 相似文献

Production of MAG by a lipase-catalyzed reaction is known to be effective at low temperature. This phenomenon can be explained
by assuming that synthesized MAG are excluded from the reaction system because MAG, which have low m.p., are solidified at
low temperatures. Consequently, MAG are efficiently accumulated and do not serve as the precursor of DAG. If this hypothesis
is correct, the critical temperature for MAG production, defined as the highest temperature at which DAG synthesis is repressed,
should depend on the m.p. of the MAG. Esterification of FFA with glycerol using Candida rugosa, Rhizopus oryzae, and Penicillium camembertii lipases produced MAG efficiently at low temperatures. However, Candida lipase showed very low esterification activity at high temperatures (>20°C), and Rhizopus lipase produced not only MAG but also DAG even at low temperatures. Meanwhile, P. camembertii lipase catalyzed synthesis of MAG only from FFA and glycerol at low temperatures, although the enzyme catalyzed synthesis
of DAG from MAG in addition to synthesis of MAG at high temperatures. We thus studied the effect of temperature on esterification
of C10−C18 FFA with glycerol using Penicillium lipase as a catalyst and determined the critical temperatures for production of MAG. The critical temperature for production
of each MAG showed a linear correlation with m.p. of the MAG, which supported the hypothesis. In addition, because the m.p.
of MAG are estimated from that of the constituent FA, the optimal temperature for production of MAG can be predicted from

A commercial product of CLA contains almost equal amounts of cis-9,trans-11 (c9,t11)-CLA and trans-10,cis-12 (t10,c12)-CLA. We attempted to enrich the two isomers by a two-step selective esterification using Candida rugosa lipase that acted on c9,t11-CLA more strongly than on t10,c12-CLA. An FFA mixture containing CLA isomers was esterified with an equimolar amount of lauryl alcohol in a mixture of 20%
water and the lipase. When the esterification of total FA reached 50%, two isomers were fractionated in a good yield: t10,c12-CLA was enriched in FFA, and c9,t11-CLA was recovered in lauryl esters. The FFA were esterified again to enrich t10,c12-CLA. At 27.3% esterification of total FA, the t10,c12-CLA content in FFA increased to 64.8 wt% with 89.3% recovery: The ratio of the content of t10,c12-CLA to that of two isomers was 95.9%. Lauryl esters obtained by the single esterification were employed for enrichment
of c9,t11-CLA. After the esters were hydrolyzed, the resulting FFA were esterified again with lauryl alcohol. At 62.0% esterification
of total FA, the c9,t11-CLA content in lauryl esters increased to 73.3 wt% with 79.4% recovery: The ratio of the content of c9,t11-CLA to that of two isomers was 95.6%. In a 600-g-scale purification, molecular distillation was effective in separating
the reaction mixture into lauryl alcohol, FFA, and lauryl ester fractions. 相似文献
